1. Please provide me an attested copy of the policy and mechanism for
dealing such cases.
2. Please provide me names, designations and address of the officials
dealing my case.
3. Please provide me the documents depicting action taken on my
remarks/complaint.
4. In case no action has been taken on my remarks/complaint, the reasons
for the same may kindly be apprised to me under Section 4.1.(d) of the
RTI Act.
4. I state that the information sought does not fall within the restrictions
contained in Section 8 and 9 of the Act and to the best of my
knowledge it pertains to your office.
