Established, developed and implemented a new hospice program in Rochester Hills, Mi.. Worked
independently to establish policies and procedures, developing additional policies as program
grew. Hired skilled talented professionals, negotiate contracts with facilities and individuals,
established care planning guidelines, developed and conducted IDT groups., Established QA/PI
program, developed patient admission packet forms, hospice brochures, marketing packets, staff
orientation program. Admitted and case-managed first 30 patients in hospice program, did 100%
on-call duties for 9 months. Good knowledge of CHAP, Medicare and State of Michigan
regulations. Passed State License and CHAP survey, obtaining deemed status.

St. John Hospice, Detroit, Michigan 1999-2007 Hospice Clinical Nursing Supervisor

Oversaw day-to-day operations, coordination and supervision of all aspects of patient/family


services for a team of approximately 50 patients in St John Hospice. Led IDT meetings weekly,
actively participated in the development, implementation and evaluation of policies and
procedures, and in the development and accomplishment of annual goals and team objectives.

Director, RNs, HHAs, MSW, Spiritual Care Coordinator, Volunteer Coordinator, Bereavement

stage illnesses, pain control, Nearing Death Awareness, Oncology emergencies, signs of
impending death, AIDS, and Universal Precautions. Nine years experience as a home hospice
nurse and original team member for the first hospice in Macomb County. Pronouncing patient
death, supporting family, working with police, medical examiner, facilitating notification of family
members, clergy, funeral home, staff and physician and overseeing removal of body from the
home.
