INCIDENT REPORT
Date: June 9, 2016
At around 8:20 in the morning of June 9, 2016, Mrs. Shirley U. Delim, Grade 5 teacher of
Macatbong Elementary School complained that her classroom was open and asked me if I saw
someone who entered her room because some of her things were scattered and are not in their
proper order. A broken pink plastic piggy bank was also found on the floor. I asked Mrs. Delim if
Milesly, her daughter may have entered her room and played there. She quickly responded that
her daughter would never do that. As Mrs. Delim opened the drawer in her table, we found out
that there were also malicious drawings in her folders.
Upon knowing the incident, we hurriedly reported it to Mr. Manuel Taruc who is the
night shift security in-charge of the school. Mr. Taruc told us that he already knew that the room
was open and someone entered the room. He also narrated to us that most of the things,
especially the staple wires, were scattered all-over the floor. He also added that aside from the
malicious drawings in her table drawer, there was also a drawing in her wall. Mrs. Delim was so
shocked to know that her newly painted wall was tainted with a drawing of a private part of a
male using a marker pen.
Mr. Taruc explained that as of 1:00 in the morning June 9, 2016, he took his daily round
around the school and nothing was unusual so he went home to look after his son who was
currently suffering from a fever. At around 3:00 in the morning of the same date, Mr. Taruc went
again to the school to take his daily round and was alarmed that the lights in the classroom of
Mrs. Delim was already on and the door of her room was also open. As he entered the room, he
saw that there were school supplies scattered on the floor and on top of the table. Mr. Taruc
added that he picked-up the staple wires which were scattered on the floor and put it on top of
the table.
Mrs. Delim added that some school supplies in her drawer were also missing. We also
found out that the pink plastic piggy bank was not owned by Mrs. Delim so we asked the other
teachers to know who owns the piggy bank. Afterwards, I reported the incident to the school
principal, Mrs. Eufemia C. Santiago. She hurriedly went to the room of Mrs. Delim and asked
what happened.

Later on, I found out that the piggy bank was owned by a sari-sari store owner located in
front of the school. The said piggy bank was stolen from her store.
I also found out that Mrs. Racquel DC. Catumber, a Grade 1 teacher of Macatbong
Elementary School also experienced the other day that the door of her classroom was also open.
However, since nothing was missing, she did not reported it. Ms. Analyn Dalit, Grade 2 teacher
of Macatbong Elementary School also mentioned that Mrs. Kimberly Mae Valenzuela, a Grade 4
teacher of the same school also lost something in her room during the Brigada Eskwlela but did
not also reported it.
