Internal Medicine Conversation

Situation 1
/

A.XX

A. Mr./Ms. XX, come into the doctors office, please. Can I have your NHI card please?
Thank you.
,

After seeing the doctor, take this printout to pay at the registration desk on the first floor.
After paying, have your X-rays taken on the second floor, then come back here.
B.

Please take the printout to the cashier to pay your fees first. After paying, let them draw
blood for a blood test in the Investigation Room next door. Then, pick up your
medicine at the pharmacy.
B.

Please pay the bill on the first floor. Remember to pick up your medicine at the pharmacy
after that.

Situation 2

B.

Dont forget, your next appointment with the doctor is next Tuesday morning.
B.

Before seeing the doctor again, they have to draw your blood for a blood test. Please
remember not to eat or drink anything after midnight the night before, not even water.
A.
Ok, thank you!

39

Situation 3

A.

Your blood sugar index is still a little high. Please remember to take your medicine
regularly and stick to the diet prescribed by the dietician.
B.

Also, winter is coming. Keep your feet warm, and drink enough water.
B.

If you feel sick or you vomit or have a fever, you cannot stop taking your medicine. You
should come back to the hospital immediately.
B.

Regular exercise can improve the response of body cells to insulin and control blood
sugar more easily. Therefore, doing regular exercise or sports is recommended.
B.

However, before doing exercise or sports, do a physical examination first, like the tread
mill, retina examination, and autonomic nervous system examination to make sure which
exercise or sports are suitable for you.

A.

Situation 4

Ms., Is there anything I have to look out for regarding my high blood pressure /
hypertension?
B.

You have to visit your doctor regularly to check your blood pressure. Maybe youd better
learn how to measure your blood pressure yourself. Take the medicine your doctor
prescribes and dont reduce or increase the dosage without his advice.
B.

40

Avoid eating high fat foods like liver or pork fat and fry your vegetables with vegetable
oil. Eat sufficient lean meat and tofu, which do not have too much sodium.
B.

Keep doing sufficient exercise or sports, such as calisthenics and constitutional and
stretch exercises. Remember to keep your mood free and easy, not to do things
suddenly, and to stay calm so your blood pressure dont go up.

Situation 5

A.

You have a peptic ulcer but its not too serious. Take your medicine and see your doctor
regularly, and be careful of what you eat and drink.
B.

Avoid spicy food, alcohol, and coffee, and try to stop smoking. Follow a healthy diet and
dont skip or delay any of your three meals. Dont eat too much or too little at a time.
Chew your food thoroughly and swallow it slowly.
B.

Also, take life easy and dont get nervous. Take a thirty-minute rest before your meals.
Avoid eating food that is too hot or too cold, food with tough fiber, and unboiled food.
If you have some urgent situation, such as vomiting blood, pain in your abdomen, or if
your stool (egesta) is black, come back to the hospital immediately.

41

Surgical Conversation
Situation 1

B.

Be careful with your wound. Keep it dry for a week.

A.

Is there anything else I have to watch out for?

B.

Dont carry anything heavy for the next three months. That will affect the wounds
agglutination. If the wound gets red, swells, feels very hot or painful, or is secreting
pus (has pyogenic secreta), please come back to the hospital immediately.
B.

If the skin around the wound is itchy, dont scratch it, as it can infect the wound. You can
pat on the wound instead.
B.

7-10

Because there is no exudation (leaking of pus) from your wound and we have put
surgical tape on it, you dont need to change the dressing The only thing you need to do
is to keep it dry. After the surgical tape falls off by itself in about seven to ten days, you
can put on a new dressing.
Situation 2

B.

There is a drain in your wound, so please keep your wound dry and the gauze clean at all
times. Generally speaking, change the dressing once daily, but also change it if the gauze
gets wet.
B.

42

If you want to check the wound, please wash your hands and make sure that they are
thoroughly clean and dry. Remember not to touch the wound when you open the gauze.
The wound should be kept aseptic.
B.

If the skin around the wound is itchy, dont scratch it by your hand. You can pat the
wound instead. This can prevent you from infection.
B.

When you change the dressing, wash your hands well with soap. Dip aseptic cotton
swabs in a saline solution, and then remove the secretion on the wound.
B.
5

Then, clean the wound with aseptic cotton swabs and iodine.
5
. Remember not to rub the swab back and
forth on the wound.
B.

Finally, use a cotton swab to clean the two sides of the drain. And then, use iodine to
clean the top again. After finishing, close the wound with clean gauze.
Situation 3

B.

The doctor said that the wound of your peptic ulcer is healing well. Come back and see
the doctor after one month. However, there are several things you have to keep in
mind.
B.

First, you cannot use tobacco, liquor, and drinks with caffeine like coffee, coke, and tea.
43

Also, avoid spicy condiments, such as pepper and chili.

B.

Take the medicine as prescribed. Dont take any over-the-counter no steroidal

anti-inflammatory drug (NSAID), such as aspirin or ibuprofen.
B.

Keep a regular diet. Dont skip or delay any of your three meals. Dont eat too much or
too little at a time. Chew your food thoroughly and swallow it slowly. Also, stay calm
and peaceful while eating to prevent the illness from re-occurring.
B.

You still have to watch out for symptoms of a setback. If you have severe pain or
abdominal distention, if you vomit acid fluid or blood, or if your stool (egesta) is black
or bloody, please come back to the hospital immediately.

Situation 4

B.

You just had abdominal cavity surgery.[RPDT1] Avoid eating high fat foods. Some
patients might have a slight diarrhea after surgery. However, most people improve in one
or two weeks, so dont worry too much about it.
B.

You can take a shower, but dry the wound after showering. Generally, you dont need to
apply any ointment on the wound. If the wound gets red or painful, swells, has some
abnormal secreta, or if you have a fever, please come back to the hospital immediately.
B.

You dont have to limit your daily activities. You do things normally.

44

B.

Remember to take the medicine as prescribed. See the doctor again as indicated on the
out-patient pre-registration form to remove the sutures.

45

Obstetrics-Gynecology Conversation
Situation 1

B.

When was the first day of your last menses?

B.

You will do a urine examination and blood test later. I will give you the form. Remember
to go to the cashier on the first floor.
B.

The doctor is going to do an ultrasound examination for you in a few minutes. Please lie
down on the bed/ examination table. Let me help you pull your clothes up to your
chest, and pull your pants down to the pubis. The doctor will be here in a moment.
B.

The examination is finished. Wipe the lubricant from your abdomen with this warm
moist tissue. Come back to the doctor later and he will explain the report in detail.

B.

Situation 2

Do you know how to use vaginal suppositories? When using them before going to bed,
first wash your hands with soap. Lie down on your back, bend your legs, and put the
vaginal suppository into your vagina about one index finger deep.
B.

After putting the vaginal suppository in your vagina, try to avoid walking. This will
prevent the vaginal suppository from falling from your vagina. The next morning, the
drug will have melted and will drain from your vagina. You can put on a sanitary pad
to keep clean.
46

B.

In order to keep dry, wear cotton panties instead of a girdle, pantyhose or tight pants.
B.

Dont use sanitary pads too often, except for during the menstrual period when you have
to change them very often.
B.

Keep using the vaginal suppositories prescribed by your doctor. Dont stop using them
before the infection has been cleared up. If you menstruate a lot, stop using the vaginal
suppositories for a while. If your vagina doesnt feel well while you are using the
vaginal suppositories, such as a burning feeling or irritation that wont go away, see
your doctor again.

B.

Situation 3

Common symptoms of menopause are hot flashes, night sweat, vagina infection or
itchiness, frequent urination (maturation), urinary incontinence, thinning of the skin,
breast atrophy and prolepses, increasing heart disease, and bone loss.
B.

Take the estrogen as prescribed by the doctor, and regularly come back to continue the
treatment at the clinic.
B.

Balance your nutrition, supplement estrogen, exercise sufficiently, and keep yourself
happy and optimistic. This will help you control the menopause symptoms and go
back to your normal life.
B.

47

Be careful not to eat food containing too much fat, cholesterol, salt, or alcohol. Eat more
plant proteins and food containing calcium, or high-fiber foods, and drink more water.
Situation 4

B.

You just had a myomectomy(fibroid removal surgery).[RPDT2] Dont carry heavy

objects, pick up your kids, or do heavy housework. These activities will prevent your
abdominal muscle from healing.
B.

Avoid doing activities which will increase pelvis congestion, [RPDT3]such as sitting for
a long time, or dancing. In addition, keep yourself happy, and get sufficient sleep.
B.

Your wound is healing little by little. You dont have to apply any dressing after the
stitches have been removed. You can change the surgical tape after taking a
bath/shower. Applying surgical tape for about half a year could decrease hyperplasia of
the scar. You dont need to change the surgical tape too often. Change it only after it
falls off.
B.

If you have a severe chill or a fever, your vagina bleeds a lot or for a long time, you faint,
or the wound gets red, swollen, painful, or has some abnormal secreta, come back to the
hospital immediately.

48

Pediatric Conversation

B.

Situation 1

I am going to take the babys temperature rectally. Could you please take off the babys
diaper?
B.
Your child has a slight fever. Give her/him this medicine as prescribed.
B.

Remember to give your child more fluids at home. Dont dress him/her too warmly and
dont cover him/her under heavy covers. Also, keep the indoor air circulating.
B.

Bath your child and change his/her clothes every day. Also, keep quiet when your baby is
taking a rest. Limit his/her movement if necessary.
B.

37 5
39

38 5

Bringing down your childs temperature takes several days. Please take his/her
temperature every four hours at home. If his/her axillary (armpit) temperature is over
37.5 C, lie him/her down on an ice pillow. If the childs temperature is under 39 C,
dont give him/her oral antipyretic; give him/her a suppository.
B.

If the child has convulsions, feels drowsy or lethargic, vomits seriously, or has cyanosis,
take him/her to the emergency room immediately.

Situation 2
49

B.

When your baby has diarrhea, you have to observe and keep record of the stool (egesta),
like how many times your baby has bowel movements a day, the volume of stool (egesta)
each time, and the color thereof. The best thing is to take the babys last stool (egesta)
with you when you take him/her to the doctor.
B.

1/2

You could make the formula lighter. If necessary, you can use the non-lactose formula to
avoid dyspepsia.
B.

When your baby gets a more severe diarrhea, dont feed him/her anything for four hours
to let his/her gastrointestinal rest.
B.

Even if the baby doesnt eat anything for a while, you still have to take care of the his/her
oral hygiene. Dip a cotton swab in lukewarm boiled water to clean the mouth cavity and
moisten his/her lips.
B.

Clean the babys buttocks after each bowel movement. Dip cotton swabs in lukewarm
water and clean his/her buttocks gently. Keep his/her buttocks dry to prevent diaper rash.
If your baby already has diaper rash, apply prescriptive ointment when the skin is dry.
B.

Keep quiet when your baby is resting and let him/her get enough rest and sleep to
increase his/her immunity.

B.

Situation 3

Its not easy for babies to cough up phlegm on their chests, so you should thump lightly
50

on his/her back to help him/her get it out.

B.

To do this, form your palm in a cup. Then, regularly thump lightly on the chest.
B.
To do this, form your palm in a cup. Lie the baby down on his/her back and keep the
head upward. Then, lightly thump the right and left sides of the chest down from the
shoulder. Then, turn the baby face down and, while still keeping the head upward,
thump the right and left sides of the back down from the shoulder.
B.

Afterwards, turn the baby face up and keep the head downward. Thump the chest from
the ribs to the shoulder. Then, turn the baby on one side and lift his/her arm. Thump the
babys body from the waist to the armpit. Lastly, turn him/her face down and keep the
head downward. Thump the right and left sides of the back from the waist to the
shoulder.
B.

Take off your watch and other jewelry, and make sure your nails are short to prevent your
baby from getting hurt while thumping his/her back. Also, dont do this directly after
meals. Do it before meals, or wait at least one hour after meals to keep your baby from
vomiting.
B.
Avoid thumping close to the important organs or areas where you can see skeletal
structure, such as the spine, the center of the chest, and the abdomen. Do these spots
lightly with your fingers instead.

B.

Situation 4

!
51

Remember to take your baby for his/her vaccine shots.

B.

There are three shots for the diphtheria-pertussis-tetanus vaccine [DPT vaccine] and are
given with two month intervals. The Oral Poliovirus Vaccine(OPV) must be taken
three times with two month intervals. After one year, your child should get a booster
shot. Before he/she goes to school, he/she should get another booster shot.
B.
The Bacillus Calmette-Guerin vaccine could be given on the second day after birth.
B.
The measles, mumps, and rubella vaccines [MMR vaccine] should be given when your
child is one year old.
B.

The meningococcal vaccine should be given when your child is one year old. Give the
meningococcal vaccine two times in the first year. Let him/her get the second shot two
weeks after the first shot. Then, let him/her get the booster shot every May or June.
B.

You can look up all the necessary information in the Maternal-Newborn Nursing Guide.
Please come and get the vaccination on schedule.

52

Dermatology Conversation
Situation 1

A.

What you got is urticaria, or nettle rash. This is a kind of allergic dermatosis. Therefore,
dont eat any seafood in the next two weeks. Also, dont eat food such as mangos or
nuts which may worsen your allergy.
B.

Is there anything I have to watch out for?

A.

Dont scratch when you feel itchy, as this will cause the skin to be totally scratched off.
Dont make the water too hot when you take a shower or a bath, because this will
make your skin itchier.
Situation 2

A.

The milk spots (milias) on your face are an allergic reaction that might be caused by too
much cold medicine which your body cannot metabolize. Stop all medicine you are
taking now. To recover fully, you have to apply the prescribed ointment regularly.
B.

How long will it last?

A.

At least one week.

B.

Gosh! It means I will look ugly for a whole week?

53

A.

Situation 3

Could you please tell me what I should watch out for after getting cosmetic laser
surgery?
B.

Keep the wound away from water for the first three days after treatment. You can use wet
towels to wipe your face. Use sterile cotton swabs with boiled water or hydrogen
peroxide to clean affected areas, and apply the ointment prescribed by the doctor at
least twice daily.
B.

After about three days, the wound will have formed a scab. After that, the wound may get
wet, but dont touch or scratch the scab.
B.

5~10

The scab will fall off in approximately five to ten days. At this time, the skin will look
deep red. If the wound does not heal, keep applying the ointment to avoid infection,
and dont wear any make-up during that time.
B.
Also, avoid sunshine and dont use sun tan lotion. Wear a hat or use a sunshade when you
go out for at least three months.
B.

If you feel pain, take some oral painkiller (analgesic) such as panadol.
B.

You will feel a scorching heat and some swelling for about one day after treatment. You
54

can use cold boiled water on gauze or a towel with ice tubes inside on the wound to ease
the pain.
Situation 4

A.

I keep getting acne. Its really annoying. How can I prevent it?
B.

Getting sufficient sleep is the best way. Dont stay up late at night, avoid emotional
situations or stress which may cause insomnia (sleeplessness), and get regular
exercise.
B.

Try to stay relaxed and happy, also when eating, and balance your diet. Reduce emotional
agitation or psychological stress. [RPDT4]
B.

If the situation gets worse, choose cosmetics, skin care products, and cleanser which are
suitable for acne. Wash your face once or twice daily. Wash up to three or four times if
needed.

Situation 5

A.

I get athlete's foot in summer. Is there anything I can do to prevent it ?

B.

Fungus grows easily in warm moist places. In modern society, people wear shoes all the
time. Because shoes do not breathe and feet sweat in summer, fungus grows easily in
the moist circumstances. Thats the reason why athlete's foot returns year after year for
55

many people. But, when the winter comes, feet dont sweat so much and become drier
in the cold weather, so athlete's foot seems to be cured without medicine.
B.

The best prevention is to maintain foot hygiene. Keep your feet dry to prevent fungus
from growing. Therefore, dry your feet and the gaps between toes thoroughly after
taking a shower or a bath or when your feet are wet for any other reason. Try to wear
shoes which can breathe or sandals.
B.

Also, wear clean absorbent socks made of natural fibers instead of nylon or silk. Change
them every day and alternate your shoes. Dont walk barefoot, especially in public
bathrooms, to prevent the spread of the fungus and the possibility of infection.
B.

Use the ointment [RPDT5]prescribed by your doctor regularly. Dont stop using the
ointment before the infection has cleared up totally, as your athletes foot wont be cured
completely. That will also cause drug resistance.

Orthopedic Surgery Conversation

Situation 1

B.

[RPDT6]

You have to use a crutch or walking stick while your leg is in plaster. Also make sure of
the following.
B.

Use only a walking stick with a rubber to prevent slipping. Make sure the rubber is
secure.
B.

Wear pants with proper length and non-slip shoes while walking.
B.

10

10

When standing, put the bottom of the walking stick/crutch ten centimeters in front of
your toes and ten centimeters outward.
B.

Use your arm to support your body when walking. Place the top of the crutch two fingers
away from the armpit in order to avoid pressing the nerves in your armpit (nerves
axillaries). Start to walk slowly, then increase your speed gradually.
B.

Keep your floors dry. Move items in the house that can cause an obstruction out of the
way and keep your hallway clean to prevent slipping and stumbling. If you slip and
fall, it will influence your recuperation.
B.

As the injury heals, you will fell extremely itchy inside the plaster casing. Dont scratch
61

it with any sharpened objects as this may infect the wound again.

Situation 2

B.

Your diet is too rich. Your uric acid is too high. You should start to control your diet.
B.

Be careful with your diet. Dont eat big meals and dont drink liquor. Reduce seafood,
meat and animal intestines. You should eat low purine food, such as eggs, milk and
other dairy products, sea cucumbers, rice, noodles, fruit, and jelly.
B.

Also drink more water to help discharge uric acid. If you drink any liquor, dont drink too
much because it reduces the discharge of uric acid.
B.

Also, remember to come back to the hospital to examine the uric acid consistency in your
blood regularly. Keep your uric acid at a normal level to prevent disease.
B.

If you feel a sudden severe pain in your joints, it means you have an acute level of uric
acid. You can elevate the pain by applying an ice pack. Take the prescribed medicine
till the symptoms have gone, but stop if you get diarrhea.

B.

Situation 3

You drink too much coffee. Take calcium supplements to prevent osteoporosis.
B.

Drink more milk, and eat more green vegetables and beans. Both dried fish and dried
62

shrimp contain sufficient calcium.

B.

Exercise regularly and get some sunshine. This will help to absorb the calcium.
B.

Furthermore, quit unhealthy habits, such as drinking large amounts of coffee.

B.

You can supplement your calcium levels with calcium tablets or Vitamin D .
B.

Another important thing to remember is that osteoporosis does not only affect the old. It
is good to do a bone density examination regularly while youre young to prevent
osteoporosis.

B.

Situation 4

If you move objects, take care that you dont sprain your ankle.
B.
