Contributor Release Form

We are AS Level Media Students producing an opening sequence for a thriller

film. In order to meet the legal permission requirements for your contribution,
please read the following terms and conditions then sign below:

terms and conditions:
1. Any contribution of sound may be recorded and included within the
producers thriller opening sequence, and can be edited and used in any
way the producers wish within their piece.
2. Not all that I contribute has to be featured within the piece and can be cut
and edited accordingly to suit the needs and desires of the producers.
3. Any contribution of footage featuring myself may be recorded and
included within the producers thriller opening sequence, and can be
edited and used in any way the producer may wish within their piece.
4. My contributed footage may be exhibited on all media and formats.
I hereby assign to you, the producers for this piece, the entire copyright of my
contribution to the programme:
