Dear Mr. Surveyor,


Please complete the enclosed form with the information available on site but remember always it is only a guide to assist with the reporting.
Typing is optional. Please note that only ONE copy of the report is required, which should be forwarded as soon as possible.
When reporting on the various items a tick in the respective boxes is not sufficient. Description of each object, full filling up all items in Part
C are usually required.
Please indicate together with fact repair size of bearings their nominal size.
The surveyor should use his knowledge and experience to determine if a real defect exists and consequently if a recommendation is required.
Hose test of hatch covers and pressure test of double bottom tanks: these are not essential at all surveys unless visual inspection of hatch
covers, tanktops or air pipes indicate that a test would be necessary to prove the watertight integrity of the holds. If such tests are considered
necessary, but are not possible to carry them out, surveyors should report this and detailed circumstances in the usual way.

It is important that the nature and extent of all defect should be stated in the List of defects (proforma enclosed) A recommendation for
rectification of any deficiency should also be made, but in suitable broad terms to allow the operator or owner to make his own arrangements as
appropriate.
Each defect item/recommendation should be indicated as immediate/urgent/non-urgent/dry-dock, (or repair period). The Insurer will further
define these when communicating with the Assured. The normal periods are, immediate 7 days, urgent 30 days, non-urgent 90 days, drydock next scheduled dry-dock or repair period.
The List of defects and recommendations, must be signed for receipt by the Master or any other managers or owners representative and the
original returned to the Insurer. If a signature is refused or is unobtainable this should be stated with the reason. A copy of the signed page(s)
should be left with the person signing with the request that it is passed to the ships managers or operators. The surveyor should explain that the
recommendations made would be forwarded to the Insurer who will subsequently inform the Assured as appropriate.
The Insurer usually requires a few photographs to indicate the type and layout and condition of the vessel, and then photographs of defects.
Photocopies of plans or diagrams to supplement the report, preferably A4 size. If information is required from the vessel or owners/managers
once the surveyor has departed the vessel, then the vessels owner / manager should be requested to forward these directly to the Insurer.

Due to deficiencies highlighted by Port State inspection in Trinidad
