Su traduccin
Many adults with asthma have inadequate control of
symptoms when receiving a low-to-medium dose of an inhaled
glucocorticoid. Treatment options include the addition of a
leukotriene modifier, the addition of a long-acting beta-agonist
(LABA), or
an
increased
dose
of
an
inhaled
glucocorticoid. Current guidelines of the National Asthma
Education and Prevention Program favor the last options. In
recent communications, however, the Food and Drug
Administration (FDA) and asthma experts have questioned the
safety of LABA therapy and suggested strategies to minimize
the use of these drugs. Because of such concerns and the
heterogeneity of patients with asthma, alternative controller
agents are needed.
Whether anticholinergic agents are useful for asthma
management is not clear. A Cochrane Review reported that
there is no justification for routinely introducing
anticholinergic agents (the report focused on ipratropium
bromide), while acknowledging that the role of long-acting
anticholinergic agents such as tiotropium bromide has not
been established. Tiotropium has a duration of action of more
than 24 hours and was approved by the FDA for the treatment
of chronic obstructive pulmonary disease (COPD) in 2004. In
a double-blind, three-way, crossover trial, called the
Tiotropium Bromide as an Alternative to Increased Inhaled
Glucocorticoid in Patients Inadequately Controlled on a Lower
Dose of Inhaled Corticosteroid (TALC) study, we tested two
hypotheses. The primary hypothesis stated that in patients with
asthma that is inadequately controlled by an inhaled
glucocorticoid alone, the addition of tiotropium bromide
would be superior to a doubling of the dose of an inhaled
glucocorticoid. The secondary hypothesis stated that in such
patients, the addition of tiotropium would not be inferior to the
addition of a LABA. We evaluated the primary outcome, the
morning peak expiratory flow (PEF), as well as additional
outcomes, in 210 patients with asthma inadequately controlled
by a low dose of an inhaled glucocorticoid.

CONVENIO LABORAL DE EMPLEADOS DE FINCAS URBANAS PARA LA COMUNIDAD DE MADRID

El presente Convenio Colectivo se ha suscrito por la Cmara de la Propiedad Urbana de Madrid, y las Centrales Sindicales de Trabajadores
de Fincas Urbanas.
Las partes firmantes tienen legitimacin suficiente, conforme a las disposiciones vigentes, para establecer los mbitos de aplicacin,
obligando a todos los propietarios y trabajadores en l incluidos durante el tiempo de vigencia.
Artculo 1.- mbito de aplicacin territorial.
Este Convenio establece las normas que regirn las relaciones de trabajo entre los propietarios de fincas urbanas y los empleados de stas
a su servicio, dentro de la Comunidad de Madrid.
Artculo 2.- mbito de aplicacin funcional
Estarn obligados al cumplimiento de este Convenio:
a) Los propietarios de fincas urbanas, individualmente o en Comunidad de Propietarios de cualquier clase.
b) Los Empleados de Fincas Urbanas que bajo la directa dependencia de los propietarios de fincas urbanas o representantes legales de los
mismos, tienen encomendada, la vigilancia, cuidado y limpieza de ellas as como cualquiera de los servicios comunes existentes.

aceptacin o rechazo por la educacin recibida; trayectoria de estudios; historia laboral, historia de uniones
anteriores, causa de separacin o divorcio y lecciones aprendidas.
