Abstract When an accident occurs on an offshore platformEER from it will play a vital role in safeguarding the lives To improve the efficiency and minimise the casualties of personnela qualitative analysis of the EER was carried out based on accident investigation According to the results of typical accidents
analysisaccident-causation factors in all stages of EER were extracted The factors of inner-logicpatterns and paths of fatal accidents were probed by using the logical-tree model Safety barriers were proposed as control measures The results show that accidents happening in the EER process the results of
coupling of multi-factors of environmentincluding human behaviorequipment and managementthat the
logical-tree model is practical for its direct and accurate presentation of causes and consequences of accidentsthat four kinds of death patterns and nine kinds of risk transmission paths can be found by using the
modeland that control measures based safety barrier models can efficiently ensure the EER operation
