On the operator console, turn the computer on. The


monitor and printer(s) should already be on.
The computer starts Windows XP.

Log on to the QDR.

Power Off
Step

Important: Do not turn off the


circuit breaker.

Action

From the main window, click Exit.

In the Exit QDR System dialog, select Exit QDR with


shutdown? and click OK.
Do not turn off the monitor or printer(s).

Emergency Procedures
Power Failure
Turn off all equipment. When the power returns, it may be unstable. Wait a few minutes before turning on the equipment.

Shutting Down

Note: Refer to documentation that


came with the Explorer System for
controls and indicators on the
computer, monitor and printer.

If the Explorer was operating when the power failure


occurred, assist the patient from the table.

Turn off the computer.

Turn off the circuit breaker. In a power failure condition,


the green Indicator will be off.

After power is restored


Step Action

Wait a few minutes for power to stabilize, then turn on the


circuit breaker. The green indicator light goes on.

Perform the Power On procedure.

Failure During Operation


If the equipment malfunctions (e.g., X-Ray is on and C-arm is not
moving, etc.) perform the following procedure.

Note: The power should not be


restored without consulting your
Hologic service representative.

On the Control Panel, press the red Emergency Stop


Button. The table and C-arm immediately stop moving,
and the X-rays and laser turn off.

Assist the patient off the table.

Turn off the circuit breaker.

Disconnect the power cord from the AC outlet (if


possible).

Call your Hologic service representative.

Power Turned Off


If the circuit breaker has been turned off (not due to equipment
failure), or the system has been unplugged from the wall outlet,
restore power as follows:

does not turn on, make sure that
the power cord is plugged into
the wall outlet. If it is, call your
Hologic service representative.

If necessary, place the power cord into the AC outlet.

Place the circuit breaker, on the left pedestal, in the 1 (on)


position. The green indicator goes on.

Perform the Power On procedure.

Quality Control Procedure


Start Daily QC
Note: You can also select Daily
QC from the QC drop down
menu in the main window.

Click Daily QC in the main window.

Position the Spine Phantom

Remove the spine phantom from its storage case.
Place the phantom on the table with the white dot to the
left (foot end).
Position the phantom parallel to the back of the table.
Align the dot with the laser cross-hair.
Verify that the laser cross-hair is centered on the white
dot.
Click Continue. The system performs a system test.

System Test
The QDR system test verifies proper operation of the X-ray
subsystem before scanning the spine phantom. If the test
succeeds, Passed system test displays. If the test fails, perform
the steps described in the window to correct the error, and repeat
QC.

Auto QC Passed
Upon successful completion of the system test, the system runs
Auto QC and a window displays giving either a passed or failed
message.
When Auto QC passes, the message is Daily QC has passed.
Click OK to start patient scans.

Auto QC Failed
If Auto QC fails, the message is Daily QC failed and instructions
are provided. Follow the instructions to resolve the problem.

Patient Records
Explorer stores patient and scan information in a database called
the filing cabinet. The three types of records are:

Patient
Each patient has a record of biographical and insurance
information. Retrieve this record from the Patient Drawer
each time a patient returns for a scan.

Scan
Scan data for analyzed or unanalyzed scans, along with
information about scan characteristics and the patient, is
stored in the Scan Drawer of the QDR filing cabinet.

Analysis Reports
When a scan is analyzed, a report can be generated, saved
and retrieved.

Accessing Records
From the main window, you can access records in the filing
cabinet. The method you use depends on task type or where you
are during an exam:

An asterisk appears at the heading, and Patient Name
appears next to the Search Field text box.
Click in the Search Field text box.
Type the search criterion. The patient list displays names
that match the search criterion.
Click the patient's record to select it.
To search by another criterion, return to step 1 and click
another heading (for example, Patient ID).
Complete steps 2 through 4 for each search criterion, as
needed.

Performing an Exam
To perform an examination, click the Perform Exam button. The
Select a Patient for this Exam window displays.

Verify the patient name and scan type, in the upper left corner of
the Scan Parameters window, are correct before starting the
scan.

the appropriate section of this manual.

Analyzing a Scan
To analyze a scan use the analysis step buttons, toolbox and
brightness/contrast control found at the left side of the analysis
window.

Analysis Step Buttons


Each of the analysis step buttons is used to perform one step of
the analysis. To analyze a scan, click each button in order,
starting at the top button, and perform the operation required until
all steps are completed.

Global ROI (Region of Interest)


The defined boundaries of the image that is being analyzed. The
ROI appears on the image as a box.

Bone Map
An illustration, created by the system, of the area of bone defined
by the ROI. The map is superimposed, in yellow, on the image.

Vertebral Lines (Spine)


Used to mark the intervertebral spaces within the spine region of
interest.

Neck (Hip)
Allows positioning of the hip femoral neck box.

MID/UD (Forearm)
Allows positioning of the middle (MID) and ultra-distal (UD)
regions of the forearm.

Regions (Whole Body)


Used to define regions of interest on a whole body scan.

Vertebral Boundaries (Lateral Spine)


Used to identify the anterior boundary of the vertebral bodies, as
a dashed yellow line, on a lateral spine image.

Vertebral Bodies (Lateral Spine)


Used to identify the boundaries of the vertebral bodies, as boxes,
on a lateral spine image.

Mid Regions (Lateral Spine)


Used to adjust the regions in the middle of the vertebral bodies on
a lateral spine image (rarely necessary).

Results
Displays results of the analysis on the analysis window.

Toolboxes
Toolboxes provide the tools used at each step of the analysis.
Tools available depend upon on scan type being analyzed and
step button in use.

Global ROI Toolbox


Whole Mode
Allows the entire ROI box to be moved over the image. The box
is shown as dashed yellow lines.

Line Mode
Allows one line on the ROI box to be moved. Click any line to
select it. The active line is shown as yellow dashes.

Point Mode
Allows one point on the ROI box to be moved. Points are shown
on the box as plus signs. The active point is yellow.

1/3 Distal (Forearm only)


Used to adjust the size of the 1/3 distal region of the forearm
(rarely necessary).

Bone Map Toolbox


Add Bone
Used to connect outside edges on an incomplete bone map and fill
in the missing area (rarely necessary).

Delete Bone
Used to erase an area on the bone map (rarely necessary).

Lines Toolbox (Spine)


Used to mark the intervertebral spaces within the spine region of
interest.

Neck Toolbox (Hip)


Neck Box
Allows the neck box to be moved and/or resized (rarely necessary). The box is shown as dashed yellow lines.

Other Regions
Expands the Neck Toolbox to include Midline, Wards Triangle
and Trochanter tools.

Midline
Allows the midline to be moved or pivoted (rarely necessary).

Wards Triangle
Allows Wards Triangle to be adjusted (rarely necessary).

Trochanter
Allows the Trochanter area to be adjusted (rarely necessary).

Auto Position
Allows the system to automatically locate a region.

Brightness/Contrast Control
Used to adjust brightness and contrast of the image to obtain best
definition of anatomical features. Does not effect calculations.

AP Lumbar Spine Examination


Start the exam as described in Performing an Exam on page 10. In
the Select Scan Type window, select AP Lumbar Spine.

Positioning the Patient


A pillow may be used under the patient's head.

Action
Lay the patient on his or her back with the head at the
right end of the table (see Figure 3).
Position the patient's body so the spine is straight on the
table pad. The patient's shoulders should be at the upper
scan limit line.
Verify that the patient's pelvis and shoulders are aligned
on the table pad and centered with the marks on the table
pad.
Press and hold Center Table on the control panel to
move the table and C-arm to the center.
Place the knee positioner under the patient's lower legs
and adjust it.

Instruct the patient to remain still and breathe normally.

Determine the position of the patient's iliac crest.
Press Laser on the control panel.
Use the Arm and Table controls on the control panel to
place the laser's cross hair 2.5-5 cm (1 to 2 inches) below
the iliac crest and centered at the patient's mid-line.

Note: If the control panel X-ray
indicator fails to shut off within
10 seconds after the end of the
scan, press the red Emergency
Stop button immediately. Call
your Hologic service
representative before resuming
operation.

Click Reposition Scan before the scan is completed.

Position the cursor over the spine image. It changes to a

Adjust the Velcro strap to hold the foot in the correct

parallel with the table edge, you can offset the hip

Place the Lumbar Rest under the patient's waist. Most


patients (especially thin or with large hips) need the
lumbar rest tucked under the waist to keep the vertebral
column in the horizontal plane.
Place the Knee Rest between the patient's knees. Make
sure the patient's legs are perfectly superimposed to
prevent pelvis rotation.
Place another piece of foam (knee/ankle rest) between the
ankles. Use any combination of foam pieces between the
knees and ankles to make the patient comfortable.

forearm on the forearm rest portion of the head rest.
Position the patient's right arm perpendicular to the long

Comparing the current scan to a baseline scan lets you determine
the BMD rate of change over time. The baseline scan is usually
the first scan acquired for the patient.

Restoring a Baseline Scan

Before acquiring a new scan, restore the patient's baseline scan.


The patient's baseline scan is either on the computer's hard drive

oldest scan highlighted. The oldest scan should be the
baseline scan; if it is not, click and highlight the baseline
scan.
Click Next >> to display the Analysis window with the
baseline scan on the right and the follow-up scan on the
left.
Use the Analysis Step Buttons and Toolbox tools to
analyze the follow-up scan. Match the analysis of the
follow-up scan to the baseline scan.
When the follow-up analysis is complete, click Results.
The follow-up scan results replace the baseline scan.
Click Close.

Removing a Read/Write CD

Hologic recommends an immediate second archive of the
same scans to another cartridge or disk. This second
archive protects from loss of scans in the event of damage
to the first cartridge or disk.

Click on a single scan to select it.
To select multiple scans that are not adjacent to
each other, depress and hold down the Ctrl key
while clicking on each scan to be restored.
To select a range of adjacent scans, click on the
first scan in the range, depress and hold down the
Shift key, then click on the last scan in the range.
Selected scans display as white text on blue background.
Locate and place the cartridge or disk with the correct
Label into the disk drive.
Click Restore Scans.
If the list of selected scans includes scans on different
media, the system will prompt to insert that media.
The Scan Transfer in Progress window is displayed
while the selected scans are duplicated and transferred to
the system location.

Table Pad

information to the DICOM files Comments field. These entries
are not required and there is no restriction on their content.

The Preview button allows you to view the DICOM report before
you save or send it.

Printing a DICOM Report


The Print button on the DICOM Preview screen allows you to

desired location.

Sending a DICOM Report


The Send Report button allows you to send the DICOM report to
your configured destination remote node(s).
Note: Multiple scan selections
are allowed for the Send

waiting to be sent.

Viewing a History of Sent Reports


The View Log button on the View Queue dialog box allows you to
view a history of reports previously sent.

Dose Area Product Meter

The Dose Area Product (DAP) Meter measures the amount of


radiation a patient receives during an examination. This is done in
software by multiplying the amount of radiation for the scan
mode (dose) by the time it takes to perform the examination. The
measurement is displayed when the examination is exited

System Configuration
Allows changing the configuration settings in functional areas of

Usage
Displays and prints billing information for leased systems. Click
Usage and use Help for information.

Database Tools
Allows movement of patient, reference and QC data to and from
other databases.

Patient Management
Allows patient and scan data to be erased. All scans listed for a
patient must be erased before the patient can be erased.

Export
Allows data to be moved into a new, or existing, database on
another system. Click Export and use Help for information.

Import
Allows data to be moved from another system into Explorer.

Reconcile
Compares the system database with the scan files in the system
