REQUIREMENTS FOR ORIGINAL APPLICATION FOR PERMIT TO OPERATE


SCHEDULED INTERNATIONAL AIR TRANSPORTATION SERVICES
I. TO ESTABLISH JURISDICTIONAL REQUIREMENTS:
1. Verified Application/Petition (with prescribed CAB form)
2. Official Receipt of Payment of Filing Fee P 87, 264.00
3. Notice of Hearing issued & signed by the Hearing Officer
4. Affidavit of Publication (Proof that the Notice of Hearing was published at least once a week for three (3)
consecutive weeks in a newspaper of general circulation)
5. Actual newspaper copies of the publication
6. Proof of Service of Application/Petition and Notice of Hearing to affected airlines
II. TO ESTABLISH JURIDICAL PERSONALITY OF THE APPLICANT:
7. Certificate of Registration/Incorporation from SEC
8. Articles of Incorporation & By-Laws
9. Latest General Information Sheet (GIS)
Note: If one of the stockholders is a corporation, applicant is required to submit an Articles of Incorporation and ByLaws and/or GIS, if available, of the said corporation.
10. Secretarys Certificate regarding the present stockholders, citizenship, number of shares held, amount subscribed
and paid-up
Note: Applicant must be a Filipino Citizen.
If Partnership, submit certified true copies of Partnership Papers.
12. Mayors Permit
13. BIR Certificate of Registration
14. SSS Certificate of Membership / Registration
15. Registration of Business Name with the Department of Trade & Industry, if any
III. TO SHOW THE MANAGEMENT OF THE CORPORATION:
16. Organizational Chart
17. List of Key Officers and Board of Directors
18. Brief Resume of Officers
19. List of Pilots and their valid licenses
20. List of Aircraft Mechanics and their valid licenses
IV. TO SHOW THE FACILITIES OF THE CORPORATION:
21. List of Aircraft (owned or leased)
22. Certificate of Registration from the CAAP
23. Certificate of Airworthiness from the CAAP
24. Insurance coverages (passengers, pilots, 3rd party, hull)
25. Aircraft Lease Agreement, if aircraft is being leased / Proof of ownership if owned
26. Aircraft Specification
27. List of Tools (Equipments)
28. List of Spare Parts
29. Copy of Lease Contract (Office), if leased; Certificate of Ownership, if owned
30. Hangar Lease Contract, if hangar is being leased; Proof of Ownership, if owned
31. Air Carrier Operating Certificate (ACOC)/Air Operating Certificate (AOC) from CAAP, if any
32. Ground Handling Agreement & Catering Services Agreement
V. TO SHOW THE FINANCIAL & OPERATIONAL CAPABILITY:
33. List of proposed scheduled routes and corresponding rates/fares
34. Audited Financial Statements
35. Project Study or Business Plan of the Proposed Business for next three (3) years adopting international charter
rates/fares and routes proposal
