Whats New for 2016


NTT DATA will be partnering with Delta Dental in 2016. Delta Dental offers
one of the largest networks for more convenience, more preventive
services, and substantial claims savings thanks to the deep discounts they
have negotiated with dentists. Plus, their PreventistryTM approach is aimed
at eliminating oral disease and keeping members healthier.
Delta Dentals FREE Mobile app that allows you to find a dentist, manage
your coverage and everything in between.
BCBSMA Enrollee Identify Protection
BCBSMA will begin offering free identity protection services to all eligible
BCBSMA members beginning January 1, 2016. This service will remain in
effect for as long as the member remains enrolled in an eligible plan.

How to Enroll

Where you enroll depends on your Employee Self Service Tool:

If you currently use HRDirect for your Employee Self-Service Tool


you will continue to do so in 2016.

Legacy Optimal Employees will enroll through the ADP/HRB Portal.

Carlisle & Gallagher employees will continue to enroll through the


ADP site accessed InsideCG.

Cloud Services employees who have not used HRDirect will receive
an email with an enrollment form. This form can also be found on
the Salesforce Site.

Three Medical Plans Offered:
Blue Cross Blue Shield (BCBS) National Provider Network

Choose any doctor or hospital


No primary care physician or referrals required
Two levels of benefits where you choose to go determines benefit level: in-network or
out-of-network

For in-network benefits, use a BCBS national preferred provider or any emergency room
For out-of-network benefits, use any licensed provider

Employer HSA Seeding Contribution

NTT DATA will match your HSA contribution


Eligible employees have the ability to have your contribution matched $.50
on the dollar up to a maximum of $300 when you enroll yourself only, or up
to $600 when you enroll yourself plus one or more eligible family
members.
You also have the ability to increase the employer contribution to your HSA
by participating in various incentive programs.

Must Open an HSA Account


Seed money is available only if you currently have an HSA account with
Health Equity or are planning on opening an account with Health Equity.
You may choose any HSA vendor outside of Health Equity, but you will not
receive the seed money or have the convenience to make pretax payroll
contributions.
If you already have an HSA account with another vendor, you can still
open one with HealthEquity, and rollover your balance if desired.

Contribution Limits
Be sure the total amount you contribute, plus NTT DATAs contribution,
does not exceed the 2016 annual contribution maximum:
o $3,350 if you cover yourself only
o $6,750 if you cover one or more eligible family members

Wellness Program Engagement

NTT DATA is pleased to offer its employees enrolled in the NTT DATA
BCBS medical plan the chance to participate in our wellness program.
This program is designed to encourage you to understand your current
health status and educate you on using your NTT DATA benefits wisely to
get the most from your healthcare dollars. More importantly, this program
encourages you to live a healthier life through various programs and

From January 1, 2016 November 14, 2016, you will be able to earn points
from a variety of activities. Certain activities and points will accumulate
toward a contribution into your Health Savings Account and some will enter

Dental and Vision


Dental Insurance Delta Dental
This plan includes in-network coverage levels of Preventative 100%; Basic - 80%, Major Services - 50% with a low annual
deductible of $75 person/$125 Family for in network services
(applies to basic and major services only).
Vision Insurance - VSP
Helps pay expenses associated with eye care. Plan provides
coverage for exams, lenses, frames, contact lens expenses, etc.
To get the most benefit from the plan, use a VSP network
provider, although benefits are available through non-network
providers. The plan covers annual eye exams at 100% every 12
months after a $10 copay. Allowances for hardware such as
frames, lenses, and contacts are every 24 months.

Identity Protection Services


BCBSMA will begin offering free identity protection services to all eligible
BCBSMA members beginning January 1, 2016. This service will remain in
effect as long as the member is enrolled in an eligible plan and will include:

Credit Monitoring
Fraud Detection
Credit/Identity Repair

This is another step to protect our members as we continue to make every


effort to ensure that all personal information maintained by BCBSMA is
secure and safe. BCBSMA is enhancing the safety and security of their
customers and members in light of heightened cyber-security risks.
The Identity Protection Services will be offered through BCBSMAs vendor
partner Experion and additional details on how to enroll and take advantage
of the services will be available from BCBSMA shortly.

Flexible Spending Account


Health Care, Limited Scope Healthcare, and Dependent Care Flexible
Spending Accounts (FSA) will now be administered through WageWorks. If
you enroll in one of the NTT DATA medical plans, you will only be eligible to
contribute to the Limited Scope Healthcare FSA.
The Limited Scope FSA works in conjunction with the high deductible health
plans. You may submit eligible dental and vision expenses to the Limited
Scope FSA and medical expenses after the deductible is met. If you are
planning for dental and vision expenses in 2016, you may wish to use the
Limited Scope FSA instead of your Health Savings Account. Please note: the
Limited Scope FSA is a use it or lose it plan.

Note: Dependent care (day care) reimbursement plan and Limited Scope

Accident Insurance:
Having the Aflac Accident plan means that your family may have added financial
resources to help with the costs resulting from an unexpected covered accident.
Hospital Indemnity Insurance:

A Hospital Indemnity insurance plan provides cash benefits for covered sicknesses or
injuries due to a covered accident to use as you see fit. The predetermined benefits
are payable regardless of any other insurance you have.
Pet Insurance:
Veterinary Pet Insurance provides protection for pets when they need it most. VPI
policies cover a multitude of medical problems and conditions related to accidents
and illnesses, including cancer. Coverage is available for dogs, cats, birds and exotic

All U.S.-based Employees:
If enrolled in the medical plan, you will also need to complete the tobacco
declaration form for 2016 declaring whether or not you use tobacco products.
New Plans Begin January 1, 2016 Enroll this November
HSA Contributions Begin January 1, 2016 Open HSA Account:
Eligible employees have the potential to earn employer HSA contributions beginning
in 2016. In addition, you will have the opportunity to increase these contributions
with numerous wellness incentives programs and challenges reaching up to $700
per family annually.
Additional Health Programs:
Select group of individuals will be selected based on biometrics and health to
voluntarily participate in a 10-week online Naturally Slim program paid for by the
company, as well as other healthy initiatives.
