Ensuring that all workers are given orientation/briefing or induction prior
to deployment to the site;
Establishing a site safety committee to act as policy making body of the
project om all issues pertaning to safety and health;
Ensure the conduct of daily toolbox meeting;

Acts without delay in addressing all hazards and unsafe conditions

Consistently observes work conditions, operation, equipment and
tools for the purpose of accident prevention.
Complies with all regulation as well as safe work practices and

1. Ensure that the construction wastes are segregated from that of domestic

2. All domestic waste is to be collected on daily a basis.
3. Contruction debris (broken hollow blocks, spliced concrete, loose concrete,
etc.,) should be taken out on the staging area.
4. Oil spills and spoiled greases should be wrapping in the black garbage bag
and will be properly disposed.
5. Application of good housekeeping.

INTRODUCTION

The Guidelines in Construction Safety of Department Order No. 13


signed in 1998 mandates OSH requirement for this sector. Site safety officers
play key rules in the implementation of effective OSH programs.
In the interest of ensuring the protection and welfare employed in the
construction industry, the protection and welfare of the general public within
and around the immediate vicinity of any construction worksite as well as the
promotion of harmonious employeer-employeee relationships in the
construction industry, and after consultations with the stakeholders in the
construction industry, taking into consideration industry practices and
applicable government requirements, the following guidelines are hereby
issued for all concerned.
We will promote in our work place a work ethic and culture that will
enhance the personal conviction of management, employees and contractors
to comply reasonably with all applicable health, safety and invironment
protection standards, codes and laws. We will pursue continuous
improvement and strive to establish a high standard of prevention and
environment protection to avoid accident and illness.

OBJECTIVE (ZAP)

Zero accident prevention shall be considered of primary importance in


all phases of operation and administration. It is the intention of the
mangament to provide safe and healthy conditions and to establish and
insist upon safe practice at all times by all workers project sites.
The prevention of accidents is in objective affecting all levels of the
company and its operations. It is therefore, a basic requirement that each
supervisor make the safety of all workers and integral part of his/her regular
management functions. It is equally the duty of each worker to accept and
follow established safety regulations and procedures.
Every effort will be made to provide adequate training to workers.
However, if a worker is ever in doubt about how to do a job or task safety, it
is his/her duty to ask a qualified person for assistance. Workers are expected
to assist management in accident prevention activities. Unsafe conditions
must be reported immediately. Fellow workers that need help should be
assisted. Everyone is responsible for the housekeeping duties that pertain to
their jobs and work areas.
The Health and Safety Program covering all aspects of the work will be
instructed and compliance enforced ditifully including the required minimum
safety and reporting requirements of the Occupational Health and Safety
Standards and other relevant DOLE issuances.

SAFETY AND HEALTH PROGRAM

Safety and Health Personnel


The designated prsonnel will be administer the safety and health program for
the company. The responsibilities are as follows:

A. Safety Man
1. Implementing the safety policies stipulated in the companys health
and Safety Manual by insisting the that workers observed and obey all
the rules and regulations necessary to maintain a safe workplace and
safe work habits and practices.
2. Being knowledgable of potential jobs hazards.
3. Assuring compliance withconstructio safety and health standard
requirements.
4. Making regular safety inspections.
5. Establishing safety procedures.
6. Maintaining safety records.

B. First Aider
1. Give immmediate temporary treatment in case of injury or illness,
before the services of a physician becomes available.
2. Maintain first aid kits and required contents in the project site.

Job Site Inspections


The safety in-chrage or other designated person will tour the job site and
observe potential safety/health hazards and develop a plan for safeguarding the
workers which may include the following:

Periodic Safety Meetings (Tool Box Meetings)


Safety meetings will be conducted during the duration of the site work
activities to convey safety information and discuss the content of the safety
program, work site hazards, and other serious concealed dangers. The safety in-

charge or safety desigated person will review the companys safe work practices or
other safety information with workers. Whenever a new practice of procedure is
introduced into the workplace, it will be throughly reviewed for safety. Minutes of
the meeting shall be recorded and distributed by the safety in-charge or safety
designanted person and signed by all attendees. A daily toolbox meeting will also
be conducted among workers and their respective supervisors for the purpose of
instruction, disscussion and proper briefing on the planned work, the asssesment of
the past work, the possibility or actual occurence of accidents at the work areas,
tips and suggestions on how to prevent accidents and etc.
Personal Protective Equipment
1. Safety helmet wil be worn on job sites when there are potentials of falling
objects, hair entaglement, burning, or electrical hazards.
2. Google wil be worn when there are potential hazards from flying objects or
particles, chemical, glare, or dust.
3. Safety shoes shall be worn to protect from falling objects, chemicals, or
stepping on sharp objects.
4. Protective gloves or clothing shall be worn when required to protect against
hazards.
5. Harnesses and lanyards shall be utilized for all protection as required.

Safety Rules
1. All workers shall follow the safe practices and rules of the companys Health
and Safety Manual and such other safety rules and practices required to
safety execute any specific tasks.
2. All workers shall report all unsafe conditions or practices to the proper
authority including but not limited to their immediate supervisor, and if
corrective action is not taken immediately, a government authority with
proper jurisdiction over such practices.
3. Keep your mind at work at all times, Horseplay, scuffing, and other acts which
tend to have adverse influence on safety or well-being other workers are
prohibited.
4. Suitable clothing and footwear must be worn at times. Personal safety
equipment (hard hats, eye protection, gloves and safety shoes) must be worn
as prescribed by each job.
5. Watch where you are walking Dont run.
6. Anyone under the influence of intoxicating liqour or drugs, including
prescription drugs which impair motor skills and judgement shall not be
allowed on the job. Inform your supervisor if taking strong prescription drugs
that warn aginst driving or using machinery.
7. No one shall be permitted to work while the workers ability or alertness is so
impaired by fatigue, illnes or other causes that it might expose the worker or
others to injury.
8. Do not distract the attention of fellow workers. Do not egage in any act which
would endanger other workers.
9. Sanitation facilities have been provided for use. Defacing or damaging these
facilities is forbidden.

10.Good housekeeping must be practiced at all times in the work area. Clean up
all waste and eliminate any dangers in the work area.
11.Disposed of all trash, waste, and debris properly and carefully. A god is a
clean job, and a clean job is the start of a safe job. So keep your working area
free from rubbish and debris.
12.Never moved an injured person unless it is absolutely necessary. Further
injury may result. Kepp the injured as comfortable as possible. All injuries
should be reported immediately to the immediate supervisor and to the
safety in-charge so the arrangement can be made for medical or first-aid
treatment.
13.When lifting heavy objects, use the large muscles of the legs instead of the
smaller muscles of the back.
14.Nobody but the operator shall be allowed to ride on equipment unlees proper
seating is provided.
15.Do not use power tool and equipment until you have been properly instructed
in the safe work methods and become authorized to use them.
16.Workers should be alert to see that all equipment guards and other protective
devices are in proper places and adjusted. Do not removed, displace,
damage, or destroy any safety device or safeguard furnished or provided for
used on the job, nor interfere with the use thereof.
17.Before serving, repair, or adjusting any powered tool or piece of equipment,
disconnect it, lock out the source of power, and tag it out.
18.Use only extension cords of the three-prong type. Use ground fault circuit
interrupters at all times and using tools in wet atmosphere (e.g.outdoors) or
with any temporary power supply. Check the electrical grounding system
daily.
19.Never enter an area that has been barricaded.
20.If you must work around earthmoving equipment including trucks make sure
the operators can always se you.
21.Never oil, lubricated, or fuel equipment while enguine is running.
22.Barricade danger areas. Guard rails may be required.
23.Keep out of trenches or cuts that have not have been properly shored or
sloped.
24.The use of harnesses with safety lines when working from unprotected
heights is mandatory. Always keep your line as tight as possible.
25.Know where firefighting equipment is located and be trained how to used it.
26.Know what is emergency procedures have been established for the job site
(Location of the emergency phone, first-aid kit, fire extinguisher locations,
etc.).
Job Safety Instructios
A. After inspecting the job site, the safety in-charge or other designated
person wil identify and evaluate all potential hazards for:
1. Injury severity potential.
2. Probability of an accident.

B. The safety in-charge or safety designated person will also appraise the
skill and acknowledge level of exposed workers.
C. Appropriate instructions wil be given on the following:
1. Where hazards are pointed out.
2. Necessary precautionary measures will be explained.
3. The higher the hazards the more detailed will be the instruction.
D. Records will be maintained for all instruction sessions.
Safety Descipline
A. Four-step System
First Violation : Oral warning; notation for workers file.
Second Violation : Written warning; copy for worker and workers file.
Third Violation : Written warning; one day Suspension without pay.
Fourth Violation : Written warning; one week Suspension, or termination if
warranted.
B. A record will be maintained of all discipline with the Personnel Office.
Safety Procedures
1. Housekeeping
The practice of proper housekeeping and cleanliness is highly enforced. Poor
housekeeping is a major factor in many accidents. A cluttered area is bouth
unsafe and ineffecient. Every worker is responsible for keeping a clean area and
ensures that all areas remian clean and orderly.
Worksites must be clean and orderly. Work surfaces must be kept dry or
appropriate means taken to assure the surfaces are slip resistant. Spills must be
cleaned up immediately. Combustible dust must be cleaned up. The
accumulated combustible dust must be removed routinely.
Covered metal waste cans are provided for oily and paint-soaked waste.
Make sure all pits and excavation and floor openings are either covered or
otherwise guarded.

2. Temporary Facilities Electrical Supply


All temporary electrical installation will be inspected and certified as safe by
an Electrical Engineer wil be used on the following requirements:

Conduit system whether PVC or galvanized iron shall be used whichever is


appropriate.
Circuits are to be provided with rate MCB for protection.
Circuit panel boards must have an earth leakage circuit breaker of 30m A
tripping current.
Lighting fixtures (flourescent, etc.) must be installed with insulated
seperator against wooden ceiling at the point support in order to avoid to
avoid heat build up on the surface of the wood due to ballast operation.
Electrical conduits in kitchen shall galvanized iron and all accessories
shall be metal clad. Installation shall be expose and electrical sccessories
shall be located a distance away from source of heat.
Electrical outlets installed shall be based on actual requirements only.
Unauthorized extension is prohibited which if in case shall be done by
competent persons only.

3. Work Permits
Works permits must be secured all the time before starting any work so
the details of applicable safety requirements as formulated for the project
will be strictly followed.
4. Safety Equipment
The use of warninf signs and signage, barricades, and caution tapes
and other safety equipment necessary to execute the work safely will be
enforce. Dangerous areas shall be barricated and the use of guard rails or
perimeter cables if required will be used.
5. Personal Protective Equipment
Appropriate personal protective equipment and clothing will be always worn
according to
the activity, specifically:

Where there is danger of lying particles, workers must wear protective


googles and/or dace shields.
Workers are required to wear safety glasses at all times in work areas where
there is risk of eye injuries.

Hard hats must be worn specially in areas subject to falling objects, and at all
times while in the project site.
Appropriate footwear including steel toed shoes must be worn in areas where
there is any risk of foot injury from hot, falling objects, crushing or
penetrating action.

According to the activity, Specifically:

Where there is danger of lying particles, workers must wear protective


googles and/or dace shields.
Workers are required to wear safety glasses at all times in work areas where
there is risk of eye injuries.
Hard hats must be worn specially in areas subject to falling objects, and at all
times while in the project site.
Appropriate footwear including steel toed shoes must be worn in areas where
there is any risk of foot injury from hot, falling objects, crushing or
penetrating action.
In case where the noice levels exceed certain levels, ear protectio is required.

6. Excavations
Excavation must be barricated to protect workers and vehicles.
Trenches over five feet deep must be shored or sloped as required.
Exacavated or other
material will not be stored nearer than two feet from the edge of excavation and a
required one (1) meter strip on both sides will be leveled and kept clear to avoid
objects from falling into the trench. Personnel will not be allowed to walk close to
the edges of excavation to prevent them from falling. Excavation less than five feet
may also require cave in protection in some instances. Tenches will be inspected
everyday before work commences and after heavy rains or any other occurrences
which could affected satability.
Men working in trenches or pits are cautioned and required to protect
themselves with the proper PPE against falling objects. Men working in trench will
be cautioned to stand and work at a safe distance to avoid being hit being hit by
other workmens tools. Men working will be provided with ready access to and
egress from the trench by secured ladders or step.
In cases where the operation of jackhammer is needed the operator is
required wear the appropriate PPE and comply with noise protection regulation.
Equipment or vehicles will not be allowed to approach closer that what would
be the edge of the face battered in excavation.
7. Hoarding

The worksite will be fully barricated by protective hoarding so that the

only public from dangers within the site but also act as barrier or security to prevent
persons from trespassing into the site.
There will be an adequate safety distance the hoarding and the work site.
Where the distance from a public from the building being constructed is such that
there is the likelihood of falling materials stricking pedestrian or vehicular traffic
protection is necessary.
The entrance to & exit from the worksite will be located in such a manner as
to prevent danger and inconvenience to the public. The entry of an authorized
person or the public into worksite will be prevented.
Suitable warning signs will be posted in conspicuous places inside the
hoarded area.
No bills except warnings signs will be allowed posted in hoarding.
All construction materials will be stored and handled within the hoarded area.
Vehicles will be parked in designated areas within the hoarded area while
being loaded or unloaed. Where it is authorized for vehicles ferrying materials to the
worksite to park outside the hoarded area, suitable safety measures will be
undertaken such as cordoning off such parking areas and suitable warning signs and
flagman will be provided.
8. Scaffolding
Every scaffold and every part thereof will be good construction, of suitable
sound materials
and of adequate strength for the purpose pf which it is intended.
Scaffolds, bracings, and support shall be carried out in accordance with
standard safety procedures by competent scaffolders.
No scaffolds will be erected or be subtantially alttered or be dismantled
except under the direct supervision of a desiganted person.
Every scaffold will be properly maintained and every part thereof will be kept
so fixed, secured or place in position as to prevent, as far as practible accidental
displacement.
9. Concreting
Formwork and shoring will be properly braced or tied together so as to
maintain position
and shape.

The formwork including the shores, braces and other support will be
inspected by the designated person during erection, during and after concreting
work, and after any event likely to have affected their stability, for example after
heavy rain of following strong wind.
Worker involved in the mixing and the pouring of concrete are required to
wear proper PPE and specialized PPE for workers in contact with fresh concrete.
10.

Electric Welding and Gas Cutting

Only authorized and trained personnel are permitted to use welding and
cutting equipment.
Compressed gas cylinders will be regularly examined for obvious signs of
defects, deep
rusting, or leakage. Care will be used in handling and storing cylinders, safety
valves, relief valves, and the like to prevent damage. Precaution must be taken to
prevent mixture of air or oxygen with flammable gasses except at a burner or a
standard torch. Only approved apparatus (torches, regulators, etc.) will be used.
Cylinders will be kept away from the sources of heat. Empty cylinders will be
appropriately marked and their valves closed.
Cylinders, cylinders valves, regulators, hoses and apparatus will be kept free
of oil or greasy substances. Care will be taken not to drop or strike cylinders.
The open circuit (No load) voltage of arc welding and cutting machines will be
set as low as possible and not in excess of the recommended limits. Grounding of
the machines frame and safety ground connections of portable machines will be
checked periodically. Electrodes will be remove in the hoders when not in use. All
electric power will be shut off when no one is in attendance.
Suitable fire extinguishing equipment will be available for immediate use
before starting to ignite the weloding torch. Welders will be strictly forbidden to coil
or loop welding electrode cable around their bodies.
Fire watchers will be assigned when welding or cutting is performed in
locations where serious fire might develop.
Personnel exposed to the hazard created by welding, cutting or brazing
operations will be protected with proper PPE and clothing.
Adequate ventilation will be checked where cutting or welding is performed.
11.

Painting Works

Painting operation will be carried out in matter that will minimize dispersal of
paint mist
and solvent vapor in public areas.
12.

Electrical Works

Electrical works will be performed by experienced and skilled workmen using


proper tools
equipment. The safety standards contained in the Philippine Electrical Code will also
be adopted in order to safeguard any person employed with the project.
Final inspection will be conducted and permit taken from the local utility
company before any service or power supply will be connected to the electrical
system of the project.
13.

Driving

All workers within the project site will be advised to Drive Safety. If vehicle are
used during
the work day, seat belts and shoulder harnesses are to be worn at all times. The
speed limit imposed within the project site will not be exceeded. Vehicle will be
parked in legal spaces and must not obstruct traffic. Defensive driving will be
practiced by workers at all times.
14.

Equipment

General for all equipment


All operators of mobile equipment have obtained a license for taht class of
machine and be
certified or trained in the operation of the machine they are to operate.
Maintenance and check up schedule will be followed for each item of
equipment as established and part of the equipment operational system of the
company.
Machine operators will not carry passengers unless proper setting is provided
with the equipment.
Machine will not be left unattended while the engine is running. When
machines are unattended the engines are to be stopped and parking brakes applied
or the wheels choked; blades, buckets and other hydraulic attachments lowered to
the ground before the operators leave the machine and the ignition key removed
from the ignition switch.
If ever the operator of the machine cannot see the area around the machine
during the operation, attendant is to be positioned to direct and assist the operator.
All moving machenery is to be guarded, specially gears, pulleys, Vee-belt
drives, fans and revolving shafts common to most static equipment used in the
construction site.
A. Portable Power Tools
Equipment will be checked before use. All grinders, saws, and similar
equipment will be

equipped with appropriate safeguards. Power tools must not be use without the
correct shield, guard, or attachment will be guarded to prevent physical contact.
All cords connecting electrically-operated tools and equipment will be double
insulated or effectively grounded.
Proper PPE will be worn when operating these tools.
B. Small Concrete Mixers
The mixer will be operated by qualified personnel given adequate
instruction and
Training by a competent person to operate such machinery. Effective guards
will be in place over belts, pulleys, chains or at appropriate places. The mixer will be
properly set up on firm level ground when in use and the wheels are properly
choked to prevent movement while in the operation. A regular inspection and
maintainance will be performed specially on the drum and pulley gear in order to
ensure that the machine is in good working condition at all times.
Proper PPE will be worn when operating the machine.
C. Excavators/Payloaders
Operator of these units will have proper training and be specifically
authorized to
operate the machine.
Drivers of dump truck being loaded by the machines will be cautioned to
stand clear of their units and the excavator/loader while loading is in progress.
When working around the machines, workers are cautioned to make sure that the
operator can seee notice them by all the face the machine.
Operators will be advised not to moved loads over the heads workers, truck
cabs or any vehicle.
Whell loader when travelling will carry its bucket low so that it does not block
the vision of the operator and to maintain satability.
Proper PPE will be worn when operating the machine.
D. Dump Trucks (D Trucks)
Drivers will drive defensively, obety road signs and will never race other
vehicles.
Dump trucks will not be driven at speed exceeding the authorized speed
limit and
loaded beyond the authorized load limits.
During the loading operation driver will not enter or leave the cab while
loading is in progress; only to moved off when signaled that loading is complete.
Where material is to be transported on a public road, local government
regulations must be followed.

Dump truck carrying materials, and axcavated materials, is ensured to be


clean, well maintained and in good running conditions. If carrying loose materials,
they wil be covered and properly sealed to ensure that there will be no spillage of
materials to public road outside the project site. The wheels of the vehicle leaving
the site will be off mud and ensure that mud will not be carrried out into other
areas.
Backing is the most hazardous in truck operation. Reserving alarms will be
installed and fitted in trucks and driver will only back off when under the direction of
a signalman or when satisfied that the way is clear and will remian clear.
During unloading, drivers will not raise the trucks tray unless the truck has
fully stopped and will not move the truck unless the tray is fully down.
Every measure will be taken to suppress any dust generated by the vehicle.
Care will be taken to protect overhead utilities; if any specially electrical
cables.

Fire Prevention

Upon arrival at a new job site, the safety prsonnel or designated person shall
evaluated the
site make sure all the personnel are aware of emergency phone numbers for
notifying the local fire department and other emergency agencies.
Portable fire extinguishers shall be accessible at all workers are instructed in
the use of extinguishers and the fire protection procedures adapted for the project
site.
Construction areas will be maintained in a fire safe condition and accessible
for emergency respose forces. Proper construction fire safety practices will be
utilized during all construction activities. Excessive combustible (Paper) will not be
stored in work areas.
All combustible scrap, debris and waste materials (oil rags, etc.) will be stored
in covered materials recepatacles and remove from the worksite promptly. Proper
storage to minimize the risk of fire will be practiced. Only approved container and
tanks are to be used for the storage and handling of flammable and combustible
liquids. All flammable liquids will be kept i close containers when not in use. Gas
cylinders and oxygen cylinders will be stored seperated with a fire resistant barrier.

First Aid (Health Care)


First aid kit and required contents are maintained in serviceable condition. A
poster will be fastened and maintained either on or in cover of the first aid cabinet
and plainly stating the cell phone numbers of the first aider, the deignated health
and safety person, the Citys Medical and Fire Response Teams.
Workers Welfare Facilities

Existing clean sanitary facilities including washing, bathing, and drinking


facilities are provided for the use of all persons employed for the works by the
Client. Dining hall will be provided with access to clean drinking water.
Disposal of Waste
Construction debris resulting for the construction operations (including food
scraps and the like) will be prperly collected, segragated, recycled and disposed
from time to time as necessary. Suitable waste and garbage disposal methods and
procedures will be adapted as approve by the Client.
Environmental Control
The conduct of operations throughout the performance of the work will be
observe in such a way as to minimize the impact upon the environment by:
1. Providing dust control within work and all other areas including dust
control in common areas. Speed limit will be imposed; the covering of
construction materials transport will be required and imposed. Spraying of
water during dry weather will also be implemented to avoid dust emission.
2. Providing working machinery and equipment with effecient noise
suppression devices and evry effort will be maid to avoid unnecessary
noise emissions from the project site. Particular care will be undertaken
that ineffective exhaust mufflers are place immediately.
3. Providing equipment taht is in good state of repair before assigning to the
projcet.
4. Materials storage areas will be kept in a neat and tidy manner to minimize
hazards to persons and environment.
5. Precautionary measures will be undertaken to prevent soil erosion from
any lands during the execution of the works.

Emergency Procedures
In the event of an accident, the health and safety designated personnel and
the first aider will take immediately control of the incident. The injured person
should not be touched. The power supply will be switch off or arrange to have power
switch off before touching the injured person.
Emergency mdical assistance will be immediately ask for so the same can be
provided right away by the Client of the nearest outside medical facility. Emergency
cell phone numbers of Medical/Fire Department, the health and safety desigmnated
personnel and pf the First Aider will be posted in the field office especially in the
cover of the first aid cabinet and above the fire extinguisher.

In case of fire, attempt will be made to extinguish the fire using the aviulable
fire fighting equipment and raise the alarm by calling the Citys Fire Department.

Policy (CHSP)

Construction Safety and Health Report


All general construction shall be required to submit monthly constructing safety and
health report to the BWC or the DOLE Regional Office concerned. The report shall

include a monthly summary of a; safety and health committee agreements, a


summary of all accidebt investigation/reports and periodeic hazard.
In case of any dangerous accurence of major accident resulting in death or
permanent total disability, and concerned employeer shall initially notiofy the
DOLE Regional Office wiyhion twenty four (24) hours of occurence. After the
conduct of investigation by the concerned construction safety and health
officer, the employeer shall report all permanent total disabilities to DOLE
Regional Office on or before the 20th of month following the date of occurence
of accident using the DOLE?BWC/HSD-IP-6 from.
Work Environment refers to one combination of all physical, biological,
chemical, medical, physiological, psychological, social, technological, and
technical elements taht difficulty affect the workplace.
Physical Factor or Hazards Refers to temperature, humidity noise, ionizing
radiation, abnormal pressure (due to attitude), ventilation, illumination and
like which may place added stress on the body.
Chemical Substance means any organic or inorganic substance of a
particular identity, any element or uncombined chemical and any
combination of such substances; or any mixture or two or more chemicla
substance.
Hazardous materials or subtances refer to subtances in solid, liquid or
gaseous form knows to constitute, flammable, explosive, corrosice, oxidizing
and radioactive properties.
Establishments refer to any single proprietorship, somrporation or similar
entity operating an economic activity and engaging the service or workers.
Hazardous Establishments in one where the employee to potential risk of
disabling injury, illnes or death, either because of the presence of agents,
radiation, chemical substances, hazardous materials or physical hazard in the
work processes performed or equipment used therein.
Hazardous work processes refer to work operations or practices
performed by a worker in the establishment or worlplace with or as an
incident to such operation or practices and which expose the worker to
hazards likely to cause disabling injury, illness, death or physical or
psysiological harm.
Immenent danger means a condition or practice that could resonably
expected to cause daeth physical harm before abatement under the normal
enforcement prosedures can be accomplished.
