Speech and Language Evaluation Report


Background Information
Unique is a 16 year old male who lives at Rutland Nursing Home at Kingsbrook Jewish Medical
Center. Miss Calvert, his social worker at Rutland Nursing Home, is his primary contact, and his
mother, Tammy Makins, is the secondary contact, but she does not respond to communicative
attempts nor follow up with Uniques progress. Unique is in a 12:1:4 classroom setting at 721K
at Kingsbrook.He is diagnosed with multiple disabilities and seizure disorder. He is nonambulatory and non-verbal. Unique currently has a tracheostomy tube and is being fed via a
gastrostomy tube. He smiles and enjoys music and social interactions with others, although he
does not initiate play.
Unique has not been receiving speech therapysince October 2011, and a re-evaluation was
requested. Unique was evaluated on January 23, 2014.
Testing
Unique was evaluated via the Communication Profile, the SANDI Assessment, and informally.
A standardized assessment could not be used due to his disabilities.
Based on the Communication Profile, Unique has intact sense of hearing and sight but impaired
motor and sensory impairments including hypo reaction to auditory and visual information,
hyperactivity and distractibility, persistent self-stimulation, and poor motor planning. He likes to
bite and bang objects and likes a weighted ball. Unique does not demonstrate challenging
behaviors, though he does like to bang on walls/desk loudly which can be disturbing at times. He
demonstrates intentional communication without symbols or words by reaching for items and
using non-speech vocalizations. Unique reaches for and touches items to stimulate himself with
them but does not use them functionally.
Uniques oral motor structures are intact. He drools and displays poor oral-motor posture. His
breath control is adequate. Unique imitates tongue protrusion. He attends to objects, though all in
the same manner. Unique interacts with people by demonstrating joint attention (attending to a
partner), eye contact, and shared focus (looks at objects that someone else is interested in). He
does not shift focus from object to partner and from partner to object nor does he display
appropriate turn taking, initiation, maintenance, and termination of conversation. Unique does
not request or reject objects.

Based on the SANDI assessment, Unique can make eye contact with others and sometimes
responds when his name is called. He turns to a speaker and familiar people who walk in the
room, and smiles at familiar people. He is non-verbal and presently does not use symbols to
communicate.
Unique received a tracheostomy tube and was taken off oral feeds due to extensive seizures and
hospitalization. A modified barium swallow was done recently which showed that his swallow
was intact and that he is able to safely eat orally. As a result, Unique was recently reintroduced to
oral feedings.
Summary and Conclusions
Unique is a 16 year old male with multiple disabilities. He is non-verbal and non-ambulatory.
His receptive language is also delayed. He enjoys music and social interactions with others. He
recently began to eat orally due to clearance by hospital speech language pathologist as a result
of normal modified barium swallow study. He will benefit from speech therapy to improve his
communication skills.
The following goals will be addressed during speech therapy:
Long Term Goal
Within 1 year, Unique will participate in cause and effect activities by accessing switch on a toy
independently in 8/10 trials over 5 consecutive sessions as observed by speech provider 1 time
per quarter.
Short Term Goal
Within 3 months, Unique will participate in cause and effect activities by accessing switch on a
toy in 5/10 trials over 5 consecutive sessions as observed by speech provider 1 time per quarter.
Within 6 months, Unique will participate in cause and effect activities by accessing switch on a
toy in 6/10 trials over 5 consecutive sessions as observed by speech provider 1 time per quarter.
Within 9 months, Unique will participate in cause and effect activities by accessing switch on a
toy in 7/10 trials over 5 consecutive sessions as observed by speech provider 1 time per quarter.
Long Term Goal
Within 1 year, Unique will activate switch on voice output device to indicate desired/preferred
item independently in 8/10 trials over 5 consecutive sessions as observed by speech provider 1
time per quarter.
Short Term Goal

Within 3 months, Unique will activate switch on voice output device to indicate
desired/preferred item with hand-over-hand prompts in 8/10 trials over 5 consecutive sessions as
observed by speech provider 1 time per quarter.
Within 6 months, Unique will activate switch on voice output device to indicate
desired/preferred item with physical prompts (such as tapping the hand) in 8/10 trials over 5
consecutive sessions as observed by speech provider 1 time per quarter.
Within 9 months, Unique will activate switch on voice output device to indicate
desired/preferred item with verbal prompts in 8/10 trials over 5 consecutive sessions as observed
by speech provider 1 time per quarter.
Based on the above evaluation, Speech Language therapy is recommended for Unique 3x30x1.
