INSPECTION RESULTS: On 3-16-16, the C-501 Atmospheric Column was


subjected to a formal external Visual Inspection per API 510 Code
requirements. The following information details the findings during this
Inspection.

EXTERNAL COMPONENTS:

HEADS: The top head is fully insulated and the jacketing type is mastic. The
top head has damaged areas with exposed chicken wire which is susceptible
to moisture ingress. The bottom head is fully insulated and is inside the
vessel skirt.

SHELL: The shell is fully insulated and is only visible through the UT
inspection port holes and at the damaged/missing insulation areas. The shell
viewed through the UT port holes revealed moderate to heavy active CUI.
The accessible areas of the shell reveal scaling up to 3/16 with pitting up
to .030 (3rd Platform from top on the north side). The insulation is overall in
poor condition. 40% of the vessel has either damaged or missing
insulation/jacketing which is susceptible to moisture ingress. Moderate to
heavy active CUI is evident on all nozzle at shell areas, manways, ladder and
decking supports due to deteriorated or missing caulking and insulation.

BOOT & BOOT HEAD: N/A

NOZZLES: The nozzles appear to be in fair serviceable condition with


moderate to heavy active external corrosion and scale build up. Several
telltale nozzles look to be filled with debris which is a possible indication of a
past leak in the internal clad lining, or at the internal lining support pad seal
weld.

SUPPORTS: The decking supports appear to be in overall fair condition with


light paint failure and light corrosion noted. The vessel supports appear to be
in fair condition with light active external corrosion noted.

ANCHOR BOLTS: Anchor bolting is in fair condition with light to moderate


active external corrosion and scale build-up noted.

LADDERS & PLATFORMS: All ladders, deck railings, and platforms appear to
be in overall fair condition with the exception of the top platform which has a
broken rail on the west side of the platform. Minor coating failure is noted
with light external corrosion. All platform supports and bolting hardware are
coated and appear to be in fair condition.

COATING/PAINT DETERIORATION: N/A Fully Insulated.

FLANGES: The flange bolting appears to be in fair condition with light to


moderate active external corrosion. Light to moderate corrosion and scale
build-up between nozzle flanges noted. The man-ways and cover plates
appear to be in fair condition with light to moderate active external corrosion
and heavy external scale build-up. The man-way davits appear to be in fair
condition with light to moderate active external corrosion and rusting.

FOUNDATION: The foundation appears to be in fair condition with some minor


cracking and broken edges around the base noted. The vessel skirt appears
to be in fair condition with moderate cracking of the external fireproofing.

GROUND: The grounding cable is attached and secure and appears to be in


fair condition.

DATA PLATE: Attached and legible.

INSULATION: The insulation is overall in poor condition. 40% of the vessel


has either damaged or missing insulation/jacketing which is susceptible to
moisture ingress. Moderate to heavy active CUI is evident on all nozzle at

shell areas, manways, ladder and decking supports due to deteriorated or


missing caulking and insulation.

BIOLOGICAL GROWTH: Biological growth is noted at various locations.

Recommendations:
1) Build a full circumference scaffold and remove 100% of the
insulation and blast to perform a proper CUI inspection. (See note 2
if note 1 is not possible)
2) Remove insulation at suspect areas to evaluate the extent of CUI.
3) Repair, seal, or replace the insulation as needed to properly protect
the vessel from future CUI.
4) Repair the railing at the west end of the top platform which has
deteriorated.
5) Pull RVs and inspect.
6) Open the vessel for a full internal inspection whenever possible.
