1. Case
Yanto, a nurse in the surgical ward of a hospital VIP Islam in country. Today he is the
morning shift and care room A to room E. Mr. Burhan (50), a patient postoperative ileus day
3 were treated in room C. Mr. Burhan has been allowed to eat soft foods (porridge) with side
dishes and vegetables were also soft.
This afternoon, Yanto deliver lunch to the room C, Mr. Burhan room. When he got there,
Mr Burhan said that he wanted to eat porridge smothered pork broth. He said that it was his
favorite food.
Yanto was surprised to hear the statement of Mr. Burhan. He said that he could not meet
the demand of Mr. Burhan because the institution where he works is an Islamic Hospital. But
Mr. Burhan kept insisting and asking permission to bring their own food from the house.
Yanto but still has not allowed. Even without explaining anything, he leave Mr. Burhan own.
The next day, Mr. Burhan asked to return home forcibly by reason of nurses less favorable
treatment.
2. Completion of Case
2.1. Assessment
- Name: Mr. burhan
- Disease: ileus
- Have been treated for 3 days
- Has been allowed to eat porridge, side dishes and vegetables are soft on the third
day.
- Mr. burhan wanted to eat porridge smothered pork broth.
2.2. Diagnosis
Postoperative ileus
Ileus is a medical condition characterized by blockages in part or wholly to the
gut, because the contents of the intestine can not pass through the intestines.

2.3. Solution
These cases included in transcultural nursing, because of the factor of religion and
philosophy of life. about cultural differences between patients and nurses. Yanto nurses
can not meet the demand of Mr. Burhan to eat porridge in the flush pork broth, for nurses
working in the hospital Yanto Islam and did not leave nursing care and a clear concept of
the patient.
here does not show professional nurse caring for obvious reasons do not explain
why patients are not allowed to eat pork broth, so that patients and caregivers do not
reach an understanding that resulted in clients asking for return forced by reason nurses
do nursing unsatisfactory. and allows to make images that are not good for certain
circles.

applications for nursing, nurse yanto should explain how the effects of eating
pork, if the patient is not sure then the nurse can give evidence from a study, in a polite
way and not force the patient. and nurses should yanto need to be professional and not
leaving patients and caregivers also should be assertive in a polite way, so that the
patient can resist to eat porridge with pork broth so that the health / nursing care
achieved well. or inform the patient's family, when patients can not be notified, possibly
with the patient's family can understand.
The basic concept of nursing
- Cultural differences
Culture Islam prohibits eating any kind of food in the form of
haram like pork or the like. as described in this verse "Verily Allah has
forbidden to you dead meat, blood, the flesh of swine," (Q.S. Al-Baqarah
2: 173). Culture next because Islam forbids consuming unclean foods.
- Ethnicity
Culture feeding pigs according to Mr. Burhan is a common
practice. and used to be the favorite food.

Care
in terms of general health, to accelerate the healing process and
recovery of the patient after the surgery, we need to consider the tips
below:
Eat nutritious foods, eg rice, side dishes, vegetables, milk,
fruit.
Consumption of food (side dishes) high protein, such as
meat, chicken, fish, eggs and the like.
Drink at least 8-10 glasses per day.
Try to get enough rest.
so that in terms of health is not the recommended way to eat
porridge given pork because pork broth that contains elements of high
heat.
