How to Enroll
Cloud Services employees who have not used HRDirect will receive
an email with an enrollment form. This form can also be found on
the Salesforce Site.
Medical Plans
Three Medical Plans Offered:
Blue Cross Blue Shield (BCBS) National Provider Network
For in-network benefits, use a BCBS national preferred provider or any emergency room
For out-of-network benefits, use any licensed provider

NTT DATA is pleased to offer its employees enrolled in the NTT DATA
BCBS medical plan the chance to participate in our wellness program.
This program is designed to encourage you to understand your current
health status and educate you on using your NTT DATA benefits wisely to
get the most from your healthcare dollars. More importantly, this program
encourages you to live a healthier life through various programs and
incentives.
The following slide outlines a few of the opportunities available to NTT
DATA employees to participate in a more robust wellness program.
Employees can use this new program to help them learn how to live
healthier in a variety of ways.

Wellness Incentives
From January 1, 2016 November 14, 2016, you will be able to earn points
from a variety of activities. Certain activities and points will accumulate
toward a contribution into your Health Savings Account and some will enter
you into a raffle for prizes.

Complete the online Health Assessment and either attend
an on-site biometric screening event or have your
physician fill in the Biometric Verification form with your
body mass index, waist circumference, cholesterol, and
blood pressure results to BCBSMA. Your biometric data
will appear within 4 6 weeks of the data submission.

A Hospital Indemnity insurance plan provides cash benefits for covered sicknesses or
injuries due to a covered accident to use as you see fit. The predetermined benefits
are payable regardless of any other insurance you have.
Pet Insurance:
Veterinary Pet Insurance provides protection for pets when they need it most. VPI
policies cover a multitude of medical problems and conditions related to accidents
and illnesses, including cancer. Coverage is available for dogs, cats, birds and exotic
pets, plus optional wellness care coverage reimburses for vaccinations, flea/tick
preventive and more.

The Benefit Hub is a members-only discount marketplace that provides you with access
to hundreds of brand-name retailers and local merchants all from one website.
Special offers and discounts that you wont find anywhere else can be found at the
Benefit Hub from clothing to vacations, event tickets and appliances.
Whirlpool Significant savings for NTT DATA employees:
Whirlpool Corporation is offering NTT DATA employees the opportunity to become
Brand Ambassadors for Whirlpool when they purchase select Whirlpool, KitchenAid

Next Steps
All U.S.-based Employees:
If enrolled in the medical plan, you will also need to complete the tobacco
declaration form for 2016 declaring whether or not you use tobacco products.
New Plans Begin January 1, 2016 Enroll this November
HSA Contributions Begin January 1, 2016 Open HSA Account:
Eligible employees have the potential to earn employer HSA contributions beginning
in 2016. In addition, you will have the opportunity to increase these contributions
with numerous wellness incentives programs and challenges reaching up to $700
per family annually.
Additional Health Programs:
Select group of individuals will be selected based on biometrics and health to
voluntarily participate in a 10-week online Naturally Slim program paid for by the
company, as well as other healthy initiatives.

COMPASS HEALTH PROCONSULTANTS Your Healthcare Concierge service
available to assist employees in making educated treatment decisions based on
their needs and preferences before receiving care.
