ETA M&E DIVISION ENVIRONMENTAL INCIDENT REPORTING PROCEDURE

1.0 PURPOSE
To ensure that all environmental related incidents/ incident potentials are treated
seriously and such incidents shall be reported without failure. Encourage all
employees to report all such unforeseen events that will damage the land and air or
may cause resource depletion in large extend in the form of property damage.
2.0 SCOPE AND COVERAGE
This Procedure is applicable across all ETA M&E/UAD/MPMD Abu Dhabi Division. All
staff belong to these division are responsible to report environmental incidents which
may occur during the work execution, storage and transportation of material/finished
product including ETAs subcontractors activity. Any unsafe conditions / acts which
has the poetical to cause land, air or water pollution shall be reported as near-miss
incident.

4.0 DEFINITIONS
Environmental Incident/ Near miss
An environmental incident/near-miss is an unforeseen event resulting from natural,
technological or human-induced factors, or a combination of these, that causes or
threatens to cause environmental damage as well as impacts on human lives and/or
property.
An exhaustive list of environmental incidents is given below but will not be limited
within the identified list.
Spillages of chemicals including paint, fosters, thinner etc
Improper disposal of chemical contaminated items, waste chemicals, concrete
wastes, empty chemical containers etc
Fire causing emissions to air and fire water runoff
Spillages of fuel while refueling or leakage from fuel storage area
Site flooding/ Major Water leakage
Refrigerant release
Uncontrolled continuous noise generation from mobile / static plants or equipments
due to lack of acoustical enclosures / poor service & maintenance/ failure to follow
PPM for equipments etc.

5.0 RESPONSIBILITIES
All ETA employees including staffs and workers and all other parties (subcontractors,
suppliers, visitors etc) related with ETA shall report environmental incident
immediately to concerned HSE personnel.
Use OHSEMS-FR-16 form for reporting environmental incidents/near miss and report
within 24 hours period of the incident.
Project Manager/ Engineers/ Supervisors / HSE Personnel are responsible for ensuring
environmental incidents are timely reported to the appropriate level and necessary
corrective actions are implemented to prevent its recurrence.
HSE Manager will report the incident to the local / regulatory authority based on the
nature of incident and regulatory requirements.
HSE in-charge personnel are responsible to ensure that necessary corrective actions
are executed by the concerned personnel as identified in the incident report.
6.0 REPORTING PROCEDURE
6.1 REPORTING
All environmental incidents defined above as well as identified by the client/ local
authorities shall be reported to site HSE personnel in the same shift by the person
who witnessed the incident.
Use OHSEMS-FR-16 form for reporting environmental incidents/near miss and submit
the report within 24 hours.
6.2 INVESTIGATION
All environments related site incidents reports shall be investigated to detect the
immediate, underlying and route causes that lead to the incident.
Investigation team shall comprise of HSE personnel, manager, and engineer,
supervisor responsible for or related with the incident.
Investigation team shall identify all related corrective and preventive actions and
shall implement the agreed measures within the agreed time declared in the
investigation report.
Review impact / risk assessment after every environment incident/ near-miss
incident/ change in the process or equipment or method statement or a change in the
existing regulatory requirements. Circulate with all concerned staffs and employees
through HSE meeting/ trainings/TBTs.

Evaluate the cost of environmental incident by using the following matrix and add all
related hidden costs associated with the incident if exists.
Prepare and circulate the environmental alert among the division through head office
after the incident if it found useful to prevent the recurrence of similar incidents in
future.
