application of treatments but has not been the subject of empirical research
in the area of eating disorders.
These more complex approaches can be contrasted with the best shot
intervention approach, which involves initially implementing the intervention that is thought to be most effective, regardless of intrusiveness or cost.
At this time, proposals for integrating and sequencing different forms of
treatment have been made exclusively on rational grounds; the efficacy of
the stepped-care and decision-tree models have not been empirically tested
in the area of eating disorders. This is a result, in part, of the formidable
practical and theoretical impediments to a research protocol that would
have to be developed to test the stepped-care and decision-tree models.
We will discuss these impediments and possible directions for research later.
Nevertheless, limited clinical resources and pressures for containing health
costs are powerful incentives to develop cost-effective treatment programs
in which the most expensive procedures (e.g., inpatient treatment) are
reserved for the most seriously ill patients or those who fail to respond to
more conservative approaches.
This chapter reviews the literature on stepped-care and decision-tree
approaches applied to the treatment of eating disorders. This is followed
by a flowchart illustrating the treatment decision-making process as well
as a brief overview of the major treatment options for treating eating disorders.

from both the stepped-care and decision-tree models. This proposal was
based on the belief that there should be a progression from the least complicated and intrusive approaches to those that are more disruptive, costly,
or carry greater risks. The authors recommended an educational approach
as the initial intervention for the least disturbed bulimia nervosa patients
and as an adjunct to treatment to other eating-disordered patients, including those with anorexia nervosa. They recommended family therapy as the
primary treatment modality if the patient was 18 years old or younger and
living at home. They considered pharmacotherapy as an option in bulimia
nervosa (not anorexia nervosa) when patients failed to respond to psychosocial treatments. For patients with serious medical complications or for
those who needed to gain substantial body weight, the authors recommended inpatient treatment as the initial intervention. Finally, they recommended day treatment as a cost-effective alternative for many patients
who did not require hospitalization. Day treatment programs can provide
structure around mealtimes as well as intensive therapy, without requiring
the patient to become totally disengaged from the supports and the therapeutic challenges outside of the hospital. It also can facilitate the transition between inpatient and outpatient care.
The integration and sequencing model involves decision making at
each step, guided by considerations such as: (a) age of the patient; (b)
current living arrangements; (c) duration of the disorder; (d) current symptomatology (presence of binge eating, vomiting, or purgative abuse; stable
or unstable weight; deteriorating symptom picture); (e) previous treatments; (f) premorbid functioning with particular reference to depression,
problems with impulse control, and personality disorders; and (8) medical
condition including, but not limited to, degree of weight loss and electrolyte disturbances. Although the model proposed by Garner et al. (1986)
represents an early attempt to articulate the decision-making process in
delivering different forms of treatment for anorexia nervosa and bulimia
nervosa, it lacks clear criteria for determining how patients would be reliably allocated to different treatment options. Thus, it is difficult to see
how it could be tested empirically.
Fairburn and reveler ( 1990) briefly outlined a stepped-care approach
in managing bulimia nervosa patients that consists of five levels. These are
(a) self-help or written materials; (b) dietary education and advice, perhaps
in a group setting; (c) antidepressant drug treatment (Desipramine or
Fluoxetine) in combination with advice and support; (d) outpatient cognitive-behavioral treatment, and (e) day or inpatient care with subsequent
outpatient treatment. Fairburn et al. (1992) proposed a similar model for
managing bulimia nervosa that did not include antidepressant drugs because their position in the scheme is debatable (p. 334). The model by
Fairburn and colleagues (Figure 1) is novel because it formally introduces
the use of supervised self-help, which is potentially more efficient than a

professionally delivered psychoeducational treatment. However, the reasoning behind including supervised self-help, for which there are no supportive data, and excluding drug treatments on the basis of debatable findings, is not explained.
Agras (1993) described a decision-tree model for the treatment of
binge eating (Figure 2) that is more precise than earlier recommendations.
However, the rationale for choice points on the decision tree may not be
flexible enough to address the clinical needs of the heterogeneous patient
population. According to this model, patients are assigned to psychoeducational treatment if their symptoms are mild to moderate. If their binge
eating is severe, they are either assigned to cognitive-behavioral therapy
or interpersonal psychotherapy depending on whether dietary or interpersonal problems predominate. The initial decision to allocate patients to
psychoeducational treatment or a more intensive individual therapy is justified on the basis of one study (Olmsted et al., 1991). Although the decision to assign patients to either cognitive-behavioral therapy or interpersonal psychotherapy on the basis of principal symptoms being either
interpersonal or dietary has intuitive appeal, there is no empirical evidence
that these treatments are differentially effective based on these clinical
features. Finally, the Agras (1993) model allocated obese patients who become abstinent from binge eating to weight loss treatment, a point that remains
controversial (Garner & Wooley, 1991).

aware that a subgroup of patients may require or benefit from hospitalization. The primary objectives of hospitalization are (a) weight restoration
or interruption of steady weight loss, (b) interruption of unremitting bingeing and vomiting, (c) treatment of medical complications, (d) management
of associated conditions such as severe depression or substance abuse, and
(e) at times the disengagement of patients from a social system that both
contributes to the maintenance of the disorder and disrupts outpatient
treatment. When the goal of hospitalization is to treat medical complications, this should be clearly distinguished from inpatient treatment aimed
at recovery from the eating disorder (Figure 3). When directed strictly
toward treating medical complications, it should be presented as an urgent
medical priority that does not necessarily require a strong commitment by
the patient to recover. If hospitalization is intended to treat the psychological as well as the physical aspects of the disorder, every effort should
be made to enlist the patients commitment to participate actively in the
treatment process. Preadmission interviews should include a clear description of the program aimed at normalization of eating and weight as well
as other details regarding the ward regime. Patients should be reassured
that they will receive psychotherapy both during the period of hospitalization and thereafter to assist them in dealing with their concerns regarding
eating and weight, as well as to address psychological issues that may be
unique to them. This approach will reduce the likelihood that patients will
prematurely discharge themselves from the hospital.
Hospitalization may be a prerequisite to recovery if an adequate trial
of outpatient therapy fails to lead to a steady reduction in bingeing and
vomiting or appropriate weight restoration. Patients may perceive the recommendation for hospitalization as a threat or as abandonment, but they
should be reassured that it is a humane alternative to the tremendous
emotional and financial expense of prolonged and unproductive outpatient
therapy that cannot progress in the presence of the severe limits imposed
by dietary chaos or starvation.
Day Treatment
Day treatment is a viable alternative to hospitalization for many patients. There are various models for day treatment programs, and they generally have many features in common with inpatient programs. The major
difference is that patients receive the therapeutic services that the treatment program has to offer but do not stay overnight. This is adequate for
most patients with eating disorders. However, for those who are at serious
risk of self-harm, inpatient programs are still necessary to provide a protective environment. As with inpatient programs, day care programs can
be structured so that they are oriented exclusively toward eating-disordered
patients or so that they provide care for patients with different diagnoses.

Day treatment programs provide needed structure during meal times and
evenings when many eating-disordered patients are most vulnerable to
binge eating or other eating disorder symptoms. Day programs also allow
the patient the flexibility to work or attend school while still receiving
intensive treatment. Thus, the programs minimize the disruptive aspects of
hospital confinement and allow the gradual reintroduction of normal living
at a pace that meets the individual needs of patients. Day programs differ
tremendously in the number of days and number of hours per day that
treatment is offered, so the decision to implement day care will be greatly
influenced by whether the available services can meet the needs of the
particular patient. Finally, day programs have the distinct advantage of
being more economical than inpatient care.
Educational Intervention
There are patients who have relatively mild symptoms and can benefit
from education alone. There has been considerable variation in the content
of treatments identified as educational, with some stressing dietary management techniques and others emphasizing social skills, assertiveness, and
relaxation training or some combination of these approaches (Beumont,
OConnor, Touyz, & Williams, 1987; Connors, Johnson, & Stuckey, 1984;

Garner et al., 1985; Wolchik, Weiss, & Katzman, 1986). However, there
are some data suggesting that minimal interventions, which provide information or education, may lead to improvement with some eating-disordered patients. Ordman and Kirshenbaum (1985) found that there was
some improvement in a bulimia nervosa control group following one informational interview. Garner et al. ( 1985) developed a comprehensive
educational treatment manual for eating disorders that emphasized the conflict between the cultural pressures on women to diet and biological compensatory mechanisms that tend to defend a set point for body weight.
This educational intervention focused on challenging Western societys
harsh standards for women to be thin, educating patients regarding the
biology of weight regulation, structuring regular eating patterns, and implementing techniques for interrupting bingeing and purging. These were
combined with self-monitoring and meal planning involving the gradual
incorporation of foods that are avoided. The effectiveness of this educational approach was assessed in a study comparing individual cognitive-behavior therapy with brief psychoeducational group treatment (five
90-minute sessions during a I-month period), delivered in a group classroom format (Olmsted et al., 1991). Results indicated for the least symptomatic 25% to 45% patients in the sample, both treatments were equally
effective on several important measures of outcome.
Lassele et al. (1991) compared nutritional management, defined by the
same methods and goals as the psychoeducational approach discussed previously, and stress management. They found both treatments led to significant reductions in bingeing and vomiting, as well as measures of psychopathology at the end of treatment and at a 12-month follow-up. Nutritional
management produced a significantly more rapid reduction in binge eating,
greater abstinence rates, and greater improvements on some measures of
psychopathology.
Some bulimia nervosa patients, particularly those with less severe pathology, seem to respond favorably to information about dieting and weight
regulation. Although educational treatments have varied somewhat in content and intensity, it is reassuring that a number of bulimia nervosa patients
appear to benefit from a treatment that is likely to be both cost effective
and to minimize psychiatric labeling.
Self-Help Manuals
The use of self-help manuals alone is not included in Figure 3 because
there have been no formal studies of their effectiveness with eating disorders. Moreover, there are currently no manuals available that we would
recommend as suitable. However, the enormous popularity of self-help
books (Santrock, Minnett, & Campbell, 1994) would suggest that these
approaches need to be exposed to empirical study. From a purely clinical

point of view, we have been impressed with the improvement some patients
describe after simply reading our psychoeducational manual (Garner et al.,
1985). Nevertheless, these impressions do not warrant optimistic proclamations because many treatments appear effective in case reports or open
trials, only to fail when put to the test in comparative trials. There is some
evidence from a mail survey study that disturbed eating patterns decline
over time with no intervention (Yager, Landsverk, & Edelstein, 1987).
Nevertheless, controlled-outcome research on the supervised use of selfhelp manuals is warranted, particularly with patients who have mild to
moderate symptoms of binge eating.
Family Therapy
Family therapy has been recommended as the preferred treatment
modality in both anorexia nervosa (Minuchin, Rossman, & Baker, 1978;
Selvini-Palazzoli, 1974; Vandereycken, Kog, & Vanderlinden, 1989) and
bulimia nervosa (Vandereycken et al., 1989) or as an adjunct to other
methods (Andersen, 1985; Crisp, 1970; Garner, Garfinkel, & Bemis, 1982;
Wooley & Lewis, 1989). Family therapists have argued convincingly that
the eating disorder may have a distinctive role in precipitating or maintaining certain dysfunctional roles, alliances, conflicts, or interactional patterns within the family. This can have any one of a number of manifestations. The eating disorder can deflect members of the family away from
the developmental tensions that naturally emerge with the transition to

puberty and the attendant preparation for emancipation. The eating disorder may serve as a maladaptive solution to the adolescents struggle to
achieve autonomy in a family in which any move toward independence is
perceived as a threat to family unity. It can also provide a means by which
the parents and the child avoid major sources of conflict.
For young patients, there are practical as well as theoretical reasons
for recommending family therapy. From a strictly practical point of view,
patients share their homes with others, and their parents or guardians are
responsible for their well-being. The parents have the potential to provide
environmental controls that are consistent with therapeutic goals. Regardless of theoretical orientation of the treatment setting, family members
need assistance in dealing with a young eating-disordered patient. Often
families have been sufficiently terrified by the weight loss that they become
intimidated by the anorexic patients demands for acquiescence to their
bizarre dietary rituals. Family members may feel guilty about imposing appropriate controls because of some variant of the belief that they could not
live with the anguish of their childrens potential death or rejection resulting from the parental limit setting. In these cases, the individual members of the family or the entire group need to alter dysfunctional beliefs
and faulty assumptions that are dominating their interactional style. In the
case in which the patient is using the eating disorder symptoms to deflect
away from marital problems, then the attendant problems-as well as the
assumptions driving this only partially effective deflection-need to be addressed.
Cognitive-Behavioral Therapy
Cognitive-behavioral therapy has become the standard treatment for
bulimia nervosa and forms the theoretical base for much of the treatment
of anorexia nervosa. The earliest descriptions of cognitive-behavioral therapy for bulimia nervosa (Fairburn, 1981) and anorexia nervosa (Garner &
Bemis, 1982) adapted standard cognitive-behavioral therapy methods to
address the special needs of eating-disordered patients. It may be delivered
in a group or individual format, and it can be integrated with family therapy.
The cognitive-behavioral therapy for bulimia nervosa has been developed further by Fairburn and colleagues (Fairburn, 1985; Fairburn, Marcus, & Wilson, 1993) and has the following major points of emphasis: (a)
self-monitoring of food intake, bingeing, and purging episodes, as well as
thoughts and feelings that trigger these episodes; (b) regular weighing; (c)
specific recommendations such as introducing avoided foods and planning
meals designed to normalize eating behavior and curb restrictive dieting;
(d) cognitive restructuring directed at habitual reasoning errors and underlying assumptions that are relevant to the development and mainteSTEPPED-CARE AND DECISION-TREE MODELS

nance of the eating disorder; and (e) preventing relapse. Monitoring eating
behavior begins in the first session with the rationale that it helps the
patient and the therapist examine eating behavior and the situations in
which difficulties occur and that it helps the patient alter eating behavior
and associated thoughts and feelings (Fairburn, 1985; Fairburn, Marcus, &
Wilson, 1993). Patients are supplied with written instructions for self-monitoring and are encouraged to record all food and liquid ingested as soon
as possible after consumption. Self-monitoring sheets are reviewed in detail
in the second and in all subsequent sessions.
In the case of anorexia nervosa, cognitive-behavioral therapy has
been recommended largely on clinical grounds (Garner, 1986, 1988; Garner & Bemis, 1982, 1985; Garner & Rosen, 1990). There is some empirical
research to provide grounds for optimism (Channon et al., 1989; Cooper
& Fairburn, 1984); however, the data are insufficient to warrant any meaningful conclusions regarding effectiveness. As in bulimia nervosa, the anorexic patients behavior is determined by the assumption that weight,
shape, or thinness can serve as the sole or predominant referent for inferring personal value or self-worth (Garner & Bemis, 1982, p. 142). The
initial emphasis of treatment is to establish a sound and collaborative therapeutic relationship. Treatment is aimed at gradually assisting the patient
to normalize eating and weight through both cognitive and behavioral
interventions. Cognitive interventions are aimed at beliefs that maintain
extreme dieting and chronic weight suppression and also fundamental assumptions associated with feelings of worthlessness, ineffectiveness, lack of
autonomy, and developmental themes that have thwarted both patient and
family (Garner & Rosen, 1990). Although there is considerable overlap
between the cognitive-behavioral therapy approach to bulimia nervosa and
that described for anorexia nervosa, a major factor differentiating treatment
for these two disorders is the need for weight gain with anorexia nervosa.
This requires adaptation to the therapy, to deal with a low body weight
and other symptoms that are ego-syntonic. In the subset of patients who do
not engage in binge eating, and those who show no obvious serious physical
complications, there may be extraordinary resistance to the therapeutic
objectives of weight gain.
In summary, over the last decade, there has been a steady accumulation of controlled research indicating that cognitive-behavioral therapy
is an effective treatment for bulimia nervosa, and this has been the subject
of several reviews (Fairburn et al., 1992; Mitchell, Raymond, & Specker,
1993; Wilson & Fairburn, 1993). These reviews provide several important
conclusions regarding the present state of knowledge:

have been a number of research trials in which behavior therapy alone has
been compared to cognitive-behavioral therapy for bulimia nervosa (Fairburn et al., 1991, 1993; Wolf & Crowther, 1992). Using an individual
therapy format, Fairburn and colleagues found a poor outcome for behavior
therapy at the end of treatment (Fairburn et al., 1991) and at 1-year followups (Fairburn et al., 1993) compared to both cognitive-behavior therapy
and a form of interpersonal therapy adapted to eating disorders. In a study
of behavior therapy and cognitive-behavior therapy delivered in a group
format, Wolf and Crowther (1992) found both treatments superior to a
wait-list control group. They also found that behavior therapy was associated with greater improvements on measures of binge eating whereas cognitive-behavior therapy led to more improvement on certain attitudinal
measures at follow-up. Taken together, these results suggest that there is
no compelling evidence at this time for using behavior therapy in preference to cognitive-behavioral therapy in the treatment of bulimia nervosa.
With regard to anorexia nervosa, there has been little controlled research comparing behavior therapy to other methods. There have been
strong proponents and critics of the operant methods to achieve weight
gain and, although there is some evidence that it may lead to more rapid
weight gain, there seems to be little correlation between speed of weight
gain and long-term recovery (Bemis, 1987; Garner, 1985). In a study of
the short-term effects of different behavioral programs, Touyz, Beumont,
Glaun, Phillips, and Cowie, (1984) found that a lenient behavioral approach led to greater weight gain than a strict program during hospitalization. Again, the durability of these findings remains open to question.
Bemis incisive review of the literature concluded that operant conditioning has established credibility as an effective, if not definitive, method of
short-term intervention and appears neither as promising nor as alarming
as it did to earlier commentators (1987, p. 457). However, there has been
little empirical progress since that earlier review concluded that the research record is not particularly impressive when stretched across the 20year span that has elapsed since the introduction of the approach . . . [and
that] continued criticism must be suffered with grace until fully adequate
trials have been conducted and reported (Bemis, 1987, p. 457).
Although there has been heated discussion from advocates and opponents of behavioral intervention to promote weight gain in anorexia
nervosa, the reality is that the systematic application of positive and negative reinforcement contingencies is not unique to behavioral technologists. The proper application of operant principles, within the context of
a responsive psychotherapy, is less controversial than the ongoing debate
implies. Nevertheless, research assessing the long-term effectiveness of an
operant approach is either long overdue or the rationale for abandoning
its study needs to be articulated more clearly. The use of behavior therapy
alone is not included in the decision-tree approach presented in Figure 3

because evidence for applying to eating disorders outside of the cognitive-behavior therapy context is insufficient.
Interpersonal Therapy

Although there have been no controlled comparisons between longterm psychodynamic psychotherapy and other forms of treatment, it is important to mention that there have been other well articulated dynamic
treatments that have been recommended on clinical grounds. These deserve mention since they may play a role in a decision-tree model based
on the clinical needs of individual patients. As with the discussion of other
treatment approaches presented earlier, it is impractical to provide an extensive review here; the reader is encouraged to consult original sources to
obtain a full appreciation of different psychodynamic treatments for eating
disorders (Johnson, 1991).
Psychodynamic treatment for eating disorders may be roughly divided
into two schools of thought. The first presumes eating disorders do not
require fundamental modifications to orthodox dynamic interventions because neither the symptoms nor the disorders represent a unique underlying
process. The second conceptualization implies that eating disorders are distinctive in that they require major modifications to traditional dynamic
therapy to meet the special psychological needs of the patient. Bruch
(1973) was one of the earliest dissenters from strictly interpretive dynamic
approaches, asserting that these undermine the patient's self-initiated behavior and unwittingly recapitulate earlier interactional themes that played
a role in the development of the eating disorder in the first place. Bruch

Feminist Therapies
Therapists who treat eating-disordered women must be sensitive to
how women have been affected during the past two decades by oppressive
gender roles as well as the tension inherent in rapid evolution of social
roles. There has been a move beyond a general or gender-free psychology
toward emphasis on qualitative gender differences and how these are especially important in understanding and treating eating disorders. A new
generation of feminist therapists deviate from the traditional views by incorporating the growing literature on the psychology of women dynamics
into formulations regarding the etiology and treatment of eating disorders
(Fallon, Katzman, & Wooley, 1994; Wooley & Lewis, 1989). These theorists emphasize sociopolitical themes that should be considered (Perlick &
Silverstein, 1994; Steiner-Adair, 1994), therapist gender and treatment
(Katzman, 1994; Wooley, 1994), and the need to realign mainstream psychology to include the feminist perspectives (Striegel-Moore, 1994). They
have highlighted the importance of addressing issues such as role conflicts,
identity confusion, sexual abuse, and other forms of victimization in the
development, maintenance, and treatment of eating disorders (Wooley,
1994). Treatments are described in interactive terms, emphasizing the importance of womens interpersonal relationships. While controlled studies
are still lacking on the efficacy of feminist therapies, this is true for many
other treatment approaches to eating disorders. Recent advances in articulating feminist therapeutic principles as they relate to eating disorders
(Fallon et al., 1994) set the stage for evaluative studies using methods and
measures that better address feminist views on treatment. At this time, the
therapists orientation, rather than the special needs of the patient, probably determine the decision to apply feminist therapy to eating disorders.

However, many of the views articulated by feminist therapists have influenced the applications of other forms of therapy. Feminist therapy offers
specific treatment principles which are designed to assist patients who have
a history of physical or sexual abuse (Fallon et al., 1994).
Pharmacotherapy
Discussion of pharmacotherapy for eating disorders cannot be anything but cursory here. However, there are a number of basic principles
that can be derived from the literature (Mitchell et al., 1993; Raymond,
Mitchell, Fallon, & Katzman, 1994). Regarding anorexia nervosa, there
has been no significant pharmacological advancement to contradict earlier
suggestions that pharmacotherapy has limited value with emaciated patients and should never be the sole treatment modality (Garfinkel & Garner, 1982). At times patients may benefit from medication to deal with
overwhelming anxiety, severe depression, or intolerable gastric discomfort
after meals, but this applies only to a small minority of patients (Andersen,
1985; Garfinkel & Garner, 1982).
The research on drug treatments for bulimia nervosa has been extensive in the past decade and leads to more complicated recommendations
than with anorexia nervosa. Most of the research has been on antidepressant medications, with indications of success from some studies (e.g., Agras
et al., 1992). However, for a number of reasons, we agree with the recommendation that medication should not be the primary mode of therapy
with patients with bulimia nervosa (Raymond et al., 1994, p. 241). The
reasons include (a) the effectiveness of psychological interventions, (b) the
high dropout rates reported in most medication studies, (c) the risks of
drug side effects, and (d) data suggesting high relapse rates with drug discontinuation. Fluoxetine hydrocholride (Prozac) is currently the first
choice for treating bulimia nervosa (daily dosages of 60 mg were generally
superior to 20 mg) and probably should be used at least as an adjunct to
psychotherapy in cases that do not respond to a course of adequate psychological treatment (see Mitchell et al., 1993). Tricyclic antidepressants
may be an alternative for some patients failing a course of fluoxetine, but
side effects, high dropout rates, and greater incidence of lethality with
overdose may be a source of concern when treating some patients (Leitenberg et al., 1994; Mitchell et al., 1990; Walsh, Hadigan, Devlin, Gladis,
& Roose, 1991). In a similar way, Monoamine Oxidase Inhibitors may be
useful in a small minority of patients who fail using fluoxitine and tricyclics.
There is some evidence that patients who fail on one tricyclic may benefit
from changing to an alternative medication (see Mitchell et al., 1993).
However, the decision to avoid prescribing medication may be therapeutic
in some cases (Raymond et al., 1994).

because they are cost effective and less intrusive. Although there is some
information on costs and effectiveness of several treatments, cost-effective
analyses per se have not been done. Just because a treatment appears economical does not mean that it is cost-effective. Less intensive treatments
actually may be more than simply ineffective because their failure might
lead to the patient being more resistant to subsequent interventions. At
this time, the method best studied in relation to bulimia nervosa is cognitive-behavioral therapy, and this method has been shown to benefit most
patients with eating disorders. It could be argued that, with the exception
of the small number of bulimia nervosa patients who require hospital or
day care management, all other patients should be exposed to an initial
trial of cognitive-behavioral therapy as the treatment of choice. In addition to the theoretical arguments, this has practical advantages because it
is unrealistic to expect each clinical setting to have the resources to train
staff competently to apply the broad range of treatments outlined earlier.
It is also important to point out that some of the more accepted
treatments proposed for bulimia nervosa have gradually been integrated
and consolidated. For example, the educational component of cognitive-behavioral therapy was originally quite rudimentary, but as the informational base on educational treatment has expanded, it has taken a
broader place in the standard cognitive-behavioral therapy for eating disorders. In a similar way, there has been increasing agreement in the content
and the format of self-monitoring in the treatment of bulimia nervosa. This
may be extrapolated even further because there should be few practical
problems associated with integrating some of the elements that seem to be
most useful in interpersonal psychotherapy treatment into an enriched cognitive-behavioral therapy treatment model. This is not inconsistent with
the cognitive-behavioral therapy model that addresses relationship problems when they are a source of emotional distress. This is a particularly
important target for change in patients with personality disorders (Beck,
Freeman, & Associates, 1990) .
One could start from the interpersonal psychotherapy model and incorporate elements from cognitive-behavioral therapy that seem particularly useful. The content of therapy would focus on interpersonal relationships with standard cognitive-behavioral therapy methods integrated as
needed. It must be cautioned that this mix-and-match model presupposes
that merging models does not have a detrimental effect. I t may be that the
components are really not complimentary or that integrating treatments
dilutes the overall efficacy.The worst connotation of eclecticism is that it
reflects a failure to understand and cogently apply any one technique. By
trying to mix and match methods for bulimia nervosa patients, it may be
that training therapists and implementing treatment would be weakened
somehow.

One of the greatest pitfialls of the stepped-care and decision-tree models of treatment is that their complexity makes rigorous research on effectiveness exceedingly difficult. The ideal research protocol would allow
testing treatment efficacy for each treatment separately and in combination. It also would allow examination of the effects of treatments presented
in different sequences. Finally, complete information would be available
concerning the interactions between each treatment (and treatment combination) and relevant patient characteristics. Needless to say, the resulting
hypothetical matrix of treatment combinations and patient features would
be enormous and completely impractical.
A more realistic approach is to conduct studies using larger sample
sizes and compare different treatment sequences that are particularly relevant from a clinical point of view. It may be practical to assess the relative
efficacy of the best shot treatment compared to predetermined and limited
decision-tree approach. For example, it would be feasible to compare standard cognitive-behavioral therapy for bulimia nervosa with an approach
that systematically adds specific components such as educational treatment,
cognitive-behavioral therapy, interpersonal psychotherapy, and drug treatment.
CONCLUSION
This chapter has attempted to refine and extend the decision-tree
and stepped-care models for eating disorders. Although all available treatment options have not been covered, the main treatments have been recommended based on patient-clinical features and response to treatment.
We hope that this will be a useful model for making students and beginning
clinicians aware of the decision-making process used in considering the
major treatment options available.
