It is defined as A complete removable partial denture constructed for insertion
immediately following the removal of natural teeth - GPT. They are given in cases having teeth with poor prognosis. The denture is inserted on the day of extraction. It is of two types, namely temporary immediate partial denture and permanent immediate partial denture. Advantages It is more aesthetic (improves patientpsychology). Prevents supra-eruption, and drifting of the opposing and adjacent teeth respectively. Haemorrhage and swelling is controlled because it acts like a splint over the surgical site (also refer immediate complete dentures). Temporary Immediate Partial Denture These are given in cases where the permanent immediate partial denture is likely to become ill fitting due to more than normal bone resorption etc. The temporary immediate partial denture is fabricated only in acrylic resin. Permanent Immediate Partial Denture These are given in cases where the bone resorption is expected to be less and the denture prognosis is good. It is fabricated using the following design considerations: Retainers Retainers are made using wrought wire and are adjusted at the time of insertion for adequate retention. Framework A framework is constructed and tried-in before extracting the teeth. The teeth to be extracted are cut in the cast. The denture base minor connector (retentive loop) is soldered to the framework after try-in. Guiding Planes During framework try-in, the teeth to be extracted are sliced proximally to widen the interdental spaces. This helps to give clearance to prepare guiding planes on the abutment teeth and also to provide space for the framework to seat easily during try-in. Fabrication Technique An impression is made and the master cast is poured using a two-pour technique.

Cutting the Teeth on the Master Cast
The tooth to be extracted is cut up to the level of the gingiva in the cast. This will leave a rough area in the cast. The center of this rough area is made deeper to resemble a shallow tooth socket. After cutting the teeth, the cast is scrapped in the facial surface of the tooth socket. It is scrapped over an area of 4-5 mm to a depth of 1 mm (Fig. 22.67). This is done so that theprepared denture will compress the site of extraction and act like a surgical splint. The prepared master cast is used to fabricate the denture. For permanent immediate partial dentures, the metal framework is fabricated and tried-in before extraction. The artificial teeth are arranged and directly acrylized without completing aesthetic try-in. The dentures are trimmed, finished and polished. Insertion Disclosing wax is coated on the tissue surface to detect pressure points. Usually the prognosis of a permanent immediate partial denture is poor. Hence temporary immediate partial dentures are preferred.
