Dear Sir/Madam,
1. Forwarded herewith please find the formats of the following certificates, which may please
be filled up, signed and sent back to this office to enable us payment of your pension for the
months from November 2014 to October 2015 through your bank account without any
interruption. It may please be ensured that these certificates shall reach this office on before
15 Nov 2014 positively.
(1) Life Certificate (Annex-A)
(2) Non Employment/Re-employment Certificate (Annex-B)
(3) Certificate of remarriage/Marriage (For family pensioner Only) (Annex-C)
2. The pensioner/family pensioner, who would like to avail the income tax rebate on their
pension income may forward the details of their savings may download the form-48 from
internet of BIS and submit the details along with supporting documents for such savings.
3. Further it is suggested to those pensioners/family pensioners who have opened the bank
account in those bank other than Syndicate Bank & State Bank of India for receiving the
pension are hereby requested that they may open an account either in Syndicate Bank or in
State Bank of India and forward the details of the name & address of the bank, bank account
no., IFSC code along with a photo copy of one leaf from the cheque book & photo copy of
page of bank passbook where the details of Name & address, Account Number of the
pensioner/family pensioner are printed. This is not applicable to those pensioners/family
pensioners who are already drawing their pension from Syndicate Bank or State Bank of
India.
4. All the pensioners/family pensioners are advised to request their respective bankers to
activate SMS alerts facility in respect of any financial transaction made in their pension
account, which will facilitate them to have firsthand information regarding credit of pension
& any other arrears in their account.
5. Further, all the pensioners/family pensioners are also requested to advise their legal
heirs/family members/care takers to intimate this office & also the pension disbursing bank
immediately in the unfortunate event of death/demise of the pensioner/family pensioner to
avoid complication in recovery of excess payment of pension.
