IMPLEMENTATION OF THE PREVENTION AND WELLNESS TRUST FUNDS

HYPERTENSION AND SMOKING CESSATION INTERVENTIONS AT THE METRO WEST


MEDICAL CENTER
BACKGROUND:

The Prevention and Wellness Trust Fund was created on July 2012, with the
intention to reduce healthcare costs by preventing chronic conditions.
This community-based project consisted of research-based interventions to reduce prevalent
and preventable health conditions, increase healthy behaviors, increase workplace-based
wellness and address health disparities. 1
On January 2014, the Town of Hudson was awarded with grants through the Prevention and
Wellness Trust Fund and the Metro West Health Foundation. As a result, the Town of Hudson
was to coordinate the partnership by addressing key health conditions for high-need populations
in Framingham, Hudson, Marlborough and Northborough. This health conditions are in tobacco
use, hypertension, pediatric asthma, and falls among the elderly. 1, 2
The Massachusetts DPH is responsible for administering the Trust under the guidance of an
advisory board. 1
On December 2015, the project was started at the MWMC. I was hired with the
instruction of starting from scratch the PWTF.
INDRODUCTION:

STEPS:

1. Learning about the program: important step in the implementation of a new program,
anywhere. Took about two months, during that time, permits and credentials were
created for me to have access to the hospital internet web.
2. First contact with Hudsons Director of Community Health, to learn about the program
and the referral systems: during this meeting doubts and questions were answered. In
addition, ongoing meeting are held with the Director on a regular basis.
3. First contact with the Director of the Hudson DPH, to learn about the Google referral
systems: In this meeting, I was granted access to Google Drive, the system that the
Metro west partnership uses for communication.
4. Establishment of a secure communication way between MWMC and PWTF Partners:
MWMC does not use the authorize the google referral systems, instead we use secure
Microsoft Outlook In my opinion, the most crucial step and took approximately two
months to implement. First, we were authorized to secure fax Protected Health
Information (PHI.) HIPAA compliance is a norm at the MWMC, for this reason, we were
not authorized to use Googles secure system. Furthermore, only MWMC secure email
is authorized to use to share patients information () to PWTF partners. In addition, It is
also possible to fax patient's information to partners via secure fax.
5. Create an internet based drive (H drive,) to access patient data and logs from any
computer available.
6. Create an internet based access point to enter the MWMC net from anywhere with
laptop. This could be done only with authorized laptop from MWMC and with the help of
the MWMC IT department.
7. Learn about release of information form and how to properly submit it to Athena.
8. Recognize the program partners (Latino Health Insurance, YMCA Framingham, and
DPH Framingham.)

9. Meeting with Paula Kominow from Edward Kennedy Centers to learn about the shared
outlook drive.
10. Meeting with Kim from YMCA Framingham to learn about the CDSMP in English.
11. Meeting with Ana and Dr. Abreu form LHI to learn about CDSMP in Spanish and
Portuguese.
12. Meeting with CHW from DPH Framingham to learn about the Smoking Cessation
Program.
13. To learn about secure email and get authorization from supervisor to secure email PHI to
partners.
14. Establish a secure way of communication between the hospital and the partners.
15. Presentation to Nurses and Medical assistants about the program
16. Creation of a paper form for referrals.
17. Creation of an electronic form in Athena for referrals.
18. Establish a communication route with the providers (MDs.)
19. Create a Work Order in Athena for smoking and HTN referrals.
20. Create a Spreadsheet.
21. Provide transportation to qualifying patients (Mass Health.)
22. Create a form to meet with patients outside the hospital.
23. CHW visits to patients.
24. Authorization from supervisor to email Medical Residents.
25. Meeting with Medical Residents to present the Program.
