Insuficiencia Renal Aguda
y apoptosis (muerte celular programada), del parnquima renal que, sin embargo, en
la mayoria de los casos se regenera durante y despus de la enfermedad crtica.
La insuficiencia pre-renal, una disminucin de excrecin de agua y solubles en una
orina muy concentrada que auyuda a la preservacin del volumen sanguneo es
reversible con tratamiento adecuado de la hipovolemia y hipoperfusin. Sin embargo,
la insuficiencia pre-renal puede llevar a IRA intrnseca en casos de shock severo o
tratamiento tardio o inadecuado del shock.
De estos dos sndromes se distingue la insuficiencia post-renal por obstruccin de las
vas urinarias (9). En cualquier paciente con IRA es importante excluir o diagnosticar
obstruccin postrenal por ultrasonografa, ya que el tratamiento adecuado en este caso
deben ser medidas inmediatas (incluso quirrgicas) para resolver la obstruccin o
nefrostomia(s).
En el trabajo epidemiolgico de Liao y el grupo de estudio de IRA de Madrid en 19912, la mayora de los casos de IRA eran intrnsecos, sobre todo necrosis tubular aguda y
agudizacin de insuficiencia renal crnica; los restantes (21% y 10%) correspondieron
a fallos pre-renales y obstrucciones post-renales (2).
Adems del contexto clnico, el anlisis fsico-qumico de la orina, empezando con tiras
reactivas fcilmente accesibles puede ayudar a establecer el origen pre-renal, renal o
post-renal y la potencial reversibilidad de FRA (Tabla 1).

A pesar de esta clasificacin fisiopatolgica bastante clara, todava falta una definicin
cuantitativa precisa de la IRA, lo cual dificulta la coleccin de datos epidemiolgicos,
comparacin de resultados e investigacin clnica. Una revisin de 28 estudios sobre
factores de riesgo para IRA en el postoperatorio encontr el mismo nmero de
definiciones distintas de IRA (11) y una conferencia de consenso de ADQI (Acute
Dialysis Quality Initiative) enumer treinta (12).
A pesar de la variabilidad, dinmica e influencia de tratamiento con referencia a
diuresis, creatinina srica y tasa de filtracion glomerular que se explican detalladamente
en un articulo de Bellomo, Kellum y Ronco (13), estas mismas autoridades, en el 2001,
propusieron criterios para unificar las definiciones de los grados de disfuncin e
insuficiencia renal aguda (14). Esto se ha refinaco aun mas en el reciente sistema
RIFLE de definiciones de IRA propuesto por la ADQI en el 2003 (12). El acrnimo
RIFLE se refiere a cada fase de la insuficiencia renal: riesgo, injuria, fallo, lesin
permanente (loss) y enfermedad renal terminal. En este sistema, todos los cambios de
creatinina srica y filtracin glomerular se expresan en trminos relativos, siendo
aplicables tanto a la IRA primaria como a la agudizacin de la insuficiencia renal

crnica. Para cumplir con los criterios de IRA solo se requiere la presencia de un
criterio: oliguria o elevacin de la creatinina.

NaCl 0.9% o soluciones de electrolitos balanceadas, sobre las cuales se esperan mas
estudios (177).
Tensin arterial y presin de perfusin renal
Adems de establecer y mantener volumen sanguneo y gasto cardiaco adecuado, en
algunos pacientes, sobre todo con hipertensin arterial crnica, se requiere una presin
arterial suficiente para establecer diuresis, prevenir y tratar IRA. Es obvio que
hipotensin severa por debajo del lmite de autorregulacin disminuye el flujo
sanguneo renal y la filtracin glomerular. En pacientes con hipertensin crnica los
lmites de autorregulacin se pueden haber movido hacia niveles ms altos.
Tambin existe evidencia de que en shock y sndrome hepatorenal junto con una
vasoconstriccin renal intensa se pierde la autoregulacin de la perfusin renal y el
flujo sanguneo renal se vuelve dependiente de una manera linear de la presin de
perfusin (5 7, 48, 178). En estas situaciones la administracin de vasoconstrictores
exgenos no solo aumenta la presin de perfusin renal, sino posiblemente lleva a
vasodilatacin renal por reduccin de la actividad simptica y supresin de las
catecolaminas y la renina endgenas (5, 179, 180). Tambin hay evidencia experimental
de una prdida temporal de la autorregulacin renal por unas semanas despus de
episodios de isquemia y reperfusin renal. Durante este tiempo cadas de la tensin
arterial se acompaan con reducciones del flujo sanguneo y de la filtracin glomerular
y el mantenimiento estricto de la tensin arterial puede evitar deterioro renal con
potencial isquemia (5 -7).
Otro fenmeno fisiolgico que explica la respuesta diurtica a la elevacin de la presin
arterial es la diuresis por presin (181), parte integra de la regulacin de volemia y
presin arterial, que mantiene una diuresis normal con una presin arterial sistlica de
100 mmHg, lleva al cese de diuresis con hipotensin de 50 mmHg y eleva la diuresis de
manera exponencial con presin sistlica por encima de 100 mmHg arterial (a seis a
ocho veces en presin sistlica de 200 mmHg). Cuando se hace uso clnico de la
elevacin farmacolgica de la presin arterial para aumentar la diuresis hay que tener en
cuenta que se pierde la relacin fisiolgica entre el mantenimiento de la volemia y la
presin arterial, y se puede llegar a hipovolemia por diuresis. Si no hay sobrecarga de
lquidos y se quiere evitar hipovolemia es importante que la diuresis por presin
farmacolgicamente inducida se acompaa con un re-emplazo de volumen adecuado.
4. Frmacos y terapias especficos
Norepinefrina
A pesar de que la infusin directa de norepinefrina a dosis alta a la arteria renal lleva
dao renal en animales (182, 183), en las ultimas dcadas mltiples estudios
experimentales y clnicos han demostrado que el uso clnico apropiado de la
norepinefrina es asociado no solo con elevacin de la presin de perfusin en estados de
shock hiperdinmico sino tambin un aumento de la diuresis y de la depuracin
osmolar, de agua libre y de creatinina (184 - 188). En un estudio reciente de sepsis en
ovejas el grupo de Bellomo estableci un aumento del flujo sanguneo renal global y
medular con una dosis endovenosa de norepinefrina de 0.4 mcg/kg/min. (189). Esta
evidencia experimental y clnica ha logrado que la adicin de norepinefrina a la terapia
con volumen se ha vuelto parte integra de la prevencin y el manejo de IRA, sobre todo
en estados de hipotensin por shock hiperdinmico. Sin embargo, en un estudio

pequeo de 14 pacientes con shock sptico severo en una dosis muy alta de
norepinefrina (1.2 1.9 mcg/kg/min), no se vi ningun aumento en diuresis o
aclaramiento de creatinina durante cuatro horas en las cuales se elev la tensin arterial
media de 65 a 80 mmHg con un aumento de la dosis de norepinefrina (268). Ademas de
ser un estudio limitado en nmero de pacientes y duracin, se debe cuestionar porque
los pacientes recibieron una dosis tan elevada de norepinefrina ya antes del
experimento, o por enfermedad subyacente y shock septico excepcionalmente severos o
descontrolados o tal vez por otros aspectos del manejo no siendo ptimos.
Vasopresina
Mientras la vasopresina es sinnimo a hormona antidiurtica y a dosis alta reduce la
perfusin renal, a dosis baja (0.02 0.04 U/min) ha demostrado un efecto diurtico en
varios estudios pequeos en sndrome hepatorenal, shock sptico y cardiognico (190 192). Esto se puede explicar por un aumento de la presin de perfusin renal y por
efectos distintos sobre receptores V1 y V2 renales que llevan a vasoconstriccin de la
arteriola glomerular eferente que aumenta la presin de filtracin glomerular pero
tambin a vasodilatacin de los vasos de la medula renal, mediada por oxido ntrico
(193). Se ha establecido que el uso de anlogos sintticos de vasopresina (ornipresina y
terlipresina) en el tratamiento del sndrome hepatorenal logra mejor diuresis y funcin
renal. (179, 180, 194,195, 196).
Aumento de xido ntrico
A pesar de que hay cierta controversia sobre los posibles efectos nocivos del exceso de
xido ntrico en sepsis (197), hay evidencia de que tiene un rol beneficioso en mejorar
el riego capilar en general y la perfusin y el balance de oxgeno renal. Unos pequeos
estudios recientes con visualizacin microscpica directa demostraron que la adicin de
pequeas dosis de nitroglicerina, un donante de xido ntrico, a la terapia de shock
sptico y cardiognico con fluidos, inotrpicos y vasopresores lleva a abertura de
capilares antes no perfundidos, as reduciendo el shunt en la circulacin capilar y
mejorando el aporte de oxigeno a las clulas (198-199). A nivel renal el xido ntrico es
un mediador de vasodilatacin en la arteriola glomerular aferente y en la medula renal.
(10, 200 - 203) Se ha comprobado en un modelo de shock sptico en puercos que estos
mecanismos protegen los riones y que con inhibicin no-selectiva de la sintasa de
xido ntrico se puede inducir IRA (203). En un trabajo con riones de ratas aislados se
observ que con nitroprusiato sdico, un donante de xido ntrico, despus de isquemia
renal con presencia de leucocitos se preserv la filtracin glomerular y la absorcin
tubular de sodio (204). A los potenciales beneficios sobre la perfusin glomerular y
medular y el trabajo tubular, se aade evidencia que el xido ntrico puede prevenir la
trombosis glomerular inducida por endotoxina (205). Otro trabajo reciente demostr que
el xido ntrico es un mediador del efecto protector del precondicionamiento
isquemico sobre la funcin renal (206).
Administracin de arginina, el substrato para la formacin del xido ntrico, mejor la
perfusin renal en modelos de IRA en animales y en recipientes de transplantes renales
(208 - 210). Los efectos de N-acetilcsteina consisten en un aumento de la sntesis y
duracin del efecto del xido ntrico. Adems, como la N-acetilcistena se une con el
xido ntrico hay menos substrato para la formacin de peroxinitrito, un radical libre
nocivo formado de xido ntrico y oxgeno (34). La N-acetilcsteina ha demostrado un

efecto preventivo en mltiples estudios clnicos de nefropata por radiocontraste (32 34, 94 - 101) y tambin en un pequeo estudio sobre sndrome hepatorenal (211).
El xido ntrico tambin est involucrado en mltiples efectos protectores de las
statinas en modelos de IRA isquemica en animales, en los cuales se vio un aumento del
xido ntrico endotelial constitutivo (207) y una reduccin de la sintasa de xido ntrico
inducible (212), y tambin una reduccin en la formacin de radicales libres de oxgeno.
Un potencial papel beneficioso de donantes de oxido ntrico, N-acetilcsteina y statinas
en la prevencin y el tratamiento de otros tipos de IRA en humanos podra ser un rea
de investigacin clnica interesante.
Diurticos de asa en IRA
Como mencionado antes, los diurticos de asa pueden disminuir la funcin renal en
ciertas situaciones (72 - 78). Sin embargo, el uso de diurticos en pacientes crticos con
insuficiencia renal sigue siendo comn; por ejemplo en el estudio australiano sobre
dopamina mas de la mitad de los pacientes en ambos grupos recibieron furosemida en
una dosis total de aproximadamente 200 mg (39). Hay evidencia que el efecto de la
furosemida se logra con dosis mas pequeas y es mas consistente cuando se administra
en infusin continua (en dosis tan bajas como 1 - 4 mg/h) en vez de bolos (213 - 215).
Las ventajas tericas de los diurticos de asa en IRA consisten en
1) Reduccin del transporte de cloruro y sodio en la parte ascendente gruesa del asa
de Henle, que reduce la demanda de oxgeno cuando puede ser limitada la
entrega de oxgeno (10, 215 - 217).
2) Potencial conversin de IRA oligrica a polirica, que puede facilitar el manejo
y limitar el aumento de los niveles altos de potasio, urea y creatinina antes de la
resolucin de la IRA, posiblemente obviando o minimizando la necesidad de
dilisis o hemofiltracin (220). Sin embargo, esto probablemente indica un
epifenmeno de casos menos severos de IRA respondiendo a tratamiento
general y diurtico y no un efecto beneficioso genuino de los diurticos
Un numero de estudios pequeos y revisiones ha demostrado que el uso de diurticos de
asa en dosis alta no previene ni mejora el curso de IRA, pero tampoco empeoran el
curso o pronostico (74 76, 214, 218, 219, 264, 265). Otra revisin meta-analtica de
estudios clnicos sobre diurticos de asa en IRA demostr un tiempo ms corto hasta
recuperacin de una diuresis por encima de 1.5 litros por da y un numero menor de
sesiones de dilisis en pacientes tratados con diurticos de asa, sin diferencia en la
mortalidad (220)
Un estudio americano multicntrico retroespectivo (74) demostr que en pacientes
crticos con IRA, muchos de edad avanzada y con insuficiencia cardiaca - ambos
factores ms prevalentes en el grupo con diurticos - que recibieron grandes dosis de
diurticos de asa con poco efecto se vio un aumento significativo de la mortalidad y
menos recuperacin de la funcin renal. Sin embargo, en pacientes que
respondieron con buena diuresis a dosis bajas de diurtico (lo cual posiblemente
implica una forma menos severa de IRA) no se vieron efectos malos sobre la mortalidad
y recuperacin renal. Por ende, una de las conclusiones del estudio es que un ndice de
respuesta a furosemida, i.e. la dosis de furosemida dividida por la diuresis diarias,
podra tener valor pronostico en IRA y ayudar en elegir el tratamiento adecuado con
diurticos o hemodiafiltracn y evitar las consecuencias nocivas de seguir con altas
dosis de diurticos y posiblemente infligir un peor pronostico a pacientes que en
realidad se deben hemofiltrar o dializar tan rpido que posible. Se podra alegar que este
estudio demuestra ms sobre el mal uso de diurticos por intensivistas y nefrlogos que

sobre los diurticos mismos, especialmente teniendo en cuenta que en algunos pacientes
se siguieron usando grandes dosis de diurticos de asa a pesar de que permanecieron
severamente oligricos por varios das.
Lo que no se ha controlado suficientemente en los estudios y discutido en la
controversia sobre diurticos en IRA es el resto del tratamiento, como mantenimiento de
la volemia y de la presin de perfusin antes y durante el potencial uso de diurticos. La
cuestin tal vez no es tanto s o no usar diurticos sino como usarlos apropiadamente.
La inclusin de diurticos en infusin a dosis baja (furosemida 1 4 mg/h) en una
estrategia teraputica que provee volumen, gasto cardiaco y presin arterial ptimos as
como reemplazo de la poliuria y de electrolitos suficientes, es una estrategia
ampliamente usada por intensivistas para el manejo de IRA en pacientes que responden
con diuresis efectiva a esas medidas (215) pero no ha sido sometida a estudios
controlados de suficiente tamao y calidad.
Antagonistas de adenosina
Como mencionado antes, la adicin de dosis bajas de aminofilina u otros antagonistas
especficos de los receptores A1 de adenosina a diurticos de asa ayuda en mantener la
filtracin glomerular, que tiende a disminuir con el uso de diurticos solos (73, 82).
Adems, la aminofilina tiene un efecto diurtico propio (221 225). Mas importante
an podra ser un efecto protector de los antagonistas de adenosina sobre la funcin
renal, manteniendo la filtracin glomerular y aumentando diuresis (225) despus de
hipoxia (226, 227), isquemia (228, 229), hipotensin (230), radiocontraste (106 - 111) y
en varios tipos de IRA (231 234). En estudios peditricos se usaron dosis nicos de
1 o 8 mg/kg, en adultos la dosis convencional para tratamiento de asma: un bolo de
5 mg/kg, seguido por una infusin de 0.5 mg/kg/h. Sin embargo, el antagonismo de
adenosina se puede obtener con dosis muy bajas de aminofilina. Desafortunadamente no
existen suficientes estudios clnicos controlados ni sobre la dosis ms eficaz de
aminofilina para una potencial proteccin renal ni sobre su valor en pacientes crticos a
riesgo de o con IRA, tal vez en combinacin con reanimacin ptima y diurticos a
dosis baja. Por esto no se puede saber si los antagonistas de adenosina prestan un
beneficio genuino o si los pacientes que responden a ellos como a los diurticos de asa
simplemente tienen Insuficiencia Renal menos severa que los que necesitan
hemofiltracin o dilisis y mejorarian igual sin farmacoterapia adicional con buena
reanimacin solo, como en el estudio australiano sobre dopamina.
Dilisis y hemofiltracin
Las indicaciones para tratamiento depurador extracorpreo en IRA son:
Oligo-anuria refractaria
Sobrecarga de lquidos y edema pulmonar
Hiperpotasemia severa
Acidosis metablica severa
Hipo- o hipernatremia severas
Uremia severa y complicaciones urmicas como encefalopata, hemorragia por
disfuncin plaquetaria, derrames pericardiales y pleurales
En pacientes crticos y catablicos, sin embargo, es mas importante responder a la
dinmica de IRA y no necesariamente siempre esperar hasta que se llegue a estos puntos
extremos, sobre todo cuando no se ve respuesta suficiente a la terapia con fluidos,

Dilisis
La dilisis principalmente aclara solubles de peso molecular bajo (hasta 500 Da:
electrolitos, urea y creatinina) por difusin por una membrana semipermeable
siguiendo gradientes de concentracin entre la sangre y un liquido de dilisis al otro
lado de la membrana. La eliminacin de agua ocurre por un grado de conveccin
adicional y osmosis. Los factores que influyen el aclaramiento de agua y solubles en la
dilisis son: la superficie y el tamao de poros de la membrana, la concentracin de
solubles en el lquido de dilisis, y el flujo de sangre y lquido de dilisis.

Tal vez por todos estos factores un meta-anlisis de estudios comparando tcnicas
intermitentes y continuas de hemodilisis y filtracin encontr una mortalidad reducida
con las tcnicas continuas cuando se corrigieron los datos por factores como cualidad de
los estudios primarios y severidad de la enfermedad crtica, a pesar de que no haba
diferencia en la mortalidad analizando todos los estudios sin estos ajustes (248). Un
estudio multicntrico francs no pudo comprobar esta ventaja de la hemofiltracin
continua, sino demostr una mortalidad igualmente alta de 67 y 68% y ninguna
diferencia en morbilidad o estada en pacientes crticos multimorbidos con IRA
comparando hemofiltracin a dosis relativamente baja (29 ml/kg/h) con dilisis cada
segundo da (275). Lo que parece comprobar este estudio es que las dosis bajas de
depuramiento son igualmente malas en ambas tcnicas. Y tal vez cuando se logra una
dosis alta ya no importa tanto la tcnica depuradora, i.e. la cuestin no tanto es qu se
hace sino cmo se hace. Esto se demostr en el estudio de Schiffl y colaboradores (38)
que compar hemodilisis intermitente cada da o en das alternos. Se observ en este
estudio que la hemodilisis diaria se asoci con reducciones significativas en:
Mortalidad 14 das despus de la ultima dilisis (28% vs 46%).
La mortalidad de 28% en el grupo de dilisis diaria fue ms baja que la de los
grupos de alta dosis de hemofiltracin continua (42%) en el estudio clave de Ronco
et al. (37)
Morbilidad
(Menos empeoramiento de IRA durante el tratamiento, sangrados gastrointestinales,
insuficiencia respiratoria, sepsis y sndrome de respuesta inflamatoria sistmica)
Duracin de IRA (9+/- 2 vs 16 +/- 6 das)
Estos beneficiosos de la terapia diaria se explican por una dosis ms alta de dilisis y
depuracin total, razn ms obvia para la disminucin de complicaciones urmicas
como hemorragia gastrointestinal y sepsis, pero con menos prdida de volumen en cada
sesin de HD, lo cual se asoci con menos episodios de hipotensin.
Ya se vio en un trabajo retroespectivo anterior un efecto favorable sobre la mortalidad
de pacientes con riesgo intermedio con dosis mas altas de dilisis (249).
Otra nueva tcnica dialtica prometedora que produce dosis ms alta de depuracin con
mayor estabilidad cardiovascular es la dilisis lenta a eficacia baja (SLEDD = Sustained
low efficiency daily dilisis) (273, 274).
Hemofiltracin y hemodiafiltracin
El proceso y las fuerzas fsicas implicadas en la hemofiltracin son anlogos a la
ultrafiltracion glomerular renal: el movimiento de solutos y agua dependen del
tamao de los poros de la membrana semipermeable y el peso y la carga molecular
de los solutos (hasta 50.000 Dalton), de la superficie de la membrana, del gradiente
hidroesttico transmembranal (presin de sangre versus presin del ultrafiltrado)
al cual se opone la fuerza onctica de las proteinas en la sangre que no se filtran
(235, 236, 244). El ultrafiltrado se sustituye con una solucin balanceada de
electrolitos (menos la cuantidad necesaria para cualquier balance negativo de lquido
deseado). La prdida neta de solutos como urea, creatinina etc. resulta de la
ultrafiltracin misma pero tambin de la dilucin por el lquido de reposicin.
La hemofiltracin se descubri en los aos 70 por un grupo alemn como mtodo
scontinuo para la eliminacin de agua, sobre todo, en pacientes crticos con sobrecarga
de lquido (250). La tcnica empleada fue simple, poniendo un filtro con una membrana
de dilisis entre una va arterial y venosa, as empleando la presin arterial del paciente
y la diferencia de altitud entre el paciente y el recipiente del ultrafiltrado para generar un

Una respuesta ms clara sobre el valor de terapias de apoyo renal a dosis altas
comparadas con las convencionales se puede esperar de dos estudios multicntricos
recin comenzados en Estados Unidos y Australia (261, comunicacin en Simposio
Internacional de Medicina Intensiva, Bruselas, Marzo 2006). En el estudio
estadounidense se randomizarn 1100 pacientes con IRA en 27 centros a hemofiltracin
a 35 ml/kg/h vs. 20 ml/kg/h cuando estn hemodinmicamente inestables y a dilisis
diaria vs. en das alternos cuando logran estabilidad hemodinmica. Se analizar la
mortalidad en 60 das (reduccin de 55 a 45% estimada para la calculacin del poder
estadstico) como resultado primario, as como mortalidad en UCI y hospitalaria,
duracin y recuperacin de IRA y de otros fallos orgnicos. Tambin se incluir un
anlisis econmico sobre la relacin costo-beneficio de las dos estrategias de
tratamiento de IRA. El estudio australiano RENAL (Randomized Evaluation of Normal
vs Augmented Level of Renal Replacement Therapy) solo compara dos dosis distintas
de hemodiafiltracin continua, con un efluente combinado de ultrafiltrado y dializado de
25 ml/kg/h o de 40 ml/kg/h y su potencial impacto a la mortalidad en 90 das.
Se esta experimentando tamben
con una nueva tecnologa de apoyo renal
extracrporeo, el rion artificial, que combina un hemofiltro convencional con un
cartucho con una capa de clulas de tbulos renales humanos, que podria imitar las
funciones de transporte de solutos, metabolismo y hormonales del rion normal (272)
Resumen y conclusiones
La IRA sigue siendo un problema frecuente y grave en pacientes crticos, que conlleva
una morbimortalidad elevada por su asociacin con fallo multiorganico pero tambin
atribuible a si mismo y a su tratamiento. Para evitar esta morbimortalidad, la prevencin
de IRA toma un lugar sumamente importante en la medicina perioperatoria y de
cuidados intensivos. Hay estrategias claras y comprobadas para esto, sobre todo la
reanimacin temprana y adecuada con fluidos e inotrpicos y agentes vasooactivos
junto con el tratamiento rpido y eficaz de la enfermedad de causante de IRA y fallo
multiorgnico. Una gran parte de los casos de IRA leve o moderado se resuelven con
una estrategia teraputica similar a la de prevencin, con alta calidad de reanimacin y
cuidados intensivos. El valor de aadir diurticos de asa a dosis bajas sigue sin
comprobacin de su efecto sobre la funcin renal y morbimortalidad. Sin embargo, el
uso de diurticos sigue siendo comn en la prctica clnica y puede facilitar el manejo
de IRA. Un ensayo de diurticos a dosis baja siempre debe seguir o ser parte de una
reanimacin eficaz temprana y continua y nunca re-emplazarla. Si el establecimiento de
gasto cardiaco y presin arterial adecuada y posible adicin de diurticos no lleva
pronto a una diuresis satisfactoria y el paro del aumento de potasio, in hidrogeno, urea
y creatinina, los datos existentes indican que un apoyo renal extracorpreo temprano
y de dosis alta (hemofiltracin a > 35 ml/kg/h, o combinacin de filtracin a 2 l/h
con dilisis a 1 2 l/h continuas, o dilisis intermitente diaria) junto con el
tratamiento de la enfermedad subyacente y el mantenimiento de gasto cardiaco, tensin
arterial y otros aspectos importantes de cuidados intensivos, se asocia con mayor
sobrevivencia (60 70%), corta duracin y reversibilidad completa de la IRA.
