Assignment 4.1 Wellness Assessment and Treatment February 7, 2016 1. Stages of Change: Alisha- Preparation Stage. Evidence of this stage can be perceived in her comments of, I know I need to change, I really consider myself addicted to food and it consumes and controls my entire life. It is clear to note Alishas evaluation of the problem and expresses an awareness and understanding of the impact food addiction has on her life through evaluating eating habit since childhood and use of food as a substitute for social relationships. Some beginning impairments in her academics (missing psychology class). Alisha has clearly been thinking about change. Alishas visit to the physician clearly demonstrates movement toward or into the action stage as she expresses her desires to change her eating habits and assess what her weight loss needs to be. Jennifer- Pre-contemplation Stage (Resignation). Jennifer acknowledges her addiction to food however her mindset is clearly conveyed in her statements, Food cant hurt me, food wont cheat on me, food wont leave me. Jennifer also commented, Food is the answer to everything in my life. Clearly Jennifers friends and family recognize her eating habits as a problem however little desire was evident in Jennifer wanting to change. When family confronted her about the problem having her to weigh herself in front of everyone, she remained in denial commenting, Thats a lie, that scale is a fraud. Jennifers resignation about changing is clear. 2.& 3. Wellness Model Conceptualization Strengths/Weakness Alisha- It is clear to see Alisha has deficits within the physical self, related to areas of nutrition and exercise. Physical deficits could be potentially contributory to lower ratings in other areas of the self, such as creative (work, thinking & emotions), social (friendships) and coping self (self-worth, stress management and realistic beliefs). Obviously Alisha has a supportive relationship with her mother in terms of love from her family. It was difficult for me to identify other strengths for Alisha due to limited information. I would like to know more about particular areas in order to ascertain more strengths. I want to know more about coping areas such as incorporation of leisure. Evaluations of the creative elements such as positive humor and essential elements such as spirituality and how they influence wellness for Alisha also need to be assessed. Formal assessments may be able to provide more information in this area.
4. Personal Wellness Plan
Alisha- Identifying physical self as an area to work on as well as social for Alisha
Wellness Plan: Physical Self Nutrition & Social Self-Friendship
Alisha reports that she would like to work on developing healthier eating habits and portion control. She indicates challenges with this due to living with her mother and home cooked meals are rare and convenience foods are made available. Alisha also reports that when she has access to cooked meals she is consumed by larger portions and often has difficulty making healthy selections/choices. Alisha also talked about challenges she had with making and maintaining friendships and her desires to increase relationships in hopes that it counteract how she uses food as a replacement for relationships. Alisha was helped to complete a wellness plan to work on nutrition through beginning changes such as food choices replacing food with healthier choices. Alisha will be supported through a referral to a nutritionist to alter eating habits and work with her on portions. Eliciting support from Alishas mother to work on selection of healthier food choices in the home for Alisha to select. Alisha identified that she engages in mostly sedentary activities such as watching TV, playing on her cell phone. She desires to meet new friends and considering this is an area she desires to work on she agrees to join a teen support group for eating addiction that incorporates a walking group with other same age peers to not only address her physical needs for exercise but also potentially begin to form relationships by making friends. Alisha will record her participation in her weekly activity log. Alisha will also follow up with her physician for regular weigh in and check up to monitor her health.
