You may wish to modify these documents to create your own customized versions with information that is specific to
your state or local area.

OJT Progress Report and Invoice Template


This template was designed for employers to complete as a part of the invoicing and evaluation process. The
template asks employers to complete progress report evaluations on the trainee as well as invoice reimbursable OJT
Trainee hours worked. The template helps to address the need that FPOs and state and local workforce providers
have in asking the employer to complete regular evaluations on the OJT Trainee. This form has the employer
complete these regular progress report evaluations as a part of the invoicing process. Our thanks to Roseana Smith,

Training Plan Template


Use this template to outline and document plans for each trainee. Information required for the template include the
job description, skills gap, training goals and outline, starting and ending wages, obligation for reimbursement to the
