laboratory of Nobel laureate Baruch S Blumberg and his colleagues.
The Hepatitis B virus (HBV) is one of the ten most common deadly
infectious diseases and is responsible for 1.1 million deaths a year
worldwide.

HEPATITIS VIRUSES

Research in his laboratory resulted in the discovery of HBV and the


invention of the vaccine which protects one against it. The research
began as an apparently esoteric study of human biochemical and
immunologic variation. This required field-work in Africa, the Arctic,
the Pacific, the Americas, and in many other locations and populations.
The overall goal was to identify inherited biological differences which
were related to differing responses to disease-causing agents. The virus
was discovered using the blood of an infected person who had developed
the antibody, to detect the virus present in another infected person
who had become a carrier of the virus. Screening of blood donors
led to the near-elimination of post-transfusion hepatitis B.

Hepatitis A and B viruses have infected nearly half


the current world population, and as many as 500
million people are still infected with the hepatitis
B or C virus today. Hepatitis B vaccination is
effective but not universally adopted and no vaccine
is available against Hepatitis C. Treatment is
prohibitively expensive for areas of high endemicity
or prevalence and not universally effective.
This important and timely book covers recent
advances in understanding the molecular biology
of hepatitis viruses. The advances have contributed
new insights into the molecular mechanisms involved in replication of genetic
information and in gene expression, and have translated into diagnostics,
prevention and development of antiviral drugs.

There are now national HBV vaccination programs in more than 70


countries. During the past decade these programs have strikingly reduced
the prevalence of HBV in many countries and there has been a
significant drop in the incidence of cancer of the liver in the vaccinated
cohorts. The HBV vaccination program is now, after smoking cessation,
the most widely used cancer prevention program in the world.

With rapid advances in medical technology and
progress in medicine during the last 27 years,
severe disability or sustained neck-shoulder-arm
pain secondary to cervical spondylosis can be
detected much earlier. Excellent or good results
of surgery can be achieved compared to the past,
due mainly to advances in diagnostic imaging and
surgical procedure.

This volume of tissue banking is the second of a


series extending the interdisciplinary field. All of
the authors of this book have been major
contributors of scientific and medical publications
in their respective specialities. There are articles
covering blood transfusion; bone marrow and stem
cell transplantation; immunology and the role of
immunology in musculoskeletal transplantation,
various aspects of cryopreservation of organs,
cartilage and tendons; organ perfusion preservation;
the history and long term follow-up of osteochondral
allografts, both frozen and fresh; neurosurgical applications; cardiovascular
applications; the use of skin and newer biological membranes for wound
coverage.

Through research into cervical spondylotic


myelopathy, particularly the conditions combined
with a narrow spinal canal, orthopaedic surgeons
in Japan have revealed the key mechanism involved in myelopathy and the
specific manifestation leading to early recognition of the affliction, the myelopathy
hand, and have invented a canal-expansive laminoplasty. Extensive
decompression of the impinged spinal cord with remarkable ill effects such
as instability or adhesion was accomplished for the first time. Further study
has been ongoing to clarify the pathomechanism of Spondylosis through
biomechanical and animal model studies.

The book is of particular importance, in that it has been written in memory


of Kenneth W Sell, MD, PhD, a pioneer and promulgator of tissue banking
and transplantation. Many of the physicians, surgeons and scientists who trained
under his leadership have gone on to distinguish themselves in the various
aspects of transplantation. Many of them have contributed to this book,
exemplifying the diversity of Dr Sells contributions to tissue banking and
transplantation.

This volume consists of contributions by experts worldwide on the recent


advances in the study of cervical spondylosis and will serve as a valuable
reference for all researchers, surgeons and postgraduate students.

Microsurgery is an important surgical technique that is applicable to many fields


of surgery in general. The contributions given by surgeons in China and other
Chinese speaking surgeons in the area of microsurgery have been important and
plentiful. This volume represents the highlights of a seminar held in 1994 in
Hongkong. Participants included prominent Chinese speaking clinicians active
in the field of microsurgery. The purpose of the seminar was to sort out the
current concepts and the state-of-the-art of this surgical field.

This book is a monograph about brachial plexus palsy. Severe brachial plexus
injuries are often caused by traffic accidents, especially motorcycle accidents.
The intercostal nerve transfer to the musculocutaneous nerve is a standard
treatment of choice in Japan to reconstruct elbow flexion for brachial plexus
palsy with root avulsion. The functional conversion of the nerve transfer
is described in detail, using electrophysiological and histochemical studies.
This book focuses on birth-related brachial plexus palsy as well as the
treatment of brachial plexus injuries in adults.

While joint replacements at the lower extremity,
particularly at the hip and knee, are performed on
a daily basis in most departments of Orthopaedics,
with great clinical success and long longetivity of
the implant, the indications are much less frequent
at the upper limb. Basic research follows the same
tendency: while there are numerous basic or clinical
research projects devoted to the hip or knee, very
scarce reports are published on upper extremity
joint replacements.

The aims of publishing this volume are to promote


the exchange of ideas, and to forster collaborative investigations among clinicians,
manufacturers, engineers and other basic scientists involved with the problems
of replacement arthroplasty in upper extremity joints, from the finger to the

of time. His work done in the field of aeroelasticity predates his
biomechanics work. The applications of some general ideas of aeroelasticity are broader than

biology.

Congenital malformations of the heart are often dismissed as a form of complex


heart disease, which is too difficult to understand, and is best referred to
the specialists. The authors of this handbook, however, aim to dispel this
myth. The advent of cross-sectional and, more recently, three-dimensional
echocardiography, enables the structural malformations to be visualised virtually
non-invasively. Without a thorough understanding of the arrangement of cardiac
structures, interpretation of these images can be very frustrating, not to mention
having to cope with difficult terminology. Contrary to popular belief, however,
the reader does not require a knowledge of cardiac embryology in order to
understand the morphology of a malformed heart.
This book takes the reader through the subject in a straightforward fashion,
beginning with recognition of the normal cardiac chambers, progressing through
the process of analysing the layout of the chambers in a sequential way, and
then dealing with the more common cardiac defects in turn. Each chapter
deals with the anatomical arrangement illustrated with diagrams and
complemented with echocardiographic images of the most important crosssections. The text is succinct, and is accompanied by numerous diagrams and
cross-sectional echocardiographic images of the highest quality. The most
common defects are described chapter by chapter, emphasising the salient
anatomical features.

congenitally malformed hearts. In each video we demonstrate the anatomy
as seen in echocardiographic cuts, relating to the three-dimensional structure
of the heart and then show the echocardiographic images themselves,

This book describes non-conventional


methods of control of human extremities,
emphasizing the fact that conventional
approaches used in robotics are limited when
used in humans for restoration of reaching
and grasping (goal-oriented movements), and
standing and locomotion (cyclic movements).
The use of artificial neural networks, inductive
learning, skill-based expert systems and
finite-state representation of movements is the
base of this non-conventional control theory.
A specific number of realized applications are included in the book to
illustrate how these computer techniques can improve the function of
assistive systems in physically challenged humans. The theory presented
is applicable to the control of robots and industrial manipulators.

Since the industrial revolution and the work of Rowntree,


Booth and other social investigators, attention has been
drawn to poor social conditions and their significant impact

groups. In the late 1970s, there was a groundswell of public

practitioners services in deprived areas and this has continued.

reading textbooks is still one of the best, in
spite of the great advances made in information technology. This is the reason why
it was considered worthwhile to summarize
the first ICOH Seminar Course into a book.
The present book is intended both for selfstudies and as a textbook to be used at
national postgraduate training courses.
Without pretending to cover all fields of
occupational health, the book focuses on
prevention of some of the most obvious
risks, such as pesticide poisoning and work-related cancer. It also
describes the function of different types of occupational health care
systems, and it discusses how to identify needs and to evaluate the
quality of the health care activities. The interconnection between
occupational health care and primary health care systems is described,

and the problems of small-scale industries are highlighted. Also, office


work is discussed because of the new types of problems introduced
by information technology and computers.
This book should be regarded as a complement to, not a substitute
for, more conventional textbooks on occupational health. Being an
organization whose aim is to foster occupational health in the whole
world, ICOH hopes that this book will become well distributed and
well read by occupational health professionals in all industrialising
countries.

Traditionally, cleft lip repair has been carried out
around 10 to 12 weeks of age. There have been
surgeons who have done surgery in newborn but
there has been no series where cleft lip repair has
been carried out in the first 48 hours as a routine
procedure over a long period with long term results.
The authors unit has performed this operation
routinely for the past 25 years and collected data
on the speech, hearing, growth and appearance
of the children, who are now in their 20s. The
main argument against this procedure was the
anaesthetic risk in performing surgery so early in life. Having analysed all the
arguments for and against, the author provides a detailed analysis of the work
in this book. As far as is known, this is the only book to analyse the longterm results of surgery in newborn. A number of centres around the world
are contemplating or performing sugery in newborn and this would be a very
good background for emphasising and promoting the operation.

Myelopeptides are a new class of endogenous bioregulatory peptides. Each has its own
definite immunoregulatory activity and differs in their final effects and mechanisms of action.
Myelopeptides act as immunocorrectors and thus prevent the development of immunodeficiency
states. They have an antitumor effect which contributes to the retention of active immune
defense in the tumor-bearing body. The ability of myelopeptides to induce terminal cell
differentiation in hemopoietic disorders (in particular, in leukemia) is demonstrated. The
protective effect of a single myelopeptide in bacterial contagion of animals, which is due
to the stimulation of macrophage phagocytosis, is established. In this book, the concept
of the general biological value of cytokines, including peptide endogenous regulators, is
discussed. Myelopeptides are promising in the development of a new generation of medicinal
preparations because of their endogenous origin, low molecular weight, directed action and
absence of side effects.

This is the first in a series dedicated to the understanding of


tropical diseases. This volume examines the basic biology and
clinical expression of lymphatic-dwelling filarial parasites,
parasites affecting over 90 million people in the tropical and
subtropical regions of the world. Although it is responsible for
the disfiguring elephantiasis, infection with this parasite leads
to a spectrum of clinical manifestations. The underlying
pathogenetic mechanisms of infection, its epidemiology, diagnosis,
treatment and prevention of these often devastating vectorborne diseases will be the focus of this book.

an introduction to the theory of friction is given.

fracture mechanical properties of powders are

Traditional folk medicine practices in China, Hong

a folk medicine may reflect cultural or regional
differences, a detailed collation of the folk
knowledge of traditional medicine can help
to identify common applications derived from
different empirical knowledge as well as
variations in appreciation of the value of the same
source in different cultural settings.

The Finnish doctor Kari Cantell is one of the


scientists to whom the development of the drugs
called interferons can be attributed. Interferons
have achieved an important place in the
treatment of cancer, viral infections and multiple
sclerosis. In the 1960s Cantell and his coworkers
developed a method for the preparation of
interferon in white blood cells. During those
years, most of the global production of interferon
took place in Finland and the vast majority of
the clinical studies in the world employed Finnish
interferon.

This volume is a compilation of the invited and contributed papers


presented at the 11th International Symposium on Ceramics in Medicine.

and American volumes of The Journal of Hand Surgery, and that these three journals complement each other.

AIMS & SCOPE
Journal of Musculoskeletal Research is an international interdisciplinary journal aimed at publishing up-to-date contributions
on clinical and basic research in the musculoskeletal system. Research into the musculoskeletal system has grown
tremendously in the past few decades. Meanwhile, several outstanding journals in the field have appeared, but with
a different emphasis and objective. With the growing number of studies in the field, the review process has become
increasingly longer. We hope this journal will serve as a new forum for both scientists and clinicians to share their
ideas and the results of their studies.
Journal of Musculoskeletal Research explores all facets of musculoskeletal disorders, with emphasis on both the clinical
and basic aspects of study. It covers the fields of orthopedics, neurology, rheumatology and rehabilitation. The method
of study used can be clinical studies, molecular biology, biomechanics or biomaterial research. Published on a quarterly
basis, the journal publishes review articles, original articles, technical notes and case reports.
