Instructions:

1. Group name and File name


Surname of all group members arranged alphabetically and separated by a comma.
2. Variables
Indicate the variable and in parenthesis the type of variable. Each variable should be
separated by a comma.
3. Proposed sample
Indicate the sample where data is be collected.
4. Font type and size
Use Tahoma font size 10.
5. Source of items
Indicate the source (e.g., empirical, book) of the survey items. The entry should comply
with the proper APA referencing format.
6. Entry for survey items
Use table grid in two columns. Column 1 item number and Column 2 item. Do not
place a period at the end of the item. Place the letter (R) at the end of the item to indicate a
reverse item. Ensure that the items are aligned with the source. An incorrect item entry
would automatically lead to a major revision and re-administration of the survey in MPRACT2.
