in multidisciplinary teams. As part of this effort, we have asked each student to identify nurses during their clerkships to assess
them with the Professionalism Assessment Tool. You have been identified by __________________ to assess this student.

FSM Education Portal:

You will need to log in to the Education Portal using the following username and password:
Username: pat123
Password: pat4565!
After logging in, click Professionalism Assessment Tool. Enter the students name and select it from the list. Then click the
Submit button. Click the New link that corresponds with the date range displayed. (You may need to change the Academic
Year to 2014-15 in some cases). Constructive comments are incredibly valuable to the students as they reflect on their
professional development. The more specific to the students behaviors that you can be in providing comments, the better.
