HIDING AT THE VENT OF THE ROOF. WE RESTRAINED THE SUSPECT AND READ
HIS CONSTITUTIONAL RIGHT AND ARRESTED HIM.
6. WE BROUGHT THE SUSPECT TO DULAG POLICE STATION TO BLOTTER
THE INCIDENT. AND SUBSEQUENTLY BROUGHT HIM TO THE HOSPITAL FOR
MEDICAL EXAMINATION AFTER THAT, WE TURN OVER THE SUSPECT TO THE
INVESTIGATOR FOR PROPER DISPOSITION.

I. AUTHORITY
1. Reference: Radio message cite number LNIDMS 0105-20-2014 directing this
police station to conduct in depth investigation and submission of IR re-subject as
above.
2. Inherent function of investigation.

II. MATTERS TO BE INVESTIGATED/FACTS OF THE CASE

3. On April 23, 2014 at about 12:30 am at Brgy.San Jose Dulag, Leyte,
Edilbertos family returned from an out-of -town to attend a family gathering in Quezon
province. Upon arrival, they noticed that the front door knob was broken. They
suspected that there was something wrong, immediately they called a police and
reported the incident that there might be a culprit inside their house. The police
immediately responded and cautiously entered the house. They found out that one of
the suspect was hiding at the vent of the roof. The police officers restrained the suspect
and arrested.
4. Taking an immediate action two(2) police officers namely: PO1 BERNARDO B.
PALATINO JR. and PO1 KIM SAPE while we are conducting a patrol, we received a call
coming from our station that there are suspected persons inside the house of Edilbertos
family. We immediately responded. Upon arrival we cautiously entered the house and

found out that one of the suspect was hiding at the vent of the roof, we restrained the
suspect and his constitutional right. We conducted bodily search and ask for some
identification. Lastly, we brought the suspect together with the victim to the police station
to put record in the case police blotter and for the preparation of the case.
5. Further investigation was conducted by SPO2 JHON BICO. According to him
the suspect is named, Jomar Estaco 29 yrs old, single and a residence of Brgy. San
Rafael Dulag, Leyte.

III. INVESTIGATIONS/FINDINGS
6. The responding policemen personally witnessed the said incident and they
immediately arrested the suspect. After, conducting the bodily search they did not
recovered any weapon from the suspect.

IV. RECOMMENDATION
7. In view of the forgoing, undersigned further request/recommends that an
appropriate award be given to the following PNP members, to include the undersigned
for their remarkable degree of efficiency demonstrated, sense of responsibility,
professionalism and team work that made possible the early solution of the crime of
robbery (AKYAT BAHAY) and the eventual filling of the case for inquest proceeding
against the suspect with the provincial prosecution office, to wit:

investigation and who named the suspect, Jomar Estaco 29 years old, single and a
residence of Brgy. San Rafael Dulag, Leyte.
That I hereby state, that in answering the question asked from me as appearing
below, I was fully conscious that I did so under oath and that I may criminal liability for
false testimony or perjury;

Answer: I am here to file a case of robbery against Jomar Estaco.
