What is hazardous energy?
Energy sources in machines and equipment that can be hazardous to workers.
During the servicing and maintenance of machines and equipment, the unexpected
startup or release of stored energy can result in serious injury or death to workers.
Energy sources include:

Electrical
Pneumatic
Hydraulic
Mechanical
Thermal
Chemical
Force of gravity
And Other energy sources

What are the harmful effects of hazardous energy?


-Workers servicing or maintaining machines or equipment may be seriously injured
or killed if hazardous energy is not properly controlled.
- Injuries resulting from the failure to control hazardous energy during maintenance
activities can be serious or fatal!
Injuries may include:

Electrocution
Burns
Crushing
Cutting
Lacerating
Amputating
fracturing body parts
others

What can be done to control hazardous energy?


Proper lockout/tagout (LOTO) practices and procedures safeguard workers
from hazardous energy releases

What is lockout/tagout
-is a set a safety procedures designed to reduce the risk of injuries due to
accidental activation of machinery or energizing of electrical system during
servicing, maintenance or repair.
-is required to ensure that before employee performs any servicing,
maintenance or repair on a machine or equipment where the unexpected
energizing, start up or reuse of stored energy that could occur and cause injuries.

effectiveness of the lockout device, tagout device and other energy control
measure.

3 type of employee and training


1. Authorize employee
a. are trained to know how machine are powered and the procedures
necessary to lockout/tagout the machine or equipment
b. Must know how to recognize hazardous energy sources and what type
and amount of energy they contained.
c. They must know how to isolate and control that energy source to
prevent accidents
d. Only authorized employee is allowed to disconnect the equipment
being service or maintain from the power source or services using
either logout or tag out or both methods.
2. Affected employee
a. An employee whose job requires him/her to operate or use a machine
or equipment on which servicing or maintenance is being perform
under lockout/tagout on whos job requires him or her to work in an
area which services on maintain is being performed.
b. They must know the purpose and procedures for energy control to fully
understand the importance of not using the machine that is
lockout/tagout.
3. Other emplooyees
a. Employees whos work in an areas where energy control are used but
not part of the process
b. It is important for them to understand lockout/tagout rules and to not
attempt reenergised machines that are lockout/tagout
c. They are also learn the limitation of the tagout devices
d. It is important that all employees follow of the central rules(room
should shut the equipment that is lockout/tagout)

What to do during inspection

The employer on authorized employee checks to see if all employee are


following the lockout/tagout procedure
The inspector reviews lockout responsibilities with all authorized employees
Inspections help find and correct problems and keep employees safe
Inspection documentation should identify the machine on which the energy
control procedure was inspected, the date of the inspection, employees
included in the inspection and the person performing the inspection.

Standards for locks and tags


Criteria for locks
1. Locks can be either keyed or combination
2. Locks cannot be used for any other purpose
3. Locks must be durable enough for the heat, cold, humidity or corrosiveness
or the environment in which they will be used.
4. Every lock used for the lockout procedure must be standardized within the
facility in at least one of the following criterial: color, size, shape.
5. Locks must be strong enough that they cannot be removed without heavy
force or tools like bolt cutters
6. Each lock must identify the name of the employee who installs it
Requirements for a tagout
1. Each tag must have the same print and format throughout the facility.
2. The tags must be easy to read and understand even if used in corrosive,
dirty, or damp areas.
3. Tags must be tough enough that they cannot be removed easily.
4. A nylone cable must be used to attach each tag.
a. The cable cannot be reusable.
b. It must be able to be attached by hand
c. The cable must be self-locking.
d. It must be able to withstand 50lbs of pressure before release.
5. Tagout devices should include a legend such as:
a. Do not Start
b. Do Not Open
c. Do Not Close
d. Do Not Energize
e. Do Not Operate
6. The installers name must be located on the front of the tag.

Proper procedures for lockout/tagout


Six step procedure for hazardous energy control
1. Preparation for shutdown.
Before shutting down the machine or equipment an authorized Employee
Needs to know
The type and amount of energy
The hazards of the energy
The method and means to control it
2. Equipment shutdown.
The machine will gently Shutdown according to procedures required by
the manufacturer to avoid increase hazards to employees as a result of
incorrect shutdown.
3. Machine or equipment isolation.
All the energy that the machine utilizes will be located and isolated from
their sources.

4. Application of lockout/tagout device.


Lockout/tagout device then can be placed by the authorized employee.
Lockout device - should be lock the energy isolation device in a safe or off
position.
Tagout device - If not capable of lockout then the use of tag is required.
5. Release stored energy.
The stored energy must be released, disconnected, restrained or rendered
safe.
6. Verification of isolation.

Steps for removing locks and tags


1. Clean out tools or materials in the machine.
2. Make sure all employees are a safe distance away.
3. Notify all affected employees that the machine is being activated.
Absent Authorizes employee procedure
1. The employer needs to verify that the employee is absent from the facility.
2. Reasonable efforts to contact the employee about the lockout/tagout removal
need to be made.
3. The absent employee will need to be informed that their lockout/tagout
device was removed before they return to work
