PURPOSE

This Standard Operating Procedure (SOP) is issued and maintained by FO/MNT/ELC and
reviewed by HSE as it belongs to the TOTAL E&P INDONESIE (TI) HSE Manual.
The objectives of the Electrical Safety Procedures (ESP) are the following:
- to provide directives for the application of safety electrical work rules,
- to eliminate personnel injuries, reduction of damage to equipment/property and to
prevent reoccurrence,
- to establish responsibility and accountability,
- to assure the safety of the company's employees and contractors who work on electrical equipment,
- to ensure the continuity of the electrical supply in the best safe conditions.
A hard copy of this procedure is issued to Electrical Section at each site. This SOP is also
part of the TI CMS, Corporate Management System, available at Intranet.

SCOPE

This SOP is applicable in all TI premises and entities.


Any works related to electricity including instrumentation and mechanical works, which require isolation and de-isolation of electric power supply shall refer to this SOP.
Each employee belonging to TI or contracted by TI who may be concerned with the operation or work on electrical equipment / installation within the TI premises must strictly follow
this SOP.

DEFINITIONS

AC/ DC: Alternating Current/ Direct Current


Accessible Parts: Parts of electrical equipment or apparatus, which are liable to be touched
by persons.
Circuit of an Installation: Assembly of electrical equipment (conductors, appliances, etc.)
having different phases supplied by the same power source and protected against
overloads by the same protective device.
Dead: at or about earth potential and disconnected from any live system.
De-Isolate: to remove earthing and/or make live by switching on control and mains supplies
to an equipment or system.
Electrical Shock: Pathophysiological effect of the passage of an electric current through the
human body.
Electrical Ability Permit: is issued by the Sr. Electrical Authorized Person to a TI employee
and the employee contracted by TI who meets the definition of an Electrical Authorized Person. It will state the type of work the person can undertake and any limitations that may apply, as described in this procedure.
Electrical Authorized Person: is a person who has sufficient technical knowledge and experience to enable him to avoid danger when carrying out specific work on electrical
equipment as certified by Sr. Electrical Authorized Person.
Electrical Installation: Group of circuits for a given function and having coordinated characteristics.
Electrical Isolation: to make a piece of equipment or electrical system "Dead" by switching
off all sources of electrical power and connect it to the earthing network.
Electrical Work: all operations on connection or insulation of equipment, installation of electrical devices, cabling work, etc on dead equipment of electrical installation.
Hazardous Area: Area/location that classified has potential to create explosion due to presence of gas
Insulation: All the insulators involved in the construction of equipment for the purpose of insulating the live parts, also designates the act of insulating.
Basic Insulation: Insulation applied to live parts to provide basic protection against
electrical shock.
Supplementary Insulation: Independent insulation applied in addition to the Basic
insulation, in order to provide protection against electrical shock in the event
of a failure of the basic insulation.
Double Insulation: Insulation comprising both Basic Insulation and Supplementary
Insulation.
Intervention: operation activities such as test, troubleshooting or measurement on an electrical installation

Isolation Responsible: Electrical authorized person who has responsibility to lock off/on
equipment on specific location based on his/her ability permit.
Isolation Log Record: is a book/record located in site electrical maintenance section which
is used to record all information concerning the work carried out and the user of the
padlock. Refer to the form attached to this SOP.
Live Conductor/ Parts: Conductor or conductive part which is energized in normal operation, including a neutral conductor.
Locking off: is performed to put and maintain the equipment in one status; stop, draw-out
and test position in such condition that nobody is able to manipulate without authorization.
Maneuver: Successive operations leading to an electrical installation status change (switching on, switching off, closing and opening loops...) with switches, breakers, isolators
and other devices specially designed to this purpose.
Except the maneuvers, whatever the voltage class, all other operations performed on
the electrical installations or equipment are categorized as works.
Neutral Conductor: Conductor (in an AC system) connected to the neutral point and liable
to carry current.
Operating Authority: is formally appointed by the RSES and he is directly responsible for
acceptance of permits at the planning stage for operations of which he is in charge.
Refer to SOP 03-001 Work Permits.
Partial Electrical Isolation: this type of isolation is performed to avoid any possibility of putting back into service an electrical motor or device driving a machine, on which a
non-electrical job has to be carried out. Locking off request is required on the electrical isolation permit and on the main work permit.
Performing Authority: TI representative in charge of the work performance. He is present
on the installation or in the immediate vicinity for the entire duration of the work. Refer to SOP 03-001 Work Permits.
RSES: Responsible for Health, Safety and Environment on Site appointed by General Manager. Refer to SOP 03-309 RSES.
Safety Padlock: padlock, which used for locking off purpose.
Safety Extra-Low Voltage (SELV): denotes a voltage, which does not exceed 50 V a.c.
r.m.s. or 120 V d.c. between conductors or between any conductor and earth in a circuit which is isolated from the supply mains by means such as a safety isolating
transformer.
Sr. Electrical Authorized Person: is the Senior Electrical Engineer who is appointed by
Executive Vice President & East Kalimantan District Manager (EVP) in written, and
must be a competent person on electrical subject and has the responsibility of issuing Electrical Ability Permit for TI.
Substation: the part of power system, concentrated in given place, including mainly the
terminations of transmission or distribution line switchgear and housing and which
may also include transformers. It generally includes facilities necessary for system
safety and control (e.g. the protective devices).
Switchgear: A general term covering switching device and their combination with associated control, measuring, protective and regulation equipment, also assemblies of
such devices and equipment with associated interconnections, accessories, enclosures and supporting structures, intended in principle for use in connection with generation, transmission, distribution and conversion of electric energy.

Total Isolation: this type of isolation is performed to allow work on electrical equipment.
This equipment is out of service, current switched off without any possibility of it coming back and all other powers (pressured air, potential energy, etc) are disabled.
Locking off/ isolation requisition is required on the electrical permit and the main work
permit.

OPERATIONS OF ELECTRICAL SYSTEM


5.1 WORK ON LIVE PARTS
As a general rule, the interventions or the works of electrical field shall be performed on
dead equipment or installation.
Work on High Voltage is absolutely prohibited.
Work on live-parts (only on Low Voltage) could only be performed under the following
circumstances:
- 1st case: the switching off of the installation or equipment may jeopardize the
life or the health of person,
- 2nd case: imperative necessities of exploitation prevent the switching off of the
installation or equipment,
- 3rd case: the nature of the work itself requires the presence of voltage (verification of circuits, relays, measuring devices, search and localization of faults,
etc.).
On above 1st and 2nd cases, the specific working procedures and safety measures have
to be set up by Site Electrical Supervisor and approved by Head of Electrical Method
Service and during work execution, Electrical Authorized Person shall supervise the
work closely.
For above 3rd case i.e. calibration, test, measurement and recording of electrical network are considered as intervention on live parts with the following condition:
- only could be performed on LVA,
- could only be done by competent person having ability permit to do so,
- precaution should be taken to avoid electrical danger,
- all working and used measurement devices are in safe and proper condition.

5.2 INTERVENTION
In case of any doubt (in implementation of the following rules), it is the responsibility of
the RSES to appreciate if electrical works require the application of the work permit procedure or not.
5.2.1

WITHOUT WORK PERMIT AND ELECTRICAL ISOLATION PERMIT

Unless:
- RSES does ask for issuing a Work Permit,
- the works are located within an Explosive Area,
the following works do not require the Work Permit and Isolation Permit:
- replacement of LV fuses,
- replacement of lamps and ballast including minor repair,
- systematic maintenance of battery,
- running test of emergency generators,
- running test of turbine generators,
- change-over turbine generator operation,
- restarting of thermal electric generator/CCVT,
- all works in workshop area,
- routine systematic maintenance and performance test on engines, HVAC,
domestic equipment and generators.
However:
- all above works should be recorded in a logbook available in control room,
- those interventions should always be made in the absence of voltage (if applicable).
- those intervention shall be covered by written instruction
5.2.2

WITH WORK PERMIT AND ISOLATION PERMIT

The work should not be started without following existing work permit procedure.
If work location is not in Electrical Room, each working zone should be delimited by
protection tape
Passage for evacuation should be preserved every time.
The work could be started only when any possible voltage sources are isolated.
The electrical supervisor shall issue an electrical isolation permit to cover electrical
locking off and total isolation.

5.3 WORKING IN HAZARDOUS AREAS


When working in classified area, with risk of presence of ignition source, emission of nonaked flame work permit associated or not with risk assessment shall define precautions to be implemented to avoid explosion, such as but not limited to:
- use of non sparking tools or intrinsically safe
- ventilation of the atmosphere,
- permanent monitoring of presence of explosive gas

5.4 ENTRANCE TO SUB STATION, ELECTRICAL TECHNICAL ROOM OR TECHNICAL


SWITCHGEAR
Entry to electrical room housing HV apparatus which is live is strictly limited to electrical
authorized persons or someone accompanied by him/her.
All Substations, Electrical Technical Rooms or Technical Switchgears shall be kept
locked. The keys shall be available in Electrical Supervisor Room.

5.5 OPERATION OF SWITCHGEAR


Circuit breaker or switch must not be operated without the presence of Electrical Supervisor or his/her delegation (who having suitable Electrical Ability Permit to do so) except
in circumstances where any circuit breaker or switch has to be opened to prevent danger to life or in extreme emergency.
When a circuit breaker or switch has been opened, it must not be closed again without
consent of the Electrical Supervisor or his/her delegation who has suitable Electrical
Ability Permit.
No switching operations could be performed by persons other than those authorized to
do so.
All electrical distribution switching operation will be reported, detailed sequenced and
suitably time/date recorded in Isolation Log Book.

5.6 RE-CLOSING AFTER FAULT OPERATION


No incoming or outgoing circuit breaker in substation may be re-closed after fault trip
until the cause of tripping has been definitely ascertained.

5.7 OPERATION OF CIRCUIT BREAKER IN SUBSTATIONS & ELECTRICAL ROOMS


The operations of circuit breaker and means of isolated on the distribution network will
be performed only by Isolation Responsible.

5.8 OPERATION OF OTHER ELECTRICAL EQUIPMENT IN PLANT AREA


The start-stop operations of motors driving process equipment and the switching of
other electrical equipment such as plant lighting, process heating, etc. will be carried out
by operators or maintenance personnel having Ability Certificate to do so and also as
required by the process operations.

5.9 CUTTING AND JOINTING OF CABLES


Before any cable is cut, it must be identified and made dead and when practicably
should be earthed.
Cutting of cable in hazardous area requires a hot work permit procedure.
Jointing of cables shall be performed using only the appropriate material.

5.10

MECHANICAL WORK ON ELECTRICAL DRIVEN MACHINE

Locking off (partial/electrical isolation) and locking off all other sources of power are
necessary, and must appear on main work permit. For this type of work among aother
requirements (refer also to SOP 03-002), an electrical isolation permit is required. After
checking that the right machine has been locked off, no possibility of local or remote
start-up, using padlocks, then work can be started on the machine.

WORK IN A SUBSTATION OR CLOSE-TO ANY ELECTRICAL LIVE INSTALLATION

The work does not require electrical isolation work as long as there is accessible livepart, however main work permit is necessary.

5.12

EXCAVATIONS

Refer to the SOP 03-508, Excavations.


Before any excavation or other underground work is started, the necessary excavation
permit must be signed by an Electrical Authorized Person.
During work execution, a competent person should be at work location to ensure that
cables or cables covers are not damaged, disturbed or removed without the specific
consent from an Electrical Supervisor.
A cable layout drawing shall be attached to the excavation permit.

5.13

SIGNAL BETWEEN PERSONS

Instruction to make apparatus alive or dead following a given signal or pre-arranged understanding after an agreed interval of time, verbally or by body language is strictly
forbidden. All actions for isolation and/or de-isolation shall be based on this procedure.

5.14

REPORTS TO ELECTRICAL SUPERVISOR

Occurrence of any fault or abnormal condition or operating of protecting relays from


whatever cause shall be reported as soon as possible to Electrical Supervisor.
During failure of electrical supply, all electrical devices, equipment and conductors shall
be considered as being alive until isolated.

5.15

PORTABLE ELECTRICAL APPARATUS

Refer also to SOP 03-315 Hand and Portable Power Tools.


5.15.1 REQUIREMENTS
In order to minimize the effects of electrical shock, all portable electrical tools and
lamps shall be:
- Safety Extra Low Voltage (SELV) Class, or
- Using Cordless Battery power, or
- double insulation class II with the double square identification sign, or
- supplied by a feeder equipped with a residual differential 30 mA device including ground conductor,
In the very conductive area or enclosure, only SELV Class tools and lamps shall be
used.
If the space is very limited (such as small tank, etc), the LV-A/SELV isolation transformer shall be placed out of the working/ enclosure area.
5.15.2 INSPECTION
All Electrical portable power operated apparatus shall be subjected to regular inspection by site authority.

ELECTRICAL ISOLATION
6.1 LOCK OFF
6.1.1

PADLOCK & KEY

There are two series of padlocks, which have to be used:


- A series: Electrical Isolation Padlock, used by electrical section only,
- B series: Particular Safety at work for the work performers, used by all
maintenance sections and other party (ies).
Consequently:
- a minimum of one padlock (A series) shall be installed when a permanent or
temporary isolation concerns only maneuver,
- a minimum of two padlocks (A+B series) shall be installed when the isolation is required for a work or intervention.
All padlocks shall be key operated.
All maintenance sections shall have their own padlocks and keys.
Each padlock shall be engraved with the following information:
- Site: BSP, TTN, HDL, NPU, BPN, SPU, etc.,
- Section: Mech (M), Elect (E), Inst (I), Other (C), for operator, contractor,
etc.,
- A or B series and identification number as following examples:
A series: Electrical (E) padlocks to have numbers from 01 to 99,
B series: Electrical (E) padlocks to have numbers from 100 to 199,
Mechanical (M) padlocks to have numbers from 200 to 299,
Instrument (I) padlocks to have numbers from 300 to 399,
Other (C) padlocks to have numbers from 400 to 499.
6.1.2

PROCEDURE

1. Padlock and keys are stored in keyboards installed in each section supervisors
office.
2. If require, a master key or second keys for all safety padlocks are to be kept in
keyboard installed in the Site Maintenance Superintendent office for emergency
use only.
3. Keys for substation doors, electrical rooms and transformers fence doors, are
stored in the Electrical Supervisor's office.
4. Padlocks and keys should be delivered to each employee who works on isolated equipment by his section Supervisor.
5. Padlocks and keys shall never be lent.
6. Padlock used for isolation shall be special type with unique key (not generic
type).
7. A tag has to be attached to every padlock to identify the Work Responsible or
Isolation Responsible. It is recommended also to indicate the reason for locking-off and the Work Order Number.
8. Each corresponding Supervisor shall draw up a list of the assigned and installed locks, with their discreet.

9. After the circuit breaker or switch has been opened, the worker or workers who
will be involved in the job snap their lock or locks on the control lever/ locking
off arrangement or on multiple lock adapters.
10. When the work is completed and checked by the Performing Authority, the isolation responsible is the last to remove his padlock. (A type).
11. Removing the electrical padlock should be under the supervision of the isolation
responsible.
12. All padlocks and keys shall be returned back to the responsible Supervisor at
the completion of the work.

6.2 ELECTRICAL ISOLATION PERMIT


6.2.1

GENERALITIES

1. For each work which requires an electrical isolation permit, a separate electrical
isolation permit shall be issued, even if the same item is already isolated for
other work. In this case all the works shall be completed before the electrical
isolation could be cancelled.
2. The Electrical Isolation Permit is required for any intervention or work on
equipment or an installation, which has to be electrically isolated.
3. It can only be issued as a complimentary permit to the main work permit and
can never be used alone. It has to be signed off to indicate that the equipment
has been de-isolated.
4. The electrical isolation shall use padlock and follow the lock off procedure (especially for HV equipment) available on each site for each equipment.
5. When the electrical section requires locking off/ electrical isolation of equipment
for electrical work, the same procedure shall be adopted.
6. Separation of equipment or network from possible voltage sources should be
clearly visible and secured (breaker or contactor draw-out, perfect operating indication).
7. The Electrical Isolation Permit can only be prepared by Electrical Authorized
Person and the isolation work should only be done by Isolation Responsible.
The isolation work has to be documented in special isolation logbook. Basically
a record of each activity will be made as follows:
- the work permits number,
- the tag number of the equipment concerned,
- the padlock number,
- the name (and badge number) of the work responsible,
- the date and time of this operation started,
- the date of return of the padlock (and key) to the section Supervisor,
8. The isolation logbook should be kept in a good manner.
9. The five copies of the isolation permit should be signed together.
9.1. During performing work:
- white copy shall be put in work place and available there permanently
and to be presented at any request
- yellow copy shall be kept by Operating Authority
- blue copy shall be put in Control Room
- pink copy shall be kept by Electrical Authority
- green copy shall be kept by RSES

9.2. After signing off:


- white copy is kept by Performing Authority,
- yellow copy is kept by Operating Authority,
- blue copy is kept in Control Room,
- pink copy is kept by Electrical Authority,
- green copy is kept by RSES
10. When signing, each responsible shall clearly indicate his name, the date and
time of the signature.
6.2.2

REQUEST

The request for an electrical isolation permit shall be issued during the "daily work
permit coordination meeting" by electrical authorized person. The requestor shall:
- determine what type of isolation will be carried out,
- describe the nature of work to be carried out,
- indicate precisely the equipment to be locked off, the tag number and the
location,
- check all possible safety procedures.
The RSES or his/her delegate shall:
- verify the equipment to be locked off confirm to the main work permit,
- acknowledge the electrical isolation permit to be undertaken by his signature.
6.2.3

EXECUTION

The Electrical Supervisor shall nominate the isolation responsible to perform the
electrical isolation.
The Isolation Responsible shall:
1. identify the equipment/ network to ensure the work, which will be carried out
is on the isolated network,
2. read and follow the isolation procedure of the equipment isolated,
3. inform to control room to make sure the lock off is safe to be done,
4. verify the lack of voltage on the equipment or network,
5. put in place, if necessary, screen for prohibiting the access to the live parts
if exist,
6. short circuit to earth the downstream conductors by closing the earthing
switch if available and padlocking it in the closed position,
7. put the equipment in the desired condition and observe that the available
indications show the desired condition; the indication should work properly,
8. put padlock for prohibiting any manipulation,
9. when total isolation of the equipment is completed, the working zone must
be marked out with barricades or flags, if the work place is not in Electrical
Room,
10. fill in the electrical slot on the permit with the reference number of each padlock applied and sign,
11. fill in the isolation logbook.

When working on power transformers and putting them in parallel, the isolation shall be made on both primary and secondary side, since low voltage
on secondary can produce high voltage in primary.
Opening of the circuit which is supplied by the secondary of current transformer when the primary side is live or likely induced current is strictly forbidden. If opening is required, the secondary terminals of the current transformer shall first be sorted using an appropriate device.
CHECKING

The five copies of electrical isolation permit form are filled up and signed all together and distributed accordingly.
The Electrical Authorized Person shall check that the isolation is correct, perform
the necessary tests and sign on the Electrical Authorized Person slot.
Note: it is compulsory that tests of HV locking off/ isolation are performed by Isolation Responsible.
The Operating Authority shall:
- check the electrical isolation,
- if required, fill up and sign the special instruction slot by himself / herself.
The Performing Authority shall:
- check the isolation conditions,
- put his own padlock,
- fill the padlock reference number in electrical isolation permit,
- ensure that the workers have the ability permit level required to perform the
work.
The work will start only when the Performing Authority is in possession of both
given main and electrical permits.
6.2.5

SIGN OFF

The five copies of electrical isolation permit form are collected, signed all together
and archived accordingly.
The Performing Authority shall:
- remove his padlock,
- sign in the Performing Authority slot.
The Isolation Responsible shall:
- read and follow the de-isolation procedure of the isolated equipment,
- upon approval of the Operation Authority, put the involved equipment back
in service,
- perform de-isolation when any electrical isolation permits concerning this
part are already returned to him,
- return the equipment/ installation to service, sign in the signing-off slot.
The Electrical Authorized Person shall:
- check that the de-isolation is correct,
- perform the necessary tests,
- sign on the Electrical Authorized Person slot.
The Operating Authority shall:

check that the deisolation is correct,


accept the de-isolation by signing on the Operating Authority slot.
UNCOMPLETED WORK

The Electrical Isolation Permit will never be closed without completion of the work.
If the works are not completed and the validity of the main work permit has expired
then:
- the installation is kept isolated,
- the Electrical Isolation Permit remains valid and open,
- the reference number of valid Electrical Isolation Permit must be written on
the new main work permit,
- the work can be continued.

ELECTRICAL ABILITY PERMIT


7.1 GENERAL
When a Contractor's employee works in TI plant as an electrician, the contractor must
prove his/her electrical competence and electrical safety knowledge to the satisfaction
of the Site Maintenance Superintendent and the Electrical Supervisor.
All permanent employee and permanent contracted personnel working on electrical installation must obtain an Electrical Ability Permit from Sr. Electrical Authorized Person.
Each employee (permanent or contracted) who works in electrical installation in Total
Indonesie Sites has to know the content of this SOP 03-006.
The person shall follow the electrical training before getting his / her ability permit and
shall pass/ succeed the training test.
The validity period of the ability permit is 3 (three) years and could be extended every
other 3 (three) years. Extension of permit could only be issued if the person has followed the refreshment training and pass/ succeed the training test.
The ability permit is only valid for specific site, type of work, etc., as indicated on the
ability permit. Consequently, each person who moves to other site shall obtain the new
ability permit upon request to the Electrical Supervisor.
The ability permit is strictly personal and non-transferable to other persons.
The holder must carry his/ her ability permit all the times during working hours or have it
easily available.
In the event of loss of his/ her ability permit document, the Electrical Supervisor should
be informed immediately.
The ability permit does not follow the order of hierarchy. It concerns only the safety consideration of electrical operations.
The ability permit does not authorize the holder to perform an operation or works covered by it on his/ her own initiative.
Each Supervisor could ask ability permit for his/ her subordinates to Sr. Electrical Authorized Person by mentioning the capability and competence of the proposed holders.
Site Sr. Electrical Supervisor will keep the record of all the Electrical Authorized Persons
at his/her site and inform Sr. Electrical Authorized Person for any change.

7.2 ABILITY PERMIT QUALIFICATION


Ability permit is identified by Letters and Numerical Indexes
7.2.1

LETTERS

First Letter: L (low voltage) and H (high voltage)


L & H characterizing the voltage range of equipment and installations on which
the entitled qualified person is authorized to work or perform an intervention

Second Letter:
R: Intervention responsible, person who:
- has to be aware of application of safety regulation,
- is responsible of interventions or maintenance work on Low Voltage
range only.
C: Isolation responsible, person who is responsible to perform electrical isolation and takes the corresponding safety measures.

7.2.2

NUMERICAL INDEXES

Characterizing the functional level of person who is entitled to perform the job:
0: Non electrician: person who:
is not in charge of the interventions or works in the scope of electricity,
- is authorized only to enter into substations to do the works such as
cleaning, maintenance of premises and reading of measurement apparatus.
1: Work Performer: person who:
- is in charge of the interventions and the maintenance works in the scope
of electricity,
- can only take action after having received a written or verbal instruction
from the work or intervention responsible.
2: Work Responsible: person who:
- has to be aware of his own safety and the personnel under his responsibility,
- shall continuously supervise his personnel (if necessary).
7.2.3

LETTER AFTER NUMERICAL INDEXES

This letter specifies a special assignment:


V: indicates that the holder can work or perform an intervention on the proximity
of live parts.
