Datos subjetivos, datos objetivos,
valoracin y plan (diagnstico y
teraputico) del paciente
hospitalizado

S: 78-year-old white female with multiple

complaints. Hx of chronic sinusitis, esophagitis, a
fibromyalgia-type syndrome, and depression.
Today, c/o continued problems with pain in the L
cheek and preauricular area, especially AM. Intense
pain at times. Increased postnasal drainage, which
gives her a sour feeling in her stomach. Also c/o
dizziness over the last few months, usually when
working around the house, associated with
sweating and nausea. No LOC reported. Also c/o
recurrent constipation, increased over the last three
months. Using Correctol, which gives her runny
stools for 24 hrs and then constipation recurs.
Colace was not helpful.

S: Mujer blanca de 78 aos con

mltiples dolencias. Antecedes mdicos
de sinusitis crnica, esofagitis, sndrome
de tipo fibromilgico y depresin. Hoy,
la paciente manifiesta dolores continuos
en la mejilla y el rea preauricular
izquierdas, en especial, en las maanas.
Dolor intenso, en ocasiones. Incremento
de secrecin retronasal, que le provoca
acidez estomacal. La paciente tambin
manifiesta mareos en los ltimos meses,
asociados con transpiracin y nuseas,
usualmente mientras realiza labores
domsticas. No se notific prdida de
conciencia. La paciente tambin
manifiesta estreimiento recurrente, que
ha venido aumentando en los ltimos
tres meses. Actualmente, toma
Correctol, que le provoca diarrea
durante 24 horas y luego le regresa el
estreimiento. El Colace no hizo efecto.

O: General: Well-nourished, well-developed,

elderly white female in no acute distress. She
appears somewhat sad and tearful. HEENT:
Tympanic membranes clear bilaterally. Nose with
pale mucosa, otherwise clear. Tenderness along the
L maxillary and L preauricular areas, and some
mild TMJ tenderness. Throat clear. Neck supple.
Lungs clear to auscultation. Cardiovascular:
Regular rate and rhythm w/o murmur. Abdomen:
Soft and diffusely tender to a mild degree. Bowel
sounds active.

O: General: Mujer blanca, anciana, bien

nutrida, bien desarrollada, sin ninguna
afliccin aguda. Parece estar algo triste
y llorosa. Examen fsico de cabeza,
odos, ojos, nariz y garganta: ambas
membranas timpnicas ntegras y
mviles. La nariz presenta mucosa
plida; pero, por lo dems, es normal.
Sensibilidad a lo largo del maxilar y las
zonas preauriculares izquierdas, y una
leve sensibilidad en la ATM. Garganta
despejada. Cuello flexible. Auscultacin
pulmonar normal. Cardiovascular:
frecuencia cardiaca regular, rtmica y sin

soplos. Abdomen: blando y con leve

sensibilidad dispersa. Borborigmos
normales.
A: 1. Depression
2. Recurrent sinus pain
3. Constipation
4. Esophagitis
P: 1. Patient has been off Zoloft for a while, so we
will have her resume that. No record in the chart of
adverse reaction to it.
2. Beconase AQ 2 puffs b.i.d.
3. For constipation, Metamucil or some other type
of similar fiber; increase her fluid intake. Referral
to Dr. Suess at discharge, for f/u on changes. May
need ENT consult for continued sinus pain.

A: 1. Depresin
2. Dolor sinusal recurrente paranasales
3. Estreimiento
4. Esofagitis
P: 1. La paciente no ha tomado Zoloft
por algn tiempo, por lo tanto
indicaremos que lo retome. No hay
antecedentes de reacciones adversas.
2. Beconase AQ dos inhalaciones, dos
veces al da.
3. Para el estreimiento, prescribimos
Metamucil u otro tipo de fibra similar;
incrementamos la ingesta de lquidos de
la paciente, la remitimos al Dr. Suess
luego del alta para hacerle seguimiento a
los cambios. Puede que necesite
consultar con un otorrinolaringlogo
debido al dolor continuo en los senos
paranasales.
