San Lazaro Hospital (COMMUNICABLE DISEASE)
First day of our duty at San Lazaro Hospital, we attended the flat ceremony at 8:00 in
the morning the after the flag ceremony is our general orientation in the hospital at
8:30am on the amphitheater of San Lazaro hospital and after the orientation we took a
tour at the hospital. We go to the different departments and after we took a tour
discussion of requirements
Second day of duty at San Lazaro Hospital, we go to the nursing training office and then
our clinical instructor discuss about the activities and ward where we will be assigned,
after discussing the activities we go to 4th floor pedia ward and the receiving of our
patients, checking the IVF, taking vital signs, making FDAR and after the we took a
break, after the break reporting of the students for some specific topic MEASLES and
PNEUNOMIA.
Third day of our duty at San Lazaro Hospital, we go again at the nursing training office
at the third floor. And then at around 8:00am at the pavilion 6 adult male on second floor
we receive our patients, introducing myself to my patient and then asked the patients

name, checking of IVF, taking vital signs of the patients, making FDAR, and changing
the patients IVF bottle.
Fourth day of our duty at San Lazaro Hospital, we go at the nursing training office at the
third floor. And around 8:30am we go to PHILHEALTH ward pavilion 7. And we receive
our patients introducing myself and asking the patients name, after asking my patients
name. checking the IVF, taking vital signs of the patients, FDAR making, and changing
IVF bottle of the patients.
It is good having our clinical rotation at the San Lazaro Hospital because we acquired
knowledge about the different communicable diseases such as chicken pox,
pneumonia, measles, cholera, tuberculosis, rabies, leptospirosis, meningitis and
dengue. Are clinical instructor teach us and shown us all that diseases. She did not let
us finished are shift without knowing it.

V-LUNA (NEURO)
First day of our duty at V-Luna on Neurosurgical ward we login at the training hall and
then we go to 4th floor at neurosurgical ward and our clinical instructor assigned us the
cubicles that we will going to handle I was assigned at the pediatric room. Receiving my
patient and I introduce myself on the patients and after introducing myself checking of
IVF, taking vital signs of the patients, monitoring input and output of the patients and
looking at the patients chart and checking the cases of the patients that is assigned to
me
Second day of our duty at V-Luna on Neurosurgical ward we login at the training hall
and go to 4th floor at neurosurgical ward and I was assigned on neurosurgical ICU and
receiving my patient who is unconscious. Checking of IVF and taking vital sign of the
patient every hour and also performed CPR and ambu bagging on the other patient.

patients
Third day of duty at V-Luna we login at the training hall and go to 4 th floor at
neurosurgical ward and I was still assigned on neurosurgical ward ICU and receiving

again my patient who is still unconscious. Checking of IVF and taking vital signs of the
patient every hour. Charting and flatting on the chart of the patient and monitoring input
and output of the patients.
Fourth day of duty at V-Luna logging in at the training hall and go to 4 th floor at
neurosurgical ward and I was assigned at cubicle A. and receiving my patients,
introducing myself and asking my patients name after asking the names of patients.
Checking the IVF of the patients and taking vital signs and after taking vital signs
charting and flatting on the patients chart. And also monitoring input and output of the
patients
Fifth day of duty at V-Luna logging in and go to 4th floor at neurosurgical ward and I was
still assigned at cubicle A and morning care was done, checking of patients IVF, taking
patients vital sign, monitoring input and output. Charting and flatting on the patients
chart.
Sixth and last day of our duty at the neurosurgical ward logging in and go to the ward. I
was assigned at cubicle B. and receive my patients. Introducing myself and asking the
patients name after asking the patients name. checking the IVF of the patients, taking
vital signs and monitoring input and output. Charting and flatting on the patients chart.
Our clinical instructor teach us the different cases in the neurosurgical ward and the
different complications of the patients in the ward.

V-LUNA (ORTHO)
First day of our duty at V-Luna we login on the training hall and after logging in we go to
3rd floor at Orthopedic ward when we arrived at the ward we listened on the
endorsement of the nurses and after endorsement our clinical instructor distributed as
on the different wards on the area I was assigned at the cubicle A. receiving my patients
and introducing myself and after introducing myself I asked my patients name. after
asking checking if my patients has IVF and then taking vital signs and after taking vital
signs charting and flatting on the patients chart. and after charting we do bedside care,
assisting the patients and interviewing the patients.
Second day of our duty logging in and go to 3 rd floor at Orthopedic ward and we listened
again on the endorsement after endorsement go to the cubicle where I was assigned.
Morning care for the patient, assisting the patients on the comfort room. Taking vital
signs, charting and flatting on the patients chart and bedside care.
Third day of duty logging in and go to 3rd floor on our ward and listened on the ongoing
endorsement and then after endorsement going to the cubicle and performed morning
care for the patients and assisting them. And after that taking patients vital sign and

giving patients medication for the day. Charting and flatting on the patients chart after
taking their vital signs. And on also giving the patients food on the lunch time. And also
bedside care.
Fourth day of duty at V-Luna on the orthopedic ward. Listening on the endorsement and
after endorsement preparing the patients medications on the nurse station and after
preparing the patients medication, distributing the medications in all the cubicles on the
ward. And after distributing the medication assisting the patients and morning care.
Taking vital signs, charting and flatting on the patients chart. and we do bedside care
and giving the patients food for lunch.
Fifth day of our duty logging in and going to the ward. Listening on the endorsement
after endorsement we go to the cubicle where I was assigned and performing morning
care and assisting the patients. Taking vital signs, charting and flatting on the patients
chart and bedside care. Assisting my one of my patient on the x-ray department for
some analysis.
Sixth and last day of our duty at V-Luna on the Orthopedic ward. Listening on the
endorsement performing morning care and assisting the patients. Taking vital signs,
charting and flatting on the patients chart and bedside care.
We learned a lot from our clinical instructor because she teaches us the different cases
and injuries on the patients on the orthopedic ward.

NCMH (Pavilion 3 - Female)
First day of our duty at NCMH weve been oriented about the hospital policies and our
pre-orientation. After the orientation attended the flag ceremony and after the flag
ceremony we go to our pavilion at pavilion 3 and our clinical instructor discussed us the
general orientation and after the general orientation is our ward orientation and planning
of activities , discussion of requirements, distribution of patients assignment. And writing
of our autobiography and self-awareness activity
Second day of our duty at NCHM, we continue our self-awareness activity and after the
activity we have a pre-test and review of the pre-test exam. And after the setting of
objectives and review of NPR, communication techniques and process recording,
psychosocial assessment, mental status examination and NPI orientation phase after
that reporting and discussion of the assigned psycho therapeutic activity dance aerobics
and recreational activity.
Third day of our duty at NCHM, continuation of reporting of the assigned psycho
therapeutic activities and after discussing we go to the ward and we received our
patient. After receiving our patient we do our first nurse patient interaction which is the

orientation phase, introducing myself and asking the patients name. and discussing the
patient about the duration of our duty at the ward.
Fourth day of our duty at NCMH, continuation of reporting of the assigned psycho
therapeutic activities and after discussing the reports we go to the ward and received
our patients after we do our first activity for the day which is dance aerobics we and our
patients have the aerobics and after the dance aerobics is the second activity which is
recreational therapy we made some games for the patients and also after the activity we
give some foods and drinks for the patient. And after that is our second NPI for the
patient.
Fifth day of our duty at NCMH, continuation of reporting of the assigned psycho
therapeutic activities and after discussing the reports receiving again our patients the
we also do the dance aerobics after the dance next is the recreational therapy and
personal hygiene for the patients. And continuation of our NPI the after the NPI we go
into the ward and checked the patients chart and we do chart reading.
Sixth day of our duty at NCMH, staff nurse shadowing and preparation of activities.
Logging in and receiving the patients. Dance aerobics and recreational therapy was
done and after that music and art therapy and then personal hygiene for the patients.
Continuation of the NPI we are now at the working phase and the process recording,
NCP making, and writing of case.
Seventh day of our duty at NCHM, continuation of shadowing of the staff nurses,
preparations of the activity and getting the patients of their units. Psychotherapeutic
activities dance aerobics, recreational therapy and remotivation therapy and personal

hygiene for the patients. Continuation of NPI, process recording, NCP making and
writing of cases.
Eighth day of our duty at NCMH, staff nurse shadowing, preparation of activities and
getting the patients on their units. Psychotherapeutic activities dance aerobics,
recreational therapy and personal hygiene for the patients. Continuation of NPI, process
recording and writing of cases.
Ninth day of our duty at NCMH, pre-conference preparation for our grand socialization.
After preparation getting the patients on their units and we begin our grand socialization
with the patients. After our socialization, NPI on the patients termination phase and after
NPI post-conference, signing of RLE and completion of the requirements.
We learned a lot from NCMH and from our clinical instructor about the different cases
and disorders of the patients and the different psycho therapeutic activities.

EMMAUS (GERIATRIC)
First day of our duty at EMMAUS, we took a mass. And after the mass is our general
orientation at EMMAUS. Our clinical instructor discuss about the activities that we will
going to do at EMMAUS, and also discussion about the requirements and assigning of
our ward. After assigning is we took a tour in all the wards at EMMAUS. After the tour
we go to the ward where we are assigned. Receiving our patients and having
conversations with them. Introducing myself and asking my patients name.
Second day of our duty at EMMAUS, we go to our assigned ward and we do morning
care for the patients such as bathing, shaving our patients, feeding them and also giving
our patients medications. And taking the vital signs of our patients and after taking the
vital signs our clinical instructor discuss us the conditions of the patients.
Third day of our duty at EMMAUS, we do morning care for the patients. Bathing the
patients, shaving, putting lotions and powder on the patients. And after the morning care
we feed the patients for breakfast and giving them their medications and changing the
diaper of the patients. After the we took the vital signs of the patient. Checking the blood
pressure, pulse rate, respiratory rare and temperature of patient after the vital signs was

taken we feed the patients for their lunch and after feeding them we give their
medications.
Fourth day of our duty at EMMAUS, we do morning care for the patients. Bathing,
shaving, putting lotions and powder on the patients. After the morning care was done
changing the linens of the bed of the patients and feed them their breakfast and after
eating we give their medications and then we took the vital signs of the patient.
Assisting them on the bed and do bedside care. And feeding them again for their lunch,
giving their medications.
Fifth day of our duty at EMMAUS, we do morning care for the patients. Bathing,
shaving, putting lotions and powder on the patients. After morning care was done
changing the linens of the bed of the patient and feed the patients for breakfast and give
their medication. And after eating vital signs was done and assisting the patients on the
bed and we do bedside care. Feeding the patients for lunch, giving their medications.
And after giving medication our clinical instructor discuss us about the complications of
the patients.
Sixth and last day of our duty at EMMAUS, morning care was done. Bathing, shaving,
putting lotions and powder on the patients. Changing patients diaper and linens of the
bed. After morning care was done we feed the patients breakfast and give their
medications. And taking the patients vital sign, assisting the patients on the bed and we
do bedside care. On the afternoon we give the patients their lunch and feed them and
give them their medications. After the activity giving the requirements on our clinical
instructor and signing of RLE.
