For good and valuable consideration, including but not limited to possible exposure for the Material (as defined
hereafter), and for other good and valuable consideration, the receipt and sufficiency of which are hereby
acknowledged, we are making the representations, warranties, disclosures, and agreements set forth in this
Participant Agreement and Parental Consent.

A. Eligibility and Participation

1. I am the parent or legal guardian of the Child, a minor, participating in the OTWOLISTA (the Promo)
implemented by ABS-CBN Broadcasting Corporation, Digital Media Division (ABS-CBN). I will supply a copy of
my Child's birth certificate or such other documentation as ABS-CBN may require to confirm such.
2. I hereby consent to the participation of my Child in the Promo including my Childs submission of usergenerated content (photo/video).
3. I acknowledge that my Child is voluntarily participating in the Promo, without any compensation and assume
all risks arising from or associated with the participation in the Promo. As such, we my Child and I
acknowledge that we have read and understood the procedure, rules and other material terms for participating in
the Promo (the Terms) and we accept and agree to be bound by the Terms.
4. I am aware that ABS-CBN shall have full and final authority to determine my Childs eligibility and qualification
to enter into and participate in the Promo, including the determination of the winners of the Promo. I agree and
accept that ABS-CBNS decisions on all matters related to the Promo (including without limitation the awarding of
any and all prizes, and decisions regarding the creation and implementation of terms, conditions and rules
governing the Program) shall be within the Sponsors sole control, and shall be final and binding on me and my
Child and will not be challenged or appealed.

B. Warranties
5. I hereby warrant that my Child has the full power and authority to enter into and participate in the Promo, that
my Child has no agreement with any person, firm or corporation which will in any way interfere with his
participation in the Promo, and that my Child is qualified to join the Promo in accordance with the Terms. I further
warrant that neither I nor any member of my immediate family (i.e., spouse, mother, father, daughter, son, sister or
brother) nor any members of my household (whether related or not) is or has been within the past year an
employee, officer, director, representative or agent of the Sponsors, or its subsidiary and affiliated companies, or is
related to any employee of ABS-CBN or its subsidiary and affiliated companies.

6. I represent and warrant that I and/or my Child has no knowledge of or access to, and I do not know anyone
with knowledge of or access to, any material, assignments, challenges or prizes that may be selected for possible
use in the Program.
C. Non-Disclosure
8. I acknowledge that I and/or my Child may be given access to or become acquainted with confidential
information and trade secrets of ABS-CBN and the Promo. We shall not disclose directly or indirectly to any third
party any such confidential information and trade secrets of ABS-CBN, including the terms of this Participant
Agreement and Parental Consent and the terms and conditions of any contract that we may execute with ABSCBN. This obligation shall survive my Childs participation in the Promo, and will continue even if my Childs
Material was not chosen as winner in any of the categories of the Promo.
9. I and/or my Child shall not make public any complaint or grievance about the Promo, including any decision
made in the Promo, and will take this up directly with ABS-CBN or its proper officers.
