In light of anticipated dam rehabilitation, recommendation is to not spend too much effort at this point on vegetation removal, but to maintain
the current condition and not allow vegetation to become more prominent.

(1) Some rocks, but no real protection provided; (2) significant scarping/erosion at and above the high water line; (5) too steep to walk across
the upper portion of the slope; (9) handful of smaller, active rodent burrows on the upper slope, one or two larger burrows above high water
line.

gpm, reported by Brad Setter to have been flowing about 4 times that amount previously after heavy rain and runoff with similar reservoir
level; likely has some groundwater connection; also sits below the diversion ditch around the right side of the reservoir and is likely fed by
this as well; recommended to continue to visually monitor, especially for any sediment in the flow, or deposits below where the flow is
exiting the hillside; saturated area below the right toe, source could be the hillside flow, and/or additional seepage exiting the toe.

(35) Flow coming from under the left outlet, valve is sealed tight, so flow is either coming from the upstream connection between the pipe
and the valve, or from somewhere else along the pipe, on the order of 2-3 gpm, clear.

OVERALL CONDITIONS

-Update the Emergency Action Plan using a provided template (W.W. Wheeler retained to assist with this and other activities)

Dam rehabilitation alternatives currently under consideration by City staff. Intent is to potentially budget for construction in 2017. It is
anticipated that all items from this inspection report will be addressed as part of that project.

Increased seepage noted this year from the left toe, under the outlet, and the right hillside; spillway capacity still significantly reduced by
large rocks in the channel downstream; and upstream slope erosion is bordering on severe in some areas; efforts towards an overall plan
for this structure are appreciated.
Based on this Safety Inspection and recent file review, the overall condition is determined to be:
