JOB CONTRACT
This CONTRACT for Technical and/or Professional Service entered into and between the
Republics of the Philippines through DepEd, Division Office of Catarman Northern Samar,
Represented in this CONTRACT by its School Head _____________ & MRS. ___________________legal
age and resident of _____________________________ after referred as JOB ORDER.
1. That I, ____________________ herein after known as Party of the First and Mrs.
________________________ known as party of the Second Part, both parties being Filipino
Citizens do hereby agree to enter a contract of work performance under the following
condition/stipulations with the party of the Second Part to execute and work on the job
herein describe. To perform the duties and responsibilities as Utility Worker, at Paticua
Elementary School, Catarman, N. Samar.
2. That the contractual personnel agree to devote his Knowledge, training and skills in the
employment of Utility Worker from July 1 to Dec. 31, 2016 inclusive from after the date
of this contract for the sum of _______________ pesos Monthly as per allotment of the
MOOE from the School without additional benefits shall grant, subject to the availability
of funds for the purpose.
3. That it is granted that the employer, at its discretion and without change of
emoluments and other privileges may transfer CONTRACTUAL PERSONNEL TO
ANOTHER assignments within the school when his skills and training is needed.
4. That this contract may be renewed by subsequent contract between the parties herein
for the interest of the service and/or valid reasons Party of the Second Party may also
terminate prior to the date of termination of his contract.
5. That it is agreed further bun the parties herein that this contract shall cease to be in
any force and effect in the event of unsatisfactory performance of work by
CONTRACTUAL PERSONNEL or for such cause maybe provided by the law.
6. That the party of the Second Part shall observe the regular working hours for a given
day and during night shift in the same manner as that prescribed for government
offices, however including Saturdays, Sundays and Holidays without extra
compensation unless authorized by higher authorities concerned.
7. That this contract entered into a good faith with full knowledge of the condition and
stipulation herein described by our free will and violation and without mental
reservations or purpose of evasion.
IN WITHNESS WHEREOF, we hereunto affix our signatures this ___ day of___ 2016 at
Catarman, Northern Samar.
