COMES NOW, the undersigned petitioner and before this Honorable Board, most
respectfully alleges:
That the petitioner is of legal age, married, Filipino, and with residence and postal
address at Simata, Molave, Zamboanga del Sur;
That petitioner is a grantee of a Certificate of Public Convenience to operate MINI BUS
on the line DON VICTORIANO-OZAMIS CITY VIA JOSEFINA-MOLAVE and VICE VERSA
with the use of ONE (1) unit as described hereunder which will expire on JANUARY 30, 2020:

That petitioner would like to downgrade her CPC to OPERATE from MINI BUS to PUJ
SERVICE, as reflected in her Certificate of Registration with the Land Transportation Office, it
is note that the same was registered under type of Body: MINIBUS because the weight from
5065KG to 3940KG;
That applicant has been complying with all the rules and regulations of this Honorable
Board and in order to make straight the records of the above-mentioned vehicle;
WHEREFORE, prescinding from the foregoing disquisition, it is most respectfully
prayed that after due hearing, this Honorable Board will DOWNGRADE the vehicles CPC from
MINI BUS to PUJ SERVICE.

other action or claim is pending therein, if I should thereafter learn that the same or
