DIFFICULTIES IN AESTHETIC REHABILITATION OF FRONTAL MAXILAR TEETH

Introduction
Esthetic rehabilitation of teeth is increasingly requested more and more by patients. ( POTI

Esthetic rehabilitation of teeth is frequently requested by patients. Dental disharmonies caused
by a lack of teeth alignment, form anomalies and/or natural color alteration raise a number of
concerns for therapeutical prescriptions, especially when these affect the exposed teeth. In color
disharmonies, the treatment is far more difficult as the number of affected teeth with discromies
of different age and etiologies are increased.
Purpose
The purpose of this paper is to exemplify a pragmatic therapeutical approach to rehabilitation of
dischromic maxillary front teeth of a young male patient. The difficulty of the case resided in the
different dischromic etiologies and different levels of pigmentation of maxillary incisors.
Material and method
In order to establish an etiologic dischromic diagnostic it was used a clinical and radiographic
examination while the anamnesis evidentiated the age of the dischromies. The base color of the
teeth was determined by using the classic and digital method; also, as a reference point for the
final color of the restoration were the unaffected teeth. Taking into consideration the etiological
factor, the patients desires and the economic possibilities of the patient, the chosen restoration
technique was the direct technique. Therefore, the composite resin Filtek Ultimate (3M Espe)
was used to replace the old restorations which were pigmented and infiltrated, and to restore the
untreated caries. In the case of a tooth which presented carious lessions and marked dyschromia
due to endodontic etiologies, the canal treatment was redone, inside whitening treatment applied
and direct technique of applying veneers.
Results
Although through a direct technique it is more difficult to obtain a perfect harmonization of the
teeth which present different dischromic degrees, the result is more than satisfying, being
obtained not only a color improvement, but also a correction of the position and proportion of the
maxillary incisors.
Conclusions
The use of state of the art materials for restorations adhesive systems and composite resins
combined with the accuracy of clear clinical protocols makes the direct techniques to extend
their clinical recommendations. The advantage of direct restoration techniques is found in their

reduced cost and in the treatment duration. Also, besides gratitude, the patient admires the
aesthetic talent of the doctor, while the joy of creation is shared.
