Quality of Life in Patients with
Minimal Change Nephrotic Syndrome

1. Introduction
Longer life expectancy and advances in medical care have
increased the problems associated with chronic diseases in
many countries, including Japan. Chronic diseases have a
long duration and generally slow progression, and people
with these diseases desire to live longer and to live better. For
these reasons, quality of life (QOL) is an increasingly important issue in healthcare for chronic diseases.
We have an interest in studying QOL of patients with renal
diseases, as a typical example of chronic disease. The daily
life of patients with renal disease is often limited by fac- tors
that are common to other chronic diseases, and these
factors can easily decrease QOL. Furthermore, the number of
patients with renal diseases is increasing worldwide. In 2010,
about 297,000 people were receiving hemodialysis in Japan,
and globally more than 500 million people have some degree

QOL [37]. Therefore, the aim of our study was to investigate
QOL and the extent of self-care in adult patients with MCNS,
with the goals of clarifying the relationship between QOL and
self-care and using the results to suggest how to improve
QOL in this patient population.
3. Results
Of the 30 patients with MCNS, 27 lived with their family and
2 lived alone; 19 were employed and 9 were homemakers; 22
were taking medication and 7 were not; and 22 had experienced recurrence of MCNS and 5 had not had recurrence.
QOL scores for patients with MCNS compared with
Japanese normative data from 2007 for the general population
and for patients two or more chronic diseases are shown in
Table 2. The SF-36 health-related QOL score was obtained
from scores for all 8 subscales (physical functioning, physical
role, bodily pain, general health, vitality, social functioning,
emotional role and mental health), based on a 0- to 100-point
scale with higher values indicating better QOL. The physical
and mental component scores are calculated as summaries of
multiple SF-36 subscales. For comparison, in the normative
data for the general Japanese population, each subscale was
given an average score of 50 and a standard deviation of

4. Discussion
The results of the study revealed unique changes in QOL in
patients with MCNS and a relationship between the levels
of QOL and self-care behavior. The patients had lower QOL

1. Population
Penelitian ini dilakukan terhadap 30 pasien yang di rawat jalan dengan Minimal Changes
Nephrotic Syndrome (MCNS) di Hirosaki University Hospital, Jepang.
2. Intervention
Penelitian ini hanya mengambil data sesuai dengan kriteria inklusi dan eksklusi tanpa dilakukan
intervensi.
3. Comparison
Penelitian ini membandingkan kualitas hidup pasien dengan MCNS menggunakan kuisioner
SF-36v2 dan The Self-Care Behavior Scale kemudian hasilnya dibandingkan dengan kualitas
hidup penduduk Jepang secara umum dan pasien dengan 2 atau lebih penyakit kronis
berdasarkan hasil penelitian sebelumnya.
4. Outcome
Pada pasien MCNS, indikator fungsi sosial yang paling terganggu dan indikator bodily pain
yang paling tidak terganggu. Self-care yang lebih baik dapat berpengaruh positif pada kualitas
hidup pasien dengan MCNS.
5. Validity
a. Apakah fokus penelitian ini sesuai dengan tujuan penelitian?
Ya, karena tujuan penelitian ini untuk investigasi atau menilai kualitas hidup pasien dewasa
dengan Minimal Change Nephrotic Syndrome dan menilai hubungan kualitas hidup pasien
dengan tingkat self-care
b. Apakah subjek penelitian diambil dengan cara yang tepat?
Ya. Subjek penelitian pada penelitian ini sebanyak 30 pasien dengan dengan MCNS di RS.
Unniversitas Hirosaki. Pasien (rata-rata usia 17 tahun) terdiri dari 16 laki-laki (umur 19
tahun) dan 14 perempuan (umur 15 tahun).
c. Apakah data yang dikumpulkan sesuai dengan tujuan penelitian?
Ya, data yang dikumpulkan sudah sesuai dengan tujuan penelitian.
d. Apakah penelitian ini mempunyai jumlah subjek yang cukup untuk meminimalisir
kebetulan?
Ya, penelitian tidak membutuhkan besar subjek minimal.
e. Apakah analisa data dilakukan cukup baik?
Ya, analisis sudah dilakukan dengan cukup baik dengan menggunakan analisis univariat dan
bivariat. Analisis univariat digunakan untuk mendeskripsikan karakteristik pasien dan
analisis bivariat dengan korelasi digunakan untuk membuktikan hubungan bermakna
beberapa item yang berhubungan deengan skala kualitas hidup pasien.
6. Importance
Apakah penelitian ini penting?
Ya. Penelitian ini penting karena menilai tingkat kualitas hidup pada pasien dengan MCNS dan
hubungan kualitas hidup dengan self-care yang dapat mempengaruhi progresifitas penyakit
pasien tersebut dan aspek penting dari segi kualitas hidup manusia terutama komunikasi, faktor
sosial, dan faktor fungsional (kesehatan mental), dan beberapa indikator kualtas hidup lainnya
sehingga keadaan tersebut bisa diminimalisir dengan upaya meningkatkan vitalitas dan
kesehatan mental yang berpengaruh terhadap progresifitas penyakit pasien seperti bisa
dilakukan konseling ke psikolog atau dengan rehabilitasi medik jika diperlukan.
7. Applicable
Apakah penelitian ini dapat diaplikasikan?
Ya, hasil penelitian ini dapat digunakan sebagai suatu acuan untuk penelitian selanjutnya
mengenai kulaitas hidup pasien dengan MCNS (ataupun penyakit lain), terutama karena
penelitian ini diambil pada usia rata-rata 17 tahun yang merupakan usia remaja yang sangat
memerlukan konseling psikologi.
