I, the undersigned, being duly sworn and under oath, state that the following facts known by me or told to me by the other
reliable persons form my belief that Melissa Jean Thomas the defendant committed the crime charged.
On Sunday, January 17, 2016 at approximately-2257 hours, I was dispatched to the .residence of 11014 N.E. State Street In Sac City for
a noise disturbance. On the scene, I spoke with the residence owner and the defendant. Both subjects acknowledged that Thomas had
been yelling and playing music loudly. I advised to keep the noise down and turn off the music. At approximately 2324 hours, I was
dispatched to the same location for another noise complaint. Thomas was In the apartmentldhe with the music turned up. I placed her
under arrest. She was taken to the Sac County Law Enforcement Center where she was "charged with Disorderly Conduct and jailed.

The court handled the defendant's initial appearance by telephone on January 18, 2016. The court
advised the defendant of the nature of the charge, her rights and advised her that if she wished to
plead guilty that the court would sentence her to serve 48 hours in the Sac County jail and would tax
the court cost to her. She advised the court that she did wish to enter a guilty plea and that plea was
accepted. The court then sentenced her to serve 48 hours in the Sac County jail with credit for time
already served and assesses the court cost to her. Those costs shall be paid on or before February
29, 2016 or she shall appear and show cause on March 1, 2016 at 10:15 AM.
