Anxietas Seksual Ibu Post Partum dengan Rupture Perineum Grade IV lama

Abstrak
Latar Belakang
Seksual adalah salah satu kebutuhan manusia, baik secara biologis dan
Psikologis. Kecemasan (anxietas) adalah perasaan takut yang tidak jelas dan
tidak didukung oleh situasi. Di Indonesia, masih banyak wanita yang mengalami
kecemasan seksual postpartum. Faktor ini berhubungan dengan rupture dan
penjahitan pada perineum, kualitas hubungan seksual terutama komunikasi
antara suami dan istri. Ruptur Perineum dapat terjadi karena adanya rupture
spontan maupun episiotomi perineum yang dilakukan atas indikasi ibu maupun
bayi. Komunikasi dan perawatan secara keseluruhan dapat mengurangi
kecemasan seksual postpartum.
Kasus
Pasien seorang wanita 30 tahun, gravida 2, para 1, abortus 0 datang ke RSUP
Prof Dr RD Kandou tanggal 17 juni 2015 jam 09.00 di kirim oleh dokter spesialis
kandungan dan kebidanan dengan diagnosa rupture perineum grade IV lama.
Riwayat persalinan pertama (tahun 2003) dengan episiotomi, bayi besar dan
rupture gr IV. Jahitan terbuka 1 minggu kemudian. Penderita tidak pernah kontrol
kembali. Saat persalinan kedua (tahun2005) Penderita disarankan untuk repair
perineum namun menolak. setelah 12 tahun berlalu dengan keluhan utama
sukar menahan BAB disertai berbagai kecemasan seksual, Penderita
memutuskan
dilakukan repair perineum. Penderita direncanakan Repair
Perineum. Lama operasi berlangsung kurang lebih 1 jam. Perkiraan jumlah
perdarahan yaitu 100 cc.
Kesimpulan
Kecemasan seksual postpartum berhubungan dengan perasaan takut yang tidak
jelas dan tidak didukung oleh situasi, dalam hal ini perasaan rendah diri akibat
bekas rupture perineum grade IV, serta kurangnya keterbukaan dan komunikasi
pasangan. pada pasien ini dilakukan Repair perineum oleh dokter Spesialis

Uroginekologi. Penanganan secara tepat, keterbukaan dan komunikasi dari

pasien, keluarga dan petugas kesehatan sangat diperlukan.

Abstract
Background
Sex is one of human needs, both biological and psychological. Anxiety is an
unclear state of fear that is not supported by the situation. In Indonesia, there
are still many women who experience postpartum sexual anxiety. This factor
relates to rupture and suture the perineum, the quality of communication, and
especially sexual relations between husband and wife. The perineum rupture can
occur due to spontaneous rupture or perineum episiotomy performed on
indications of mother and baby. Communication and holistic care can reduce
postpartum sexual anxiety.
Case
Patients 30 years old woman, gravida 2, para 1, abortus 0 come to the
department of Prof. Dr. RD Kandou on 17 June 2015 09:00 sent by the
obstetrician and midwife diagnosed with grade IV rupture perineum old. First
labor history (2003) with episiotomy, due to large babies and rupture gr IV.
Stitches was opened one week later. Patients never went for the 2nd visit. At the
second labor (2005) Patients was advised to repair her perineum but she refused.
after 12 years having major problem of fecal incontinence, accompanied various
sexual anxiety, patients decided to have her perineum repair. Patients was
scheduled for elective perineum Repair. The duration of the procedur lasts
approximately 1 hour. Estimated amount of bleeding was 100 mL.
Conclusion

Postpartum sexual anxiety associated with vague state of fear and unsupported
by the situation, in this case the feeling of inferiority due to the previous grade IV
rupture of the perineum, as well as the lack of transparency and communication
partner. the perineum repairmen upon the patient was done by a doctor
specialized in Urogynecology. correct method of handling the patient,
transparency and good communication between patient, family and health
workers is needed.
