Who should be assessed?

This Risk Assessment Checklist is aimed at young people aged 11-17 years.
The assessment is designed to be used by a professional who has a concern that a young person may
be at risk of early sexual activity leading to an unplanned pregnancy, contracting a sexually transmitted
infection or other sexual health issues.
This form is to be completed by professionals who already know a young person and build on
information/concerns they already have. This form should be completed with the young person in all
cases.
