AFFIDAVIT OF UNDERTAKING

I, Dr. Danilo Viterbo Ayap of legal age, married and a resident of Km. 30
National Road Tunasan, Muntinlupa City after having been duly sworn on oath in
accordance with law, do hereby depose and say:

1) That I promise to reconstruct the width of the stairways on both left and
right wing from 109 cm to 112 cm as per required under the Sec.
10.2.9.2 of R.A. 9514 until the end of the first semester of S.Y. 20162017.
2) That there will be an installation of the manual fire alarm system before
the opening of classes this S.Y. 2016-2017 as per required under Sec.
10.2.6.4 of R.A. 9514.
3) That there will be an installation of additional standpipe system at the
Senior High School Building in compliance as per required under Sec.
10.2.6.6 para D of R.A. 9514;
4) That evacuation plans has been posted and assigned in different
strategic places throughout the building as per required under Sec.
10.2.5.13 of R.A. 9514
5) That portable standby firefighting appliances has been purchased with
O.R. No. 0812 by B.J. and K Enterprises (Attached hereinto are the
Official Receipt and the Sales Invoice).
6) That the firefighting appliances subject for recondition this August 2016
as per required under Sec. 10.5.3.3 of R.A. 9514
7) That I am executing this affidavit in compliance with the requirements
for the issuance of the Fire Safety Inspection Certificate (FSIC) for
occupancy permit issued by the Muntinlupa City Fire Station under the
City Fire Marshall Gilbert D. Dolot Ph. D.
