I, RUFO BAES, of legal age, Filipino, married and a resident of

Brgy. Lawaan, Roxas City, Capiz, after being sworn in accordance with
law, hereby declare that:
1.
I am the member-consumer of Capiz Electric Cooperative
(CAPELCO) with office address situated in Timpas, Panit-an, Capiz;
2.
That I was issued an Electric Account No. 17-1709-0385 with
Meter No. 212114967 in my said residence;
3.
That upon signing of this document, I hereby WAIVE,
RENOUNCE and CONVEY all my rights and obligation over the said
electric meter unto FILOMENA DALMAN-BAES, likewise of legal age
and a resident of Brgy. Lawaan, Roxas City, Capiz, who is able and
willing to assume all my rights and obligations over the said electric
meter;

AFFIDAVIT OF ASSUMPTION OF
ELECTRIC METER (CAPELCO)
I, ESTRELLA S. GAYAMAT, of legal age, Filipino, married and a
resident of Premier De Mayo Street, Roxas City, Capiz after being sworn
in accordance with law, hereby declare that:
1.
I am the daughter of the late ERNESTO SANTIAGO who
was a bonafide member-consumer (during his lifetime) of Capiz

Electric Cooperative (CAPELCO) with office address situated in

Timpas, Panit-an, Capiz;
2.
That my father Ernesto Santiago during his lifetime
was issued an Electric Account No. 17-1732-0810 and Meter
Serial No. 0369506905 for his house situated in Premier De Mayo
Street, Roxas City, Capiz;
3.
That presently, I and my family are the ones who
occupy and reside in the house of my deceased father and we are
the ones who now consume the electric supply in said house;
4.
That considering these facts, I am now adjudicating to
myself (who is able and willing to bind myself to assume as I
hereby assume) all the rights and obligations of my deceased
father over the said electric meter as well as the monthly
payment of the electric consumption we consume;
5.
That I hereby further agree to be bound by the precise
terms and conditions imposed by the CAPELCO to its
consumers/members involving their respective electric meter;
6.
That I am executing this affidavit to attest to the truth
of the foregoing, as a form of adjudication to the said electric
meter, to inform the CAPELCO about my assumption of said
electric meter, and for whatever legal purpose this may serve
best.
