CATALOG DESCRIPTION: This course focuses on the physiological and psychological
aspects of the mental health related to acute and chronic health problems across the lifespan. The
role of the professional nurse in health promotion and illness prevention are key components.

Emphasis is placed on the mental health milieu that exists in both the hospital and community
setting.

COURSE STUDENT LEARING OUTCOMES:
Student Learning Outcomes:
At the end of this course, the student will be able to:
1. Identify signs and symptoms of mental illness disease processes in the client.
2. Establish nursing diagnosis for the adult client in the mental health care setting.
3. Assist the client in setting realistic goals for their plan of care.
4. Implement nursing interventions to assist the client in the acute care setting, as well as the
community setting.
5. Evaluate the clients response to the nursing interventions.
6. Advocate for the clients rights while receiving care for their mental illness.
7. Apply ethical and legal principles when intervening with the client.
8. Incorporate the clients cultural needs when planning and implementing nursing
interventions for the client.

Attendance:
Class (Face-to-Face and Online) Student Attendance
1. The student must attend the class (es) for which he/she is enrolled.
2. A student enrolled at Wayland Baptist University should make every effort to attend all
class meetings. All absences must be explained to the satisfaction of the instructor, who
will decide whether the omitted work can be made up. If enrolled in the online course
weekly online participation to include appropriate academic construction of
Discussion Postings is required. Refer to the Discussion Posting Rubric that is
contained within this Syllabus.
3. A face-to-face student who misses twenty-five percent (25%) or more of the regularly
scheduled class meetings will receive a grade of F for that class. An online Student not
engaging in the weekly discussions will be considered Absent and thus more than 2
weeks absent of online engagement would be considered a failure for the online
course
4. When a student reaches a number of absences considered by the instructor to be
excessive, the instructor will so advise the student and file an Unsatisfactory Progress
Report at the San Antonio Campus.

Clinical Attendance & Attire


Attendance to clinical is mandatory. This is the opportunity for the student to learn and practice
skills in the Skills Lab and discussed in didactic then apply them in a leadership role or in patient
care. Any student missing a clinical must call in to the instructor prior to the scheduled clinical.
Clinical cannot be made up; therefore attendance is critical to successfully complete required
clinical objectives. There is no acceptable reason for missing clinical.
Solid white professional uniforms and shoes are required (unless your Clinical Instructor has
provided you different clinical attire requirements such as in the case of Navy Blue Scrubs for
S.W. General Hospital). School patches must be attached to the white uniform tops and lab
coats. Patch must be purchased at WBU bookstore. Students who are required to wear the Navy
Blue Scrubs are required to have the Wayland Nursing Student ID Badge at all times during
Clinical.

Final grade will be rounded to the nearest .10 decimal place. .5 and above rounds to the NEXT
whole number. For example: If your grade is 89.49 the grade will be rounded to 89.4 which
equals a B. 89.50 will be rounded to 89.5 which rounds to 90 which equals an A.

*All Students are required to complete Focused Reviews on All ATI Practice and Proctored
Tests*
Testing: Online Students are required to take Course Exams utilizing the Secure Proctor Device.
It is the students responsibility to ensure that his/her Secure Proctor Device is functioning
appropriately. The Exams will be open with a very limited time-window. All Students will be
required to take the Exams at the same time.
Test Chapters will be published in the course syllabi for the students at the beginning of each
term. Test dates are found within the Student Calendar. Exam blueprints may be provided by the
instructor prior to each exam. The Online Students will continue to use Proctor Secure as the
secure mode for testing. Students are required to follow the mandates as identified by Proctor
Secure as this is a security camera that videos the student during testing which is then sent to the
Dean of Nursing if questionable behaviors are videoed. Refer to the Student Calendar for testing
dates. The Exams will only be offered one time (will be opened at 8:15 pm CST and will
close at 9:45 pm CST). Student is to follow the Student Calendar for the dates when the various
Unit Exams and Final Exam will be administered. If the Student fails to take the Online Exam at
the time it is offered, the Student will take an alternate paper test at the next Clinical Weekend on
the Sunday after all students have been dismissed. The Alternate (Make-up) Test will be between
10 to 20 Questions in length and will be questions that may require answers of: Listing, Short
answer, Matching, Multiple Choice / Multiple Selection. There will be no Blue Print for
Alternate (make-up) Tests but the content will come from the same chapters of the Online
Exams. If the Student does not log onto the Final Online Exam at date/time being administered,
the Student may place him/herself in a situation whereby he/she will have to arrange to make an
additional trip to San Antonio to take a paper-pencil make-up Final Exam which will have to be
taken prior to the current academic Quarter closing out. Again the Final Makeup Exam will not
be the same Exam as the Online Final and will be similar question profile as the Unit Makeup
Exams (10 to 20 Questions in length and will be questions that may require answers of: Listing,
Short answer, Matching, Multiple Choice / Multiple Selection. Due to multiple occurrences
whereby Students in the past have not been taking exam(s) when the exam(s) have been
assigned, it was decided that the above was the best option to deter students from negating
academic responsibilities. Online Students are permitted to have only 1 make-up exam
occurrence; thereafter, the Exam Grade assigned will be 0 (ZERO) for missed online Exams
(this also includes the Final Exam).It is your responsibility to ensure your Remote Secure
Proctor Device is functioning appropriately. It is also your responsibility as an Adult
Learner to view your Student Calendar as well as check your Blackboard Course
Announcements at least twice a day, once in the early morning and then prior to you going
to bed at night in case there have been changes. Online Students will receive the Exam
Grade once the Student submits the Exam. No reviews of Exams are permitted by students
due to having to maintain Test Security.
Make-up Exams (On-Line Students): If a Student fails to log in to take his/her Exam at the
Time that the exam is scheduled, the Student will be required to take an alternate Exam. An
alternate exam may contain a variety of question types: Listing, Fill in the blank, Essay as well
as the typical Multiple and Multiple Select. While a Make-Up Exam will cover the same chapter
content, no Exam Blue Print will be provided. Student is required to take the Make-up Exam

within seven (7) Days of the date of the missed Exam. Failure to follow up with the Course
Coordinator will result in a Zero (0) being assigned as the Exam Grade. The Student is required
to telephonically contact and speak to the Course Coordinator, Dr. Diaz.
The Face-to-Face Students may have either a computer-generated or a paper-version of the Unit
Exam and will utilize the appropriate Scantron Sheet to complete the Unit Exam if the Exam is
the paper-version. Refer to the Student Calendar for specific times which the Exams will be
open if the Exam is administered as a computer-generated Exam (online students).
The Face-to-Face Student Calendar will be provided to the Students in class on the first day. The
on-line Student Calendar will be posted on the Blackboard Course Site during the first week of
class. In the event a faculty must change a test date, they will notify the students as soon as
possible. If the student must miss an exam, they must notify the faculty that they will not be
present for the exam and schedule a date for the make-up exam within one week of the date of
the exam.
Make-up Tests (Face-to-Face Students)
For Face to Face Students, occasionally a student will miss a test that is scheduled for a class. A
student should contact his/her instructor to arrange for the Make-up Exam. Test must be
completed within one week from date of missed exam. The Make-up Test may not follow the
Test Blue-Print, but will be over content covered for the specific chapters covered for the Unit of
Content.

Wayland Baptist University has partnered with Assessment Technologies Institute (ATI) to aide
students in their nursing studies and prepare them to take the NCLEX. In order for you to
become familiar with this site it is required that you complete the tutorial available at the

For the Mid-Term Proctored ATI Mental Health Test and the Final ATI Proctored Mental
Health Test, the Student will be required to complete Focused Review Time based on the Level
Score obtained. Based on the Level the Student earns, he/she will be required to perform the
following Focused Review Remediation Time in order to pass the course.
For a Level III: A minimum of 1 hour of Focused Review time is required
For a Level II: A minimum of 2 hours of Focused Review time is required
For a Level I:
A minimum of 3 hours of Focused Review time is required
For a < Level I: A minimum of 4 hours of Focused Review time is required
For the 2010 & 2013 ATI Practice Mental Health A & B Tests, the Student will be required to
complete Focused Review Time based on the % Score obtained on the first attempt. Based on
the % Score the Student earns, he/she will be required to perform the following Focused Review.
Once the required Focused Review has been completed on the first attempt, the student will need
to retake the Practice 2010 A & B Mental Health Practice Tests and the Practice 2013 A & B
Mental Health Practice Tests. The score achieved on the Second Attempt is what will be

calculated into the Grade Book. Additionally failure to complete the required Focused
Reviews will result in Clinical Failure. Both Theory and Clinical Instructors will be
monitoring ATI. Focused Review Time for the Practice Assessments is as follows:

A minimum of 1 hour of Focused Review time is required

For the ATI Tutorial Mental Health Tests (Tutorial Practice Mental Health Test 1 and Tutorial
Practice Mental Health Test 2 and the Tutorial Final Mental Health Test), the score achieved on
the First Attempt will be what is assigned as the Grade in the Grade Book.
ATI Research has demonstrated that Time spent in Focused Reviews has improved Student test
scores and NCLEX-RN out-come results.

Clinical Assignments
o Students may have the opportunity to participate in Simulation Lab experiences as
made available.
o Service Learning Engagement
Students as a Class may participate in sponsoring a Service Learning
Project supporting the Volunteer Council of the San Antonio State
Hospital.
o DocuCare Charting Activities based on a select Client where you are assigned
within your Clinical Site. (Specifics of the Charting will be discussed on the first
Clinical Day). The Assigned Clinical Instructor will be responsible for reviewing
the students DocuCare Charting.
Decompression Note: The Students will maintain a Clinical
Decompression Note by writing reflective annotations of the clinical
experiences/observations. This Journaling Activity for Clinical is a
mechanism by which Students can decompress his/her clinical

experiences. The weekly Clinical Decompression Note will be maintained


inside DocuCare. Explanations of DocuCare Charting will be discussed
on the first Clinical Day. The Assigned Clinical Instructor will be
responsible for reviewing the students DocuCare.
Students will complete/write out Medications Cards as directed by the Clinical
Instructor- it is recommended that Students hand-write Psych Drug Cards (a Total
of 30 Cards) These will be reviewed at Mid-Term (15 cards)and then again at the
Final Clinical Evaluation time by your Assigned Clinical Instructor (All 30
Cards).
Three Concept Maps based upon Video-Patient Simulation associated with your
Mental Health Textbook. You will find these within the online Student Resources
once you register your Textbook online at The Point. Scratch off the Student
Code inside the front cover of your Textbook and register with The Point. You
will be required to complete 3 of the Simulation Video Sessions associated
with your Mental Health Textbook. The Concept Maps will be submitted to
your Assigned Clinical Instructor (refer to the Student Calendar for date due).
The three Video-Patient Clinical Simulation Case Studies that you are required to
complete and develop a Concept Map on are:
Caring for the Client in an Acutely Manic Phase
Caring for the Client with Major Depressive Disorder
Caring for the Client with Paranoid Schizophrenia in the Acutely
Psychotic Phase
ATI Real Live Clinical Scenarios
The Student will be required to complete with a minimum score of
STRONG PERFORMANCE (100%) 4 Mental Health ATI Real Live
Clinical Scenarios. The Assigned Clinical Instructor will monitor the
Students progress by way of the Students ATI Transcript.
ATI Real Live Clinical Scenarios include:
o Schizophrenia
o Mood Disorder
o Anxiety Disorder
o Alcohol Abuse
Pre/Post Clinical Activity & Clinical / Theory Homework may include:
Special Topic Seminars associated with Mental Health and Mental Illness
The use of NCLEX RN 10,000. Both the Theory and Clinical Professors
will be monitoring Student progress with regards to the NCLEX RN
10,000. Recommendation is that the Student selects 20 questions at a time
as he/she is working toward the Master Level 8 achievement on the
various assignments. There will be a requirement of completing 5
Assignments based off of the Psychiatric and Mental Health Nursing
Component of NCLEX RN 10,000. The Mental Health Sub-categories

that the Student will need to achieve a Master Level 8 achievement


include:
Foundations of Psychiatric Nursing
Anxiety Disorders
Mood, Adjustment and Dementia Disorders
Psychotic Disorders
Substance Abuse, Eating Disorders, Impulse Control Disorders
Completion of all Focused Reviews based on Student Scores of ATI
Practice and Proctored Mental Health Tests
ATI Focused Reviews will be monitored by both the Theory
Professor as well as the assigned Clinical Instructor.
o Failure to follow and complete minimum time
constraints, will result in the Student receiving a NOPASS in clinical which would mean the Student would
have to repeat the Course
o The first Proctored ATI Mental Health Proctored Exam will be administered
at mid-term the second clinical weekend (refer to Student Calendar). The
first Proctored ATI Mental Health Proctored Test is NOT for score but the
Student is required to complete the required number of hours of Focused
Review time.
o The Final Proctored ATI Mental Health Exam will be administered during
the final clinical weekend and will be counted as Score which is worth 25% of
the Course Grade (refer to Course Grading Rubric, page 3 of the Syllabus).
The clinical instructor will provide feedback for student assignments within the clinical realm.
The theory instructor will provide feedback for student assignments within the theory realm.
Clinical:
The clinical portion of the course is on a pass/fail basis and must be passed to pass the course.
The clinical attendance, clinical objectives/student learning outcomes and clinical
assignments will be used to justify the students standing. The level two clinical evaluation
forms will be used to determine the students pass or fail performance for this course. The
student must be evaluated at 85% or above in the area of clinical performance in order to pass the
course.
Clinical Course Student Learning Outcomes At the end of the Course, Students will be
able to:
1. Explore subjective and objective feelings, attitudes, and behaviors toward clients with
mental illness.
2. Perform psychosocial and cultural assessments for clients with mental illness.

3. Apply ethical and legal principles along with professional standards while providing care
for clients with mental illness.
4. Use therapeutic communication while working with both clients and staff in the mental
health setting.
5. Apply principals of growth and development to the client with mental illness.
6. Demonstrate caring attributes when interacting with patients and family of patients.
7. Implement appropriate nursing interventions to promote well-being to the client in the
mental health setting.
8. Apply the nursing process to provide care for the clients with mental illness.
9. Explore community resources which provide care for clients with psychosocial issues.
Jurisprudence:
Students will need to purchase and/or down load and print the current Nursing Practice Act,
Nursing Peer Review, Nurse Licensure Compact, & Advanced Practice Registered Nurse
Compact as a supplementary resource reading.
Policy on Academic Integrity:
Standards of academic honesty are expected. Academic dishonesty includes, but is not limited to,
cheating, plagiarism, and counterfeit work, unauthorized reuse of work, theft, and collusion.
Policy Regarding Due Dates for Assignments: Assignments are due to faculty on assigned
dates at the beginning of the class period. Students are responsible for contacting the course
faculty if there is any difficulty in understanding the course materials or completing the course
assignments. Up to 10% may be deducted for each day of late assignments, late or incomplete
care plans may result in clinical failure.
Student Responsibilities:
Students are adult learners and responsible for self-directed study to complete this course. Check
postings on blackboard each week, learn to use blackboard postings provided by professors, as
they are intended to increase your professional nursing knowledge bank. Learn to become a
resource broker, speak professionally and use citations, it is part of professional nursing.
Assignments, blogs, and presentation have due dates and points will be reduced if not punctual.
The profession of nursing requires punctuality in documentation and attendance. It also requires
reliability as fellow staff and administration rely on you to come to work prepared. Patients rely
on you to know the current state of their health and current professional standards. Citations and
references are to be used and documented using the professional citation of nursing, APA (5th
edition) format.
Student Conduct:
This is a statement of conduct standards. It is enforced in conjunction with the Discipline
Policy and Substance Abuse Policy found elsewhere in this handbook.
Wayland proudly adheres to high standards of intellectual, moral, ethical, and spiritual values.
Convinced that self-discipline is more desirable than outside force and that the truly educated

person must pursue what is right under all circumstances, Wayland entrusts each student with the
solemn obligation of preserving these standards. However, in the light of revelation, reason, and
the custom of the Christian community from which Wayland has sprung, certain practices are
evaluated:
1. Demonstrating disrespect (also known as unprofessional behavior) either verbally or
written toward Faculty, other Students or Wayland Administration.
2. Personal integrity in keeping with New Testament standards is expected of all students.
3. Respect for the property, knowledge, and rights of other people must prevail.
4. The use or possession of alcoholic beverages and/or illegal drugs is forbidden.
5. Gambling, hazing, and the on-campus possession of firearms or deadly weapons are
prohibited.
6. BB Guns and Paint Ball Guns are also prohibited.
7. The use of tobacco by students is discouraged, though permitted in certain designated
areas of the campus. In consideration of the rights of others and the requirements of
safety, such areas are designated as per the Alamo University Center Requirements.
8. Usage of all forms of tobacco is strictly prohibited in dormitories.(Specific to students in
Dorms)
9. The use of tobacco by students who attend classes at the Alamo University Center in San
Antonio is discouraged, though permitted in certain designated areas of the campus (at

8 CHARACTERISTICS OF A QUALITY ONLINE DISCUSSION POSTING
1. SUBSTANTIAL: Messages should relate to the subject matter and provide information,
opinions or questions about that subject matter. They may relate the subject matter to something
personal, but they should remain academic in their focus.
2. CONCISE: Studies have shown that messages that are several screens long do not get many
replies. To write an effective message, attempt to use a single screen if possible. Try to get the
point and focus of your message across so that it is clear what you are saying.
3. PROVOCATIVE: The discussion board is an interactive medium. The more interaction there
is between students the better. A good message is one that prompts others to reply or object. A
focused and pointed message that produces replies from other learners in that class in moving the
discussion forward is having an impact on the learning environment.
4. HERMENEUTICAL: The discussion board is a place where ideas are interpreted and

language is explored. To be hermeneutical is to interpret. A good message should explore,


explain, or expand on a concept or connection. The message should not simply state something,
but rather expand upon an idea.
5. TIMELY: A good message appears in the context of similar messages in the message log. If
you get on irregularly, your message will appeal late and out of context with what is currently
happening on the board. Log on regularly and reply to messages in a timely fashion.
6. LOGICAL: A good message that is not a question should contain a logical argument. This

Standards: Examinations are important mechanisms for evaluation of student learning. The
College of Nursing programs adhere to standards of academic integrity as articulated by the
College of Nursing Honor Code and Wayland Baptist University Student Conduct Code.
Cheating on examinations is contrary to both academic integrity standards and to ethical
standards and constitutes reason for academic withdrawal and/or expulsion from the College or

Some courses in the College of Nursing use the Securexam Remote Proctor System. Securexam
Remote Proctor contains a webcamera, microphone and fingerprint reader. These devices
identify the student and capture video and audio throughout the exam and upload it to SSIs
secure servers. This video will be used for the purpose of establishing if any suspicious activity
occurred during the test. The video feed provides a 360 degree view of the exam environment
and all video records are subject to review by faculty.
The Remote Proctor system must be purchased through the WBU bookstore. The fee includes
all charges for use of the Remote Proctor system for (1) year from date of purchase. Renewal

service fees are charged every year after initial purchase.


Process: The Remote Proctor system will be used with online examinations as instructed by
course faculty members. Students are required to take online examinations using a Windows or
Macbased computer that meets the minimum requirements as outlined in the instructions on the
Software Secure installation page (http://www.remoteproctor.com/install).
o Students are required to register their devices and establish identity following the
procedures outlined in the Remote Proctor instructions.
o Students are responsible for selftesting the functionality of their units well in advance
of all Remote Proctored exams in their courses, so that any troubleshooting that is
required can be accomplished. A practice quiz for this purpose can be found in the course.
Test Environment Requirements:
The online testing environment should mimic the in class testing environment, and must
conform to the following:
A quiet, secure, fully lighted room for the examination
o No other people in the room
o No pets in the room
o No communicating with others by any means (with the exception of contact
with the faculty member in an emergency)
o No leaving the room
o Clean desk or table with nothing except computer and remote proctor
system on the desktop or tabletop including removal of all books, papers,
notebooks or other materials, unless specifically permitted in written
guidelines for a particular examination
o No writing visible on desk or on walls
o No music, videos, or TV playing
o No use of headphones
o No use of cell phone for any reason other than to contact support or your
instructor
o All other computer monitors must be powered off
Have the Remote Proctor device correctly situated:
o Within an arms reach at all times during an exam
o On the opposite side of the keyboard as the computer mouse (if using one)
o Parallel to the key board (not behind the screen, etc.)
o Nothing covering the base or prism
o Lighting must be daylight quality and over head is preferred if at all
possible. If over head is not possible, the source should not be behind the
student.
All exam videos can/will be reviewed by the course faculty and the Program director. If a red
or yellow violation is detected, you will receive a zero on your exam or assignment.
