Nurse Licensure Examination
Notice is hereby given that the registration for the nurses who passed the NOVEMBER 2015 Nurse
Licensure Examination will run from January 19-22, 2016

THE SCHEDULE BELOW IS ONLY FOR THOSE WHO

TOOK THE EXAMINATION IN LUCENA CITY.
Please bring the following:
a) Downloaded OATHFORM (http://www.prc.gov.ph/online/registration)
b) Registration / Annual Fee of Php 1,050.00 and;
c) Current Community Tax Certificate (Cedula)
d.) your NOTICE OF ADMISSION (NOA) for easy verification (optional)

IMPORTANT: Nurse Registrants are advised to strictly observe the schedule to
avoid any inconvenience.
STEPS IN REGISTRATION:
1. Download & print OATH FORM at http://www.prc.gov.ph/online/registration
2. Submit properly filled out Oath Form at Window 4 for assessment of fees
3. Pay registration/annual fee at Window 17
4. Secure documentary stamp at the Photo Booth (in front of Window 17)
5. After payment, submit OATH FORM at Window 4
Wait to be called at Window 4 to sign on the Registry Sheet
6. After registration, receive your claim slip and claim your professional ID on scheduled
date

For those who want to apply for CERTIFICATION of PASSING, RATING and
GOODSTANDING:
7. Secure ACTION SHEET from the Public Assistance and Complaints Desk
8. Accomplish the ACTION SHEET properly and present this at Window 4 for assessment of fees
9. Secure documentary stamp at the Photo Booth
10. Pay certification fee at Window 17 (PhP75.00 per copy)
11. Wait to be called for the release of the certification.
