Major Accident Hazards - lessons from History

Those who don't know history are destined to repeat it.

ADNOC Approach to Risk Management

Holistic Approach To Risk Management
ADNOC Code of Practice
HSEIA Requirement - ADNOC-COPV1-02
ADNOC HSEMS - ADNOC-COPV1-09
The main objectives of a Health, Safety & Environmental
Impact Assessment (HSEIA) Report are to demonstrate:
Compliance with HSE legislation and standards;
All HSE hazards, including major accident hazards and
occupational health hazards, have been systematically identified,
assessed and mitigated;
that environmental impacts have been identified, assessed and
mitigated.

Safety Cases for Offshore Facilities

Specific focus on design and hazards associated with offshore
sites can be very different from onshore sites.
Detailed Formal Safety Assessments (FSA) to support our Safety
Cases:
FSA inclusive of:

HAZID Identification Studies,

Fire Hazard Analysis,

Escape, Evacuation and Rescue Analysis (EERA),

Emergency Systems Survivability Analysis (ESSA),

Bowtie Analysis and HSECESs,

Quantified Risk Assessment (QRA).

Continual drive for Inherently Safe Design in our installations.

Special considerations for the management of Combined
Operations.
Drive for continual improvement and driving down our risks to
personnel and our assets.
Periodic Thorough Review of our Formal Safety Assessments.

COMAH Reports for Onshore Facilities

The Control of Major Accident Hazards (COMAH) Regulations ensuring
that businesses:
Take all necessary measures to prevent major accidents involving
dangerous substances
Limit the consequences to people and the environment of any major
accidents which do occur

Specific focus on hazards associated with mid and down stream

sites.
Hazards are very different from offshore sites (lots more different
substances / exposure to public, etc.)
UK COMAH Regs. acknowledge this however still rely on a Goal
setting approach.
We are taking responsibility of our societal risks.
Making Step Change in how we manage the risks for our
onshore sites

ADMA-OPCOs Focus on Managing

Major Accident Hazards
UK Legislation Seen as industry best practice for focus on
MAH (Goal setting approach We take ownership of our risks)
Offshore, operators must
have Safety Case in
place to operate (OSC
Regs. 2015).
Offshore supporting
regulations: PFEER, SCE
and PS.
IVB under DCR

Company policy is to
adopt UK legislative model
as focus is on MAH criteria
inclusive of environmental
releases

Onshore operators must have a

Major Accident Prevention Policy
to operate in compliance with
COMAH (1999) regulations.

Both Onshore and Offshore

require operators to identify all
Major Accident Hazard
causing scenarios.
Undergoing a process of
identifying HSECES for in
relation to ALL Major
Accident Hazards which
have been identified for all
on and offshore sites

HSEIAs carried out for all our assets but complemented by asset specific
Safety Case and COMAH Reports to focus on MAH

Lets focus on Major Accident Hazards first

We are renewing our focus on MAH by developing Safety
Case for each of our operating assets in addition to the
development of HSEIA (including COMAH) as per
ADNOC CoP.
Particularly important for aging assets.
Driving the maintenance of the functionality of our HSE
Critical Systems and Elements.
Performance Standards for our HSECESs fundamentally
defend against Major Accident Hazards.
Innovative Asset Integrity Initiatives and best practices
adopted
Renewed focus on Process Safety Management
HSE Culture Step Change

Reality of management of Major Accident Hazards

Effective HSECES regime drives the management of MAH.
MAH management processes similar for on and offshore sites.
Effective corrective and preventative maintenance is at the heart of our
management of risks.

Effective maintenance of HSECES (barriers)

required over time

Drive for enhanced PSM regime

HSE Culture Step Change

The need to win minds to initiate positive change /
greater ownership and accountability
Creating a sense of Vulnerability
Assessing HSE Culture Maturity
Identifying Key Factors to enhance Maturity
Respect
Reporting

Summary
Historically as an industry we can be complacent
We need a continued focus on MAH to prevent catastrophic loss
of lives
These vary for onshore and offshore sites.
Supplements the HSEIA and ADNOC compliance with other best
practices
Safety Critical Equipment is at the heart of Major Accident
Management
Asset Integrity is vital for ensuring management of MAH.
Process Safety is a core value of ADMA-OPCO and PSM is a
tool to sustain it.
HSE Culture Step Change to enhance the knowledge and sense
of vulnerability of our personnel.
