Each Gift Card has a unique 9 digit identification number starting with GX, HX or RX
American Express® Gift Cards
which can be found on the back of the Gift Card packaging. Provide identification
numbers for all 4 Gift Cards purchased below:

HOW IT WORKS: Rebate Form and Copy of Receipt must be mailed to American Express by 7/22/08.

to you within 60 days. If the Spring Fling Card worth $25 does not arrive within 60 days, please call
800-310-0265 for delivery status information.
