Should you need to make a complaint or if you have any sugges ons on how we could

Repeat prescrip ons


Requests for repeat prescrip ons will be ready two working days aDer we receive your
request. For safety reasons we do not take requests for repeat prescrip ons over the
telephone.
Most prescrip ons have a tear-o por on lis ng all your current medica on. When
you require a repeat put a ck against the item and return the form to the surgery.
You may post it to us, hand it in at recep on, or put it in the repeat prescrip on box in
the wai ng room. Alterna vely you may request a repeat by le+er or fax. If you would
like us to return your prescrip on by post, please enclose a stamped addressed
envelope and allow enough me for delivery.
Several of the local pharmacies provide a service for ordering and collec ng repeat
prescrip ons. Please speak to your pharmacy if you would like to make use of this.
Rights and responsibili es
Much valuable clinical me is lost when pa ents do not keep their appointments. If
you cannot keep your appointment please telephone as early as possible so that we
may give that appointment to someone else who needs it. Your appointment is for 10
minutes only. If you have a number of problems you are welcome to make a double
appointment. We do try to run to me. If you are late for an appointment the surgery
runs late for all those following you. If you are late you may be asked to book another
appointment or wait ll the end of surgery.
Violent or abusive pa ents
This prac ce has adopted a policy of zero tolerance towards violence or aggression
from anyone visi ng the surgery. We understand that pa ents may be distressed and
may have genuine grievance and will be pleased to discuss this with you. If a pa ent
shows persistent aggression or any violence towards sta we reserve the right to call
the Police to deal with the ma+er. Such a pa ent may be removed from our Prac ce
list.
Conden ality and access to informa on
We undertake to respect the conden ality of your consulta ons with us and will not
release informa on regarding you to a third party without your consent unless there
are excep onal circumstances i.e. you are at risk to yourself or to others.

Registering as a pa ent
We will register pa ents who live within the catchment area indicated by the map a+ached. It will be
helpful if you bring your registra on card from your previous GP with you. You will be asked to
complete a form giving details of your current and previous addresses. If you are from abroad please
state the date you rst came to live in the UK. You will be registered with Hook Surgery but may
express a preference for seeing a par cular GP.

Temporary Residents
We are happy to see pa ents who are staying in the area for up to 3 months if they need to see a
doctor. Please speak to the recep onist and register as a temporary resident.

Home Visits
If you are too ill to visit the surgery please, if possible, request a home visit before 10.30 in the
morning. Please remember that home visits are only available for those too ill to travel. A doctor
may phone back to assess the need and urgency in order to plan their rounds eec vely.

Access for disabled people


There is plenty of parking on site. Access to the surgery and consul ng rooms is all on the level, a liD
is available for access to the rst oor. A wheelchair is also available.

We have an ac ve pa ent par cipa on group. If you would like to take part in events or par cipate
in surveys and discussions about how the prac ce is run or how you would like to see it develop,
please see our website for informa on or ask at recep on.

All surgeries are by appointment only. Some appointments are available for urgent, same
day consulta on, but otherwise pa ents are encouraged to consult one par cular doctor
for each episode of illness. Please make a double appointment if you have more than one
problem to discuss. Appointments are usually available from 8.30-11.00 each morning.
The aDernoons vary with some early appointments at 2.30 and later ones up to 8pm on
a Thursday when we oer appointments for commuters who cannot a+end at other mes
due to work commitments.

In order for you to register with the surgery you can ll out the registra ons
obtained at recep on or alterna vely go onto the Website and register. Once
registered you can telephone the surgery to book your appointment or
alterna vely register for online services and you will be able to book your
appointments online.

Check-Ups
If you are over 75 and have not been seen within the last 3 years you may
request a check - up with Debra Lacey, Nurse Prac oner or Kathy Boon,
prac ce nurse.

Computer Access
If you would like to register to book appointments, request repeat
prescrip ons and see a summary of your clinical notes on your own computer
please speak to recep on or download the forms from our website.
