(i) Replace all existing broken / worn out
chairs with new chairs in which the height
is adjustable and good lumbar support.

2. Risk of injuries from falling objects


due to improper arrangement or
shelfing of boxes / cartons and faulty
electronics equipment (waiting for
approval to dispose) in the store room
and blocking clear way.

(i) Order to rearrange the cartons and
faulty equipment according to the
stacking and unstacking procedures on
the existing shelves. Example: Heavier
objects are advisable to be kept on the
bottom of the shelves and ensure no
overloading.

storage of new and used 12V lead-acid
car batteries in the store. Inadequate
PPE for handling lead-acid batteries. No
acid resistance hand gloves and spill
kits.

(i) Order to place all used / faulty leadacid car batteries to be kept outside the
building. Battery were placed in the
covered plastic container when
transporting and handled by two persons
to reduce the chance of any possible slip,
trip or acid burn injury (direct contact)
during transporting. The batteries were
kept on the concrete ground to prevent
direct leakage to the ground or drain.

(i) Require a separate storage area for all
chemical and bio-hazard substances to
preferable outdoor with good ventilation,
concrete ground with roof and fencing.
(ii) Adequate PPEs like spill kits, eyes
protection, suitable gloves and training
should be provided to all store personnel.

(iii) Store personnel should be warn that
actions will be taken against them if they
