SubsequenttoApplicationofZythorandDuringAerationandClearance:
(1)

failingtohavetwopersonstrainedintheuseoftherestrictedusepesticide,
present on site from the first opening of the tent seal until the
securing ofthe structure atthe end ofthe initialaeration period;

(2)

failing to use a required,approved respiratory protection deviceto enter

the fumigated space atthe firstopening of the tent sealand during the
aeration procedure when the concentration ofthe restricted use pesticide
within thebreathing zoneofthefumigated space isunknown;

(3)

failing to aeratethe fumigated space with a1loperableexteriorwindows

and doorsopen,aided by theuse ofoneorm ore fanscapableofdisplacing
atleast5,000 cubicfeetofairperm inute,foram inimum ofonehour;

(4)

failing to post standard warning sir s around the fumigated structure

during the rem oval of the tent and ensuring those w arning sir s rem ain
posted until aeration w as com plete and final clearance for re-occupancy

following a complete aeration procedure by sam pling the airwithin the
