Instructions
Effective evaluation of job performance is an on-going process. Annually each employee provides a summary of
progress toward meeting job expectations. This form is to be used for annual evaluations, and at other times
during the year when formal feedback is needed.

Performance is noticeably less than expected. The employee requires close supervision, however the
need for further development and improvement is clearly observed.
3.0 Meets Expectations / ME
Performance meets all the requirements of the position in terms of quality and quantity of work. It is
described as good, solid performance. Minor deviations may occur, the overall level of performance
meets all position requirements.
4.0. Meets Expectations Plus / ME+
Performance consistently meets expectations in all essential areas of responsibility, at sometimes
possibly exceeding expectations, and the quality of work overall is very good. The employee shows
initiatives. Work is done often independently and completed on schedule with a good degrees of accuracy

5.0 Exceeds Expectations EE


Performance frequently exceeds job requirements. Accomplishments are regularly above expected
levels. Performance is sustained and uniformly above average with thorough and on time results.
This category is reserved for the employee who truly stands out and consistently demonstrate exceptional
accomplishment is terms of quality and quantity.
